What is Da Suan (garlic) used for in Chinese medicine?
Da Suan (garlic bulb) is a food-herb classed among the “herbs that expel parasites” — it kills intestinal parasites, resolves toxicity and reduces swelling of early-stage abscesses and sores, stops dysentery and diarrhea, and warms the Spleen and Stomach to move cold-type digestive stagnation. It is spicy (acrid) and warm, entering the Large Intestine, Lung, Spleen, and Stomach channels.
Its traditional actions:
- Kills parasites, expels worms — used against intestinal parasites including hookworm (ancylostomiasis) and pinworm (enterobiasis); raw garlic, especially single-clove (“solo”) garlic, is a classic first-line food-remedy in this category.
- Resolves toxicity, reduces swelling — mashed into a paste and applied topically to the early, pre-suppurative stage of abscesses, sores, and boils; taken internally for toxin-heat presentations. A related external technique, indirect (“garlic”) moxibustion — burning moxa on a slice of garlic placed over a nodule, scrofula, or early sore — draws on this same toxin-resolving, swelling-reducing action.

What is the typical dosage of Da Suan (garlic)?
Commonly 5–10 g per day, eaten raw, cooked into congee, or taken as a decoction or syrup; as a topical paste it is applied briefly and never left on the skin for long, since it can burn or blister (see Cautions). Because it is a food-herb, garlic is also simply eaten as part of the diet outside any formal dosing.
Traditional folk preparations described in the literature include garlic porridge combined with Bai Ji (Rhizoma Bletillae) for TB-pattern coughing of blood, and a soaked, filtered garlic-water preparation historically described for whooping cough. These are documented as traditional/folk practice, not treatment instructions.

Chinese Herb Contraindications & Cautions
Are there cautions for Da Suan (garlic)?
Raw garlic is irritating to mucous membranes and the digestive tract — large or raw doses commonly cause a hot/burning sensation in the mouth or stomach, heartburn, gas, nausea, or diarrhea, especially on an empty stomach; those with existing Stomach/digestive irritation, ulcers, or Yin-deficient heat patterns should use it cautiously or avoid raw preparations.
- Topical irritation (important) — applying crushed or mashed garlic directly to the skin, especially under a bandage/occlusion or for more than a short period, can cause chemical burns, blistering, and irritant contact dermatitis; case reports describe painful vesiculation and lasting hyperpigmentation from unsupervised garlic poultices. Any topical use should be brief, diluted, and discontinued at the first sign of burning.
- Excess-heat patterns — not favored in Excess-Heat / Fire presentations or where significant Stomach or Yin-deficient heat is already present, per the general caution against acrid-warm herbs in such patterns.

Chinese Herb Toxicity & Overdose
Low, as a food, at ordinary dietary and medicinal doses. Garlic has a long history of safe dietary and medicinal use in humans; oral use in research has been sustained for years without serious toxicity. The main real-world risks are gastrointestinal irritation (heartburn, reflux, nausea) at high or raw doses, and skin/mucosal burns from topical or occlusive application (see Cautions) — not systemic poisoning.

Herb-Drug Interactions
Garlic has documented antiplatelet activity and may increase bleeding risk with anticoagulant or antiplatelet drugs (e.g., warfarin) — case reports describe elevated INR and bruising/bleeding with heavy garlic intake, though one small placebo-controlled pilot study of aged garlic extract in warfarin patients found no increase in hemorrhagic events. Separately, a controlled pharmacokinetic study found that garlic supplements reduced blood levels of the HIV protease inhibitor saquinavir by roughly half, attributed to induction of intestinal drug-metabolizing enzymes — a mechanism that raises a plausible caution for other CYP450-metabolized medications generally. Is there modern research on Da Suan (garlic, Allium sativum)?
Yes — garlic is one of the most heavily studied food-herbs, with strong laboratory/mechanistic support for antimicrobial activity and a much more mixed clinical picture for cardiovascular effects. Its key bioactive compound, allicin, is formed from the odorless precursor alliin by the enzyme alliinase when garlic tissue is crushed or cut.
- Antimicrobial (mechanistic/lab) — allicin shows broad antibacterial, antifungal, antiparasitic, and antiviral activity in laboratory studies, acting via reaction with thiol groups on microbial enzymes; a 2017 study found allicin vapor killed multidrug-resistant lung pathogens including Pseudomonas, Streptococcus, and Staphylococcus strains, and a 2021 review reported antibacterial synergy with conventional antibiotics against emerging multidrug-resistant bacteria. Laboratory evidence, supportive of but not proof of clinical efficacy.
- Human clinical (infection) — a randomized controlled trial in bacterial vaginosis found garlic tablets performed comparably to oral metronidazole; a useful clinical data point, but a single indication, not general proof of antibiotic-equivalence.
- Blood pressure / lipids (mixed) — meta-analyses report a modest lowering of blood pressure (mainly diastolic) and some reduction in dyslipidemia/cholesterol risk with regular garlic intake, but other placebo-controlled trials and an earlier meta-analysis found no significant lipid or blood-pressure benefit; overall the evidence is inconsistent and appears dose/preparation-dependent (raw vs. aged vs. powdered extract).
- Metabolic syndrome — a systematic review and meta-analysis of supplementation trials found some favorable effects on metabolic-syndrome components but no clear consensus across studies.
Note: Clinical trial results for garlic’s cardiovascular effects are genuinely inconsistent across the literature — not a naming/variant issue but a real mixed evidence base, likely driven by non-equivalent preparations (raw, aged, powdered, and oil extracts differ pharmacologically). We report this as mixed rather than picking a side.

How is Da Suan different from other parasite-expelling herbs like Bing Lang and Shi Jun Zi?
All three are classed among the “herbs that expel parasites,” but Da Suan is the only one that is also a common food, used topically as well as internally, and valued for resolving toxicity and reducing swelling — not only for killing worms.
- Bing Lang (Betel Nut) — bitter, spicy, warm; a stronger, more purgative anthelmintic used specifically to expel tapeworm and roundworm and to move qi and reduce food stagnation; not used topically.
- Shi Jun Zi (Rangoon Creeper Fruit) — sweet, warm; the classic pediatric roundworm/pinworm herb, gentler for children but associated with hiccup and dizziness if taken to excess or with hot tea.
- Da Suan — spicy, warm; distinguished by its dual internal/external use, its toxin-resolving and swelling-reducing action on early abscesses and sores, its use for dysentery/diarrhea, and its role warming the Spleen and Stomach — a broader, food-grade profile rather than a dedicated anthelmintic.
Traditional combinations include garlic porridge with Bai Ji for TB-pattern blood-streaked cough, and “garlic moxibustion” (moxa burned on a garlic slice) over nodules, scrofula, insect bites, and early-stage sores — using garlic as a toxin-resolving medium for the moxa’s heat rather than in a decoction.
