What is Jiu Da Huang (wine-treated rhubarb) used for in Chinese medicine?

Jiu Da Huang is not a different herb from Da Huang — it is rhubarb root and rhizome that has been briefly stir-fried with wine, and the processing is the entire point of the page. Wine treatment moderates the herb's violent downward purgation and turns its heat-clearing action upward, making it the form the tradition reaches for when the heat is in the upper body rather than jammed in the intestines. Our record states the position exactly: its purgative strength sits between the raw herb and the steamed herb. It keeps the parent herb's bitter, cold nature and its Heart, Large Intestine, Liver and Stomach channels.

  • Clears heat in the upper body — red, swollen eyes, sore or swollen throat, mouth and gum sores, headache from fire rising. This is the classical rationale for wine treatment: wine is warm and ascending, and carries the cold, sinking herb upward against its own tendency.
  • Moderated purgation — still a purgative, but a gentler one than the raw herb. Where the raw material is used to force stagnation out of the intestines, this form is chosen where that force is not wanted.
  • Invigorates blood and moves stagnation — the parent herb's blood-moving action is emphasised rather than diminished by wine treatment; our Da Huang record notes that using it in wine or vinegar increases the blood-invigorating properties.

  • What is the typical dosage of Jiu Da Huang?

    Our record carries no separate dosage figure for the wine-treated form; the range held on our Da Huang entry is roughly 3–12 g in decoction. In practice the wine-treated material is most often dispensed as a component of a prescribed formula rather than dosed on its own, and the formulas on this site that specify it give it in the ordinary range for the parent herb.

    • Preparation is what actually differs — our Da Huang entry records that raw rhubarb is used as a purgative and cooked for less than ten minutes, because prolonged cooking breaks down the sennoside and anthraquinone compounds that drive the laxative effect. Once the herb has been wine-treated, the strongest purgative fraction has already been partly altered, which is why this form is not the one reached for when vigorous purgation is the aim.
    • Grade is not a detail — raw, wine-treated, wine-steamed and charred rhubarb are four distinct pharmacy items with different strengths. A dose written for one is not a dose for another, and a dispensing label naming the processing is doing real work.

Chinese Herb Contraindications & Cautions

  • Are there cautions for Jiu Da Huang (wine-treated rhubarb)?

    Yes — the parent herb's cautions carry over in full, and moderating a purgative does not make it safe for the people it was contraindicated in. Our Da Huang entry records the herb as contraindicated during menstruation, pregnancy, postpartum and while nursing, given its cold, downward-draining, blood-moving nature. It is also recorded as contraindicated in exterior patterns, in cold patterns from Spleen or Stomach deficiency, and where there is underlying qi and blood deficiency.

    • Pregnancy, nursing, menstruation and postpartum — contraindicated. The blood-invigorating action is if anything emphasised rather than reduced by wine treatment, so this caution is not relaxed by the processing.
    • Spleen and Stomach deficiency-cold, and deficiency without excess — avoid. The herb remains bitter and cold.
    • Strength is preparation-dependent (reference note) — raw rhubarb is considerably stronger as a purgative than this form, and the wine-steamed and charred forms are weaker again. Substituting one grade for another changes what the prescription does.
    • Long-term use (reference note) — chronic use of anthraquinone-containing laxatives, rhubarb included, is documented to cause melanosis coli, a dark pigmentation of the colon lining. A 2023 review reports this is reversible within roughly six to twelve months of stopping, and is not clearly linked to a higher rate of colorectal-cancer diagnosis, though it is associated with more polyps being found at colonoscopy (PMID 37214441; PMC 10193150; DOI 10.3389/fphar.2023.1160480).
    • Alcohol content — the wine used in processing is driven off by stir-frying, and this is not a tincture; the processing name describes how the herb was treated, not what a patient receives. Recorded here because the name reasonably prompts the question.

Chinese Herb Toxicity & Overdose

  • Is wine-treated rhubarb toxic?

    Low acute toxicity at standard traditional doses, with the documented risk attaching to chronic or high-dose anthraquinone exposure rather than to the wine treatment. Rhubarb's anthraquinones — rhein, emodin, aloe-emodin, chrysophanol, physcion — and their sennoside precursors are generally well tolerated short-term. A 90-day rat feeding study identified rhein as the metabolite that accumulates most in tissue and drives apoptosis and autophagy in colonic cells, proposing it as the mechanism behind chronic anthraquinone-laxative toxicity (PMID 32590269; DOI 10.1016/j.foodchem.2020.127363).

    • What processing changes — a 2024 review reports that wine-frying and repeated steaming measurably lower the hepatorenal anthraquinone and tannin burden relative to the raw herb (PMID 39170703; PMC 11335691). Processing therefore moves this form in the safer direction relative to raw rhubarb, which is a genuine finding and not a claim that the form is risk-free.
    • Where the anthraquinones go — a rat tissue-distribution study found wine processing significantly changed where aloe-emodin, rhein and emodin end up, with increased distribution in heart and lung tissue, no significant change in liver or kidney, and none of the three detectable in brain (PMID 29071869; DOI 10.19540/j.cnki.cjcmm.20170224.014). The authors present this as an experimental basis for the traditional claim that wine processing directs the herb upward.

  • Does wine-treated rhubarb interact with medications?

    This is one of the few processed herbs with a directly relevant animal interaction study, and it found real effects. A rat study using a probe-cocktail and pharmacokinetic approach reported that wine-processed rhubarb induced CYP1A2 activity, that both wine-processed and cooked rhubarb inhibited CYP2C6 and induced CYP2E1, and that both processed forms reduced the absorption and bioavailability of the antidiabetic drug saxagliptin while increasing its clearance. The authors concluded that drugs co-administered with processed rhubarb may need dose adjustment (PMID 23207062; DOI 10.1016/j.jep.2012.11.030).

    • This is an animal study, not a human trial. Rat cytochrome P450 isoforms are not a direct map onto human ones, and no clinical interaction trial of this herb could be located. It is reported here because it is specific to the processed material rather than borrowed from the plant class.
    • Class-level considerations — anthraquinone and sennoside laxatives can contribute to potassium loss with chronic or excessive use, which may compound potassium-wasting diuretics and increase sensitivity to cardiac glycosides such as digoxin; accelerated intestinal transit can also shorten the absorption window for other oral medicines. These are class-level pharmacological considerations, not findings for this form.

  • Is there modern research on Jiu Da Huang (wine-processed Rheum root and rhizome)?

    Yes — and unusually for a processed herb, several studies test the processing claim itself rather than the plant in general. The traditional position is that wine treatment moderates purgation and directs the herb's heat-clearing action upward. Modern work has approached that claim from three directions — how the material changes, where its compounds go in the body, and what it does in disease models — and has broadly supported it, within the limits of small animal studies.

    • Does wine processing really redirect the herb upward? A rat tissue-distribution study measured aloe-emodin, rhein and emodin after dosing with either raw or wine-processed rhubarb decoction, and found that wine processing significantly increased their distribution in heart and lung tissue while leaving liver and kidney levels essentially unchanged. The authors present this explicitly as experimental support for the traditional processing theory (PMID 29071869; DOI 10.19540/j.cnki.cjcmm.20170224.014).
    • Does it help upper-body heat patterns? A comparative animal study of crude versus wine-processed rhubarb in pneumonia and mouth-ulcer models reported that the wine-processed material significantly lowered inflammation scores, promoted repair of the ulcer lesions, normalised white-cell and neutrophil counts, lowered TNF-α and reduced inflammatory exudation in lung tissue relative to the crude herb (PMID 26214870). Small, model-based work, but directly on the point.
    • Is the purgation really milder? A serum pharmacochemistry study describes crude and wine-processed rhubarb plainly as drastic and mild laxatives respectively, and identified sixteen absorbed compounds and metabolites — mainly phase II metabolites — differing between the two preparations in normal and blood-stasis rats (PMID 24981550; DOI 10.1002/jssc.201400473).
    • What changes in the material — a 2021 review traces the processing history and the resulting shifts in anthraquinones, stilbenes, phenylbutanones, flavonoids and tannins across the four pharmacopoeial preparations (PMID 33645018), and a 2024 review correlates degree of processing with purgative strength and with hepatorenal burden (PMID 39170703; PMC 11335691; DOI 10.3389/fphar.2024.1442297).
    • Other lines — affinity chromatography using an immobilised cysteinyl leukotriene receptor was used to identify anti-inflammatory constituents specifically in wine-processed rhubarb (PMID 38395001), and a proteomics and metabolomics study compared raw against wine-processed rhubarb in a rat intracerebral haemorrhage model (PMID 33490182; PMC 7812229; DOI 10.21037/atm-20-7831). Both are preclinical.

    Honest summary: the processing claim — milder purgation, action turned upward — has more direct experimental support than most traditional processing rationales do. What it does not have is human clinical evidence: every study above is an animal or laboratory model, and none of it establishes a treatment effect in people.

  • What is the difference between raw, wine-treated, steamed and charred rhubarb?

    They are four pharmacopoeial preparations of one root, and each one moves the herb's action along the same axis: the more processing, the less purgation and the more emphasis on blood and on heat elsewhere in the body. A 2021 review of the processing history of this herb records that the Chinese Pharmacopoeia presently carries exactly four methods — cleaning (raw), stir-frying with wine, wine-steaming, and charring — and traces how each affects the herb's chemistry and its recorded actions (PMID 33645018).

    • Raw — the strongest purgative, with the highest sennoside and combined-anthraquinone content; traditionally added late to a decoction to preserve that strength.
    • Wine stir-fried (this page) — brief stir-frying with yellow wine. Purgation is moderated and the heat-clearing action is redirected upward. Our record places its purgative strength between raw and steamed, which is the standard position.
    • Wine-steamed — prolonged, repeated wine-steaming. Purgative strength falls substantially while free anthraquinones and gallic acid rise, emphasising blood-invigoration; a 2024 review reports this form also carries a measurably lower hepatorenal anthraquinone and tannin burden than the raw herb (PMID 39170703; PMC 11335691).

    Two: Ba Zheng San (Eight Herb Powder for Rectification) and Fang Feng Tong Sheng San (Ledebouriella Powder that Sagely Unblocks).
