What is Xi Xian Cao used for in Chinese medicine?
Xi Xian Cao is a bitter, cold wind-damp herb used above all for painful obstruction (bi) with heat — aching, stiff, numb or weak joints and limbs — and, because of its cold and detoxifying character, for damp-heat skin conditions with itching. It enters the Kidney and Liver channels and is classified in the Sacred Lotus among the Herbs that Dispel Wind-Dampness. It is one of the few herbs in that category that is genuinely cold, which is what gives it its particular niche.
Its main traditional actions:
- Dispels wind-damp and opens the channels — for bi syndrome: pain, numbness, spasm and paralysis in the joints, trunk and extremities; also for weak legs and post-stroke weakness of the limbs. This is its principal and most consistently cited use across sources.
- Clears heat and toxin, stops itching — for rashes, sores and generalised itching arising from damp-heat or wind-damp, and for headache and dizziness attributed to Liver yang rising.
- Calms the spirit / settles Liver yang — the Sacred Lotus record notes its traditional use where heat-type patterns produce hypertension-like presentations, consistent with its Liver-channel entry.
Beyond China, the dried aerial parts are also used medicinally in Japan, Korea and Vietnam, and the herb has been in the Chinese materia medica since the Tang dynasty.

What is the typical dosage of Xi Xian Cao?
Reference materia medica place Xi Xian Cao at roughly 9–15 grams per day in decoction, the range the Sacred Lotus records from its two cited authorities (10–15 g in one, 9–15 g in the other). It is a conventional, non-restricted herb dosed in ordinary decoction quantities.
- Which preparation is dosed matters — because the raw and wine-processed forms are traditionally held to have different actions (see the section below), the form specified in a prescription is as consequential as the number of grams.
- Larger amounts in external use — the herb also has a documented history of topical application for itching and sores, where quantity conventions differ from internal decoction.

Are there cautions for Xi Xian Cao? Is it safe?
Xi Xian Cao is a low-risk herb by materia medica standards — the Sacred Lotus records no toxicity for it — but caution is advised in pregnancy and in children, and its cold nature makes it a poor fit for cold or deficient patterns.
- Pregnancy and children — use with caution, per the Sacred Lotus record. This is a precautionary rather than absolute contraindication.
- Cold or deficient patterns — as a bitter, cold herb, prolonged or high-dose use is generally inappropriate where there is Spleen/Stomach cold or marked deficiency, and the raw form is the colder of the two preparations.
- Evidence gap on safety — the review literature explicitly notes that few experimental studies have addressed the toxicity and quantitative quality control of the three source species, and recommends further toxicity work. Absence of reported toxicity is therefore partly an absence of study, and is reported here as such.

Is there modern research on Xi Xian Cao (Herba Siegesbeckiae)?
Yes — the research is dominated by anti-inflammatory pharmacology, and by one compound in particular: kirenol, a diterpenoid that serves both as the quality marker for the herb and as its most-studied active constituent. Over three hundred phytochemicals have been reported from Siegesbeckiae Herba, including diterpenoids, sesquiterpenoids, flavonoids, oxylipins and lignoids.
- Kirenol — a dedicated review covers its isolation, chemistry and reported bioactivities, with anti-inflammatory and anti-arthritic effects the most developed themes. Its role as pharmacopoeial marker means kirenol content is also a quality-control measure, not only a pharmacological one.
- Anti-inflammatory and analgesic mechanism — a 2021 study identified kirenol, darutoside and hesperidin in an ethanol extract of S. pubescens and showed, across four animal models, reduced oedema, raised thermal pain threshold, reduced COX-2 overexpression in skin tissue and dose-dependent inhibition of inflammatory-cell migration. This aligns closely with the herb’s traditional wind-damp bi indication, though it remains preclinical.
- Breadth of reported activity — review coverage lists anti-inflammatory, antitumour, antiallergic, antioxidant, antithrombotic and antibacterial activities, plus protective effects against myocardial and cerebral ischaemic injury in models. A 2026 scientometric analysis maps 44 years of research (1980–2024) and finds “anti-inflammation,” “rheumatic arthritis” and NF-κB among the dominant keyword clusters, with China, the United States and Korea the leading contributors.
- Species and quality control — DNA barcoding combined with mass spectrometry can separate the three official species, and has identified species-specific marker compounds including previously unreported malonylkirenol derivatives.

What is the difference between raw and wine-processed Xi Xian Cao — and which plant species is it?
Two things are commonly omitted from short accounts of this herb, and both change how it is used. First, processing genuinely reverses its emphasis. Second, “Xi Xian Cao” is not one plant but three.
- Raw (unprocessed) Xi Xian Cao — the cold, draining aspect dominates. In this form the herb is used to clear heat, resolve toxicity, drain damp-heat and stop itching: damp-heat skin lesions, sores, rashes, and heat-type presentations including Liver-yang headache and dizziness. This is the form consistent with the bitter/cold nature recorded in the Sacred Lotus.
- Wine-processed (wine-steamed) Xi Xian Cao — processing with wine is traditionally held to moderate the cold nature and turn the herb toward tonifying the Liver and Kidney, strengthening the sinews and bones, and treating chronic bi with weakness and aching of the lower back and knees. The classical indication of “rheumatic arthralgia with aching and weakness of loins and knees, and numbness of limbs” belongs to this tonifying reading of the herb.
- Practical consequence — using the raw form where the processed form is intended (or the reverse) means using an essentially different medicinal. This is a real and frequently overlooked distinction, not a nuance.
- Three official species — the Chinese Pharmacopoeia recognises Siegesbeckia orientalis L., S. pubescens Makino and S. glabrescens Makino as sources of Siegesbeckiae Herba. All three are official; the material on the market may be any of them, and the review literature notes that distinguishing the decoction pieces remains difficult without molecular or chromatographic methods.
Note on species and marker compound: the three official species are not chemically identical. A 2023 study combining DNA barcoding with high-resolution mass spectrometry across 26 batches found that none of the S. orientalis batches examined contained kirenol — the diterpenoid used as the pharmacopoeial quality marker — and the authors questioned whether kirenol is an appropriate marker for that species. Sources therefore genuinely differ on how interchangeable the three botanical origins are, and we report the conflict rather than resolving it.
Note on the Chinese characters: the Sacred Lotus records this entry as 希莶草, while the Pharmacopoeia and the research literature use 豨莶草 (with the 豨 character). This appears to be a character-variant issue rather than a different substance; the identity field is left unchanged pending owner review.
