What is Bi Ba used for in Chinese medicine?
Bi Ba — long pepper — is a spicy, hot interior-warming herb for cold in the middle burner: cold epigastric and abdominal pain, poor appetite, nausea, vomiting, and cold-type diarrhoea or dysentery. Our record gives it as spicy and hot, entering the Stomach and Large Intestine channels, in the category of herbs that warm the interior and expel cold. It is a kitchen spice as well as a medicinal, which is how most Western readers will first meet it.
Its traditional actions:
- Disperses cold, warms the middle burner and relieves pain — for cold in the Spleen, Stomach and Intestines: abdominal and epigastric pain, poor appetite, nausea, vomiting, diarrhoea and dysentery. This is the core use and covers most of what the herb is prescribed for.
- Applied topically for toothache — a distinctive traditional application recorded on our own entry: the powdered fruit is placed on or near the painful tooth. This is a well-known local use for the acrid, warming peppers generally.
- Directs downward and stops vomiting — the warming, descending quality is what makes it suitable for cold-type nausea and vomiting rather than simply for cold pain.
Character of use: hot, drying and pungent, and used in small amounts. Like the rest of this category it is a short-course herb for a clear cold picture, not something to take continuously.

What is the typical dosage of Bi Ba?
2–5 g in decoction — a deliberately small dose, because the herb is hot and acrid. That is the range our record carries. It sits at the low end of the warming category for the same reason black pepper does: a little goes a long way, and more is not better.
- 2–5 g in decoction, per our own record. Some references cite a slightly wider band; the low, conservative figure is the one we hold.
- Ground into powder for pills and powders, which is how it appears in most classical preparations rather than in a plain decoction.
- Topically — powdered and applied locally for toothache, as recorded on our own entry.
- As food — long pepper is a culinary spice in South and Southeast Asia and, historically, in Europe. Culinary quantities are far below the therapeutic range, and nothing in this record is about seasoning food.

Are there cautions for Bi Ba?
Do not use where there is heat — whether excess heat or heat from yin deficiency. That is the caution our record carries, and it follows directly from the herb being hot and drying. Given to someone who is already hot or dry, a hot acrid herb makes the picture worse.
- Excess heat or deficient (yin-deficiency) heat — contraindicated, per our own entry.
- Yin deficiency and dryness generally — the herb is drying as well as hot; long courses are inadvisable where fluids are already depleted.
- Existing stomach irritation — pungent peppers can themselves provoke burning, reflux or loose stools above modest doses, which is the opposite of the intended effect. This is the practical reason for the small recorded dose.
- Pregnancy — hot, acrid, strongly moving herbs are used in pregnancy only under a qualified practitioner’s supervision, and traditional practice is cautious with the pungent warming group as a whole.
- Prescription medication — the one caution here with real modern substance. See the drug-interaction section: the herb’s main alkaloid, piperine, is a documented inhibitor of major drug-handling pathways, and that deserves specific awareness rather than a general disclaimer.

Chinese Herb Toxicity & Overdose
Low at culinary and standard herbal doses; our record does not flag Bi Ba as toxic, and long pepper is an ordinary food spice across South and Southeast Asia. The realistic adverse effect at higher doses is gastrointestinal irritation — burning, reflux or loose stools — which is a dose-dependent nuisance rather than a toxic reaction, and is the reason the recorded dose range stops at 5 g.
The honest gap is that formal toxicology on the herb is thin. A PRISMA-based systematic review of Piper longum catalogues a broad preclinical pharmacology and then states directly that it “is also essential and critical to generate toxicological data and pharmacokinetics on human subjects” for this plant (PMID 31568819). Reported safety concerns in the wider literature attach to concentrated piperine preparations at doses well above what a decoction or a meal supplies, not to the fruit in traditional use. Absence of reported harm at food-like intakes is not the same as demonstrated safety for concentrated extracts taken daily.

Herb-Drug Interactions
Does Bi Ba interact with medications?
This is the one herb in the warming category where the drug-interaction question has a genuinely well-documented mechanistic answer. Piperine, the principal alkaloid of long pepper and of black pepper, is an established inhibitor of CYP3A4 and of intestinal P-glycoprotein — two of the main systems that limit how much of an oral drug reaches the bloodstream. Inhibiting them raises the exposure to drugs that depend on them, which is why piperine is deliberately used in pharmaceuticals as a bioavailability enhancer.
- The core finding. A 2002 study characterised piperine as an inhibitor of human P-glycoprotein and CYP3A4 — the paper that established the mechanism (PMID 12130727).
- It is used on purpose. Piperine is a licensed component of a fixed-dose anti-tuberculosis product in India that combines a reduced 200 mg dose of rifampicin with isoniazid and 10 mg of piperine, specifically so that less rifampicin achieves comparable blood levels. In a randomised, controlled phase III trial in 216 newly diagnosed pulmonary tuberculosis patients it performed comparably to the standard regimen (PMID 29313577). This is a demonstration, in humans, that piperine changes how a prescription drug behaves.
- The size of the effect can be large. In healthy human volunteers, 20 mg of piperine taken alongside 2 g of curcumin raised curcumin bioavailability by around 2000% — a twenty-fold increase — where curcumin alone gave serum levels that were mostly undetectable (PMID 9619120). This is the most-cited example, and it is why piperine appears in so many turmeric supplements.
- It cuts both ways. Piperine also activates the human pregnane X receptor, which induces CYP3A4 and the MDR1 transporter — the opposite direction from acute inhibition. The authors of that work concluded that these dichotomous effects “challenge the potential use of piperine as a bioavailability enhancer” and that caution is warranted during drug treatment, particularly for people who use pepper spice heavily or rely on pepper-based remedies (PMID 23707768, PMC 3775940). Acute inhibition and longer-term induction can pull drug levels in opposite directions.
The proportionate reading. Nearly all of this evidence concerns isolated, concentrated piperine at doses of roughly 10–20 mg of the pure alkaloid, not a 2–5 g decoction of the whole fruit inside a multi-herb formula, and no clinical interaction study of Bi Ba as a decocted herb was located. The mechanism is real, well replicated and clinically exploited; the extrapolation from purified alkaloid to traditional decoction is not established — the same distinction this site drew between the licensed mulberry-twig alkaloid tablet and decocted Sang Zhi, and between standardised andrographis extracts and decocted Chuan Xin Lian.
Where it matters most: anyone taking a narrow-therapeutic-index medicine — anticonvulsants, immunosuppressants, certain cardiovascular and antiretroviral drugs, or anything the prescriber has warned not to take with grapefruit (also a CYP3A4 issue) — should tell both their prescriber and their practitioner about pepper-family herbs and about concentrated piperine or “bioavailability-enhanced” supplements. Is there modern research on Bi Ba (Piper longum, long pepper)?
Yes — a large preclinical literature built around two alkaloids, piperine and piperlongumine, plus the human pharmacokinetic work described in the interactions section. What is largely missing is clinical research on the herb itself for its traditional digestive indications.
- The systematic picture. A PRISMA-based systematic review identified piperine and piperlongumine as the major secondary metabolites alongside essential oil, flavonoids and steroids, and catalogued anti-inflammatory, analgesic, antioxidant, antimicrobial, anticancer, antidiabetic, hepatoprotective, antiplatelet, immunomodulatory, anti-arthritic, anti-ulcer, anti-asthmatic, anthelmintic and anti-amoebic activities in laboratory models. Its conclusion is measured: bioscientific support is clearest for the anti-amoebic, anthelmintic, antitumour and antidiabetic activities, while traditional claims for insomnia, dementia, epilepsy, rheumatoid arthritis and asthma remain insufficiently validated (PMID 31568819). A 2022 comprehensive review covers the same ground in more detail (PMID 36256521).
- Piperine as a pharmacological tool. The best-established modern facts about this plant are about drug handling rather than about treating disease: inhibition of CYP3A4 and P-glycoprotein (PMID 12130727), activation of the pregnane X receptor with induction of CYP3A4 and MDR1 (PMID 23707768), a twenty-fold increase in curcumin bioavailability in human volunteers (PMID 9619120), and a phase III tuberculosis trial of a piperine-boosted rifampicin formulation (PMID 29313577). A 2022 review of piperine notes 11 completed and 5 ongoing human clinical trials across conditions including osteoarthritis, fatty liver disease and metabolic syndrome, typically at low doses of the isolated alkaloid and often combined with curcumin (PMID 35127957, PMC 8796742).
- Piperlongumine. The other signature alkaloid, largely absent from black pepper, has become a widely studied anticancer lead compound in its own right, with an extensive derivative-synthesis literature (PMID 38593312, PMID 32283222). This is drug-discovery chemistry, entirely separate from any traditional use of the herb.
- Anti-inflammatory chemistry. Amide alkaloids and sesquiterpenes isolated from the fruits show anti-inflammatory activity in cell models (PMID 39149874, PMID 39413978, PMID 40803451), and a water extract of the herb reduced markers in a gout model through MAPK and PI3K–AKT signalling (PMID 38670409). Preclinical only.
- The traditional digestive indication. Older work found long-pepper fruit active against caecal amoebiasis in mice (PMID 15120461), and a herbal preparation built on long pepper was studied clinically for giardiasis (PMID 9201614) — the closest the literature comes to the traditional intestinal use, and both are small and dated.
Human evidence for the whole herb: limited. There are no modern controlled trials of Bi Ba as a single decocted herb for cold epigastric or abdominal pain. The substantial human data on this plant concerns the isolated alkaloid used as a pharmaceutical adjuvant, which is a different question entirely.

What plant is Bi Ba, and is it a real pepper?
Yes — unlike Szechuan pepper, Bi Ba is a genuine Piper. It is the dried, near-ripe fruiting spike of Piper longum L. (Piperaceae), the long pepper of India and Southeast Asia, called pippali in Ayurveda. The “fruit” is not a single berry but a small dense catkin-like spike of many tiny fused fruits, which is why it looks like a miniature dark cone rather than a peppercorn. Long pepper was the dominant pepper of the classical Mediterranean world and was gradually displaced in European kitchens by black pepper.
The pepper family tree, as this site holds it. Four herbs in our records carry “pepper” in their English or Chinese names, and only two of them are peppers:
- Bi Ba (荡茲, Piper longum, Piperaceae) — a true Piper. Spicy, hot; Stomach and Large Intestine.
- Hu Jiao (胡椒, Piper nigrum, Piperaceae) — black pepper, the closest relative and its nearest analogue in use. Also spicy and hot, also Stomach and Large Intestine. The two are near-interchangeable in character; Bi Ba is generally regarded as the stronger pain-relieving of the pair, and Hu Jiao the more food-like.
- Chuan Jiao (川椒, Zanthoxylum bungeanum, Rutaceae) — Szechuan pepper, and not a pepper at all: it is a citrus relative. It shares the middle-burner-warming indication but adds the Kidney channel, the parasite-expelling action, a slightly-toxic classification, and the numbing má sensation that no true Piper produces. - Bi Cheng Qie (荡澄茄) — a near-identical pinyin to Bi Ba and a genuine trap. Our record gives it as Fructus Litseae — Litsea cubeba, in the laurel family. Historically the name referred to cubeb, Piper cubeba, a true pepper; the two have been used under the same name in different places. A chemical study of the problem states it plainly: “In China, Litsea cubeba (Lauraceae) has been designated as the cubeb tree; in other countries, Piper cubeba (Piperaceae) is considered as the cubeb,” and all commercial cubeb samples the authors tested from one market proved to be Litsea (PMID 25212371).
A 2025 review of the spices sold as “pepper” makes the same point from the food side, listing black pepper, cubeb pepper, long pepper, pink pepper and Japanese pepper as distinct botanical species grouped under one culinary word (PMID 40363698, PMC 12073208). The practical lesson: when comparing sources, check the Latin binomial, not the word “pepper.”
Which formula contains Bi Ba?
In our own formula records Bi Ba appears in Su He Xiang Wan (Liquid Styrax Pill, from the Imperial Grace Formulary of the Tai Ping Era), at 60 g of the batch, for cold-type closed disorder — cold-phlegm misting the Heart and blocking the orifices. Its role there is warming and aromatic, opening a cold obstruction alongside the aromatic resins that give the formula its name.
