What is An Xi Xiang (benzoin) used for in Chinese medicine?

An Xi Xiang is an aromatic resin used to open the orifices and revive consciousness, and to move qi and blood for pain in the chest and abdomen. Our record places it among the aromatic substances that open the orifices — a small, tightly defined category alongside She Xiang, Su He Xiang and Bing Pian — with a spicy, bitter, neutral character entering the Heart, Liver and Spleen channels.

Its traditional actions, as recorded on this site:

  • Aromatically opens the orifices and moves qi — for sudden loss of consciousness or delirium. In the traditional framework this is an acute, short-term intervention for a “closed” pattern, not an everyday tonic use.
  • Moves qi and blood — for pain in the abdomen and chest. This second action is what distinguishes An Xi Xiang inside its own category: it is not purely a reviving agent but also a mover of stagnation.

What the substance actually is: not a root, leaf or flower but a balsamic resin, tapped from the wounded bark of Styrax trees (family Styracaceae) and hardened into brittle, fragrant lumps. It is the same material the West knows as gum benzoin, and it was a major article of the maritime and overland incense trade for centuries — the English word itself descends, through Arabic lubān jāwī (“frankincense of Java”), from a trade route rather than a botany textbook.

Chinese Herb Dosage

  • What is the dosage of An Xi Xiang, and is it decocted?

    0.3–1.5 g, and no — it is not decocted. Our record gives 0.3–1.5 g, a micro-dose range shared by the whole orifice-opening category. Like Su He Xiang (0.3–1 g) and Bing Pian (0.15–0.3 g) on this site, An Xi Xiang is a volatile aromatic resin whose active fraction is driven off by prolonged boiling, so it is folded into finished pills and powders rather than cooked with the other herbs.

    • Form — pills or powders. In practice it is almost always met inside a ready-made preparation such as Su He Xiang Wan or Zhi Bao Dan rather than dosed as a raw single herb.
    • Why the dose is so small — these are strongly dispersing, scattering substances. The tradition treats the small dose and the short course as the safety margin, not as an economy measure.
    • Duration — the category is used for acute situations and short courses; it is not a long-term herb.

Chinese Herb Contraindications & Cautions

  • Are there cautions for An Xi Xiang (benzoin)?

    Our record contraindicates it in yin deficiency and in excess heat or fire. An Xi Xiang is aromatic, dispersing and drying by nature; in a yin-deficient or heat-excess picture those qualities work against the pattern rather than with it. Two further cautions apply to the whole orifice-opening category and one is specific to this resin.

    • Short-term use only — aromatic openers are acute-situation herbs. Prolonged use is held to scatter and consume qi, and the category is not intended for maintenance prescribing.
    • Pattern matters — this category is directed at “closed” presentations. It is not appropriate for collapse-type or abandoned patterns, where the requirement is to secure rather than to open.
    • Contact allergy (specific, documented) — benzoin is an established contact allergen in the dermatology literature. In a series of 477 patients patch-tested at a Melbourne contact dermatitis clinic, 45 reacted to compound tincture of benzoin, making it the third most common allergen in that series; 14 had strong positive reactions. Most of those reactions occurred alongside positives to fragrance mix, balsam of Peru, colophony or tea tree oil, so cross-reactivity with related fragrance and resin allergens explains much of the frequency. Post-surgical allergic contact dermatitis to compound benzoin tincture has also been reported. This is a documented reaction to topical exposure; it is not a report of harm from the internal Chinese medicinal use, and it is stated here proportionately — as something for a practitioner to know about a fragrance-sensitive patient, not as a general alarm.
    • Pregnancy and children — strongly aromatic, blood-and-qi-moving substances in this category are conventionally reserved for qualified practitioner use, particularly in pregnancy. Reference note, not a prescription.

Chinese Herb Toxicity & Overdose

  • No significant intrinsic toxicity is documented for benzoin at the small doses used in this category, and our record records no toxicity entry. The realistic concerns are the ones above rather than poisoning: hypersensitivity in fragrance- and balsam-sensitive individuals, the dispersing effect of prolonged use in an already depleted patient, and product identity — roughly 30% of commercial benzoin samples in one molecular survey contained plant material from outside the Styrax genus. As a semi-solid resin its species cannot be verified visually, so provenance carries more weight here than for most plant drugs.

Herb-Drug Interactions

  • No well-established clinical herb–drug interactions are documented for An Xi Xiang. The dose is very small and the traditional use is short-term, which limits the realistic scope for interaction. One practical, non-pharmacological point is worth recording: patients with known sensitivity to fragrance mix, balsam of Peru or colophony have a documented tendency to cross-react with compound benzoin tincture, so a fragrance-allergy history is relevant information for a practitioner.   • Is there modern research on An Xi Xiang (Benzoinum)?

    Very little, and none of it clinical. Benzoin is one of the least-studied substances in its category: there are no randomised controlled trials of An Xi Xiang, and the published work is confined to chemistry, species authentication and a small amount of cell-based pharmacology. We record what exists rather than inflating it.

    • Chemistry — benzoin balsams have been characterised in both volatile and non-volatile fractions, with benzoic and cinnamic acid derivatives as the defining constituents. Four coniferyl and two morinol derivatives were reported for the first time in Laotian Siam benzoin, and two liquid-chromatographic methods were developed to distinguish Siam from Sumatra balsam.
    • Cell-based pharmacology — a new phenylpropane derivative, stybenpropol A, was isolated from benzoinum and reported to reduce inflammatory adhesion-molecule and cytokine expression and inhibit apoptosis in TNF-α-stimulated human umbilical vein endothelial cells. In vitro work on an isolated compound, in a single laboratory model; it is not evidence about the whole resin and not evidence about people.
    • Species authentication — DNA was successfully extracted from bark residues in benzoin resin and ITS2 sequencing identified commercial material as Styrax tonkinensis and Styrax japonicus, with about 30% of samples mixed with other genera. This is quality-control research and arguably the most practically useful finding of the three.
    • Dermatology — the largest body of published human data on benzoin is not about efficacy at all but about contact allergy to compound benzoin tincture (see cautions).

  • What is the difference between An Xi Xiang (benzoin) and Su He Xiang (storax)?

    Both are aromatic resins in the same orifice-opening category, but they come from different trees and behave differently: An Xi Xiang is neutral and also moves qi and blood for chest and abdominal pain, while Su He Xiang is distinctly warm and is the reference herb for cold-type closed disorders. The confusion between them is not accidental — it is baked into the pharmaceutical Latin.

    • An Xi Xiang — our record gives Benzoinum, spicy, bitter and neutral, entering Heart, Liver and Spleen. The resin is tapped from Styrax trees; molecular identification of commercial benzoin has confirmed Styrax tonkinensis (Siam benzoin) and Styrax japonicus as the traded sources.
    • Su He Xiang — our record gives Styrax Liquidis, sweet, spicy, warm and aromatic, entering Heart and Spleen. Despite that Latin name, the officinal storax resin is not obtained from a Styrax tree at all; it is the balsam of the oriental sweetgum, Liquidambar orientalis. The shared word “styrax” is a historical label, not a statement of botanical kinship.
    • Practical upshot — two resins, two families, one confusing Latin word. If a source appears to treat them as interchangeable, that is a naming artefact rather than a materia medica position.

    Is An Xi Xiang the same as friar’s balsam?

    Essentially yes — friar’s balsam, also sold as compound benzoin tincture, is benzoin resin dissolved in alcohol. It is one of the clearest points of contact between the Chinese materia medica and Western pharmacy: the same resin appears in the Chinese aromatic-opening category and, quite independently, on hospital trolleys as a skin adherent and dressing preparation. The Western use is topical and external; the Chinese use is internal and aromatic. Same substance, different traditions, different route.

    Are there different grades of benzoin?

    Yes — the trade distinguishes Siam benzoin from Sumatra benzoin, and they differ chemically. Siam benzoin (Styrax tonkinensis, from Laos, Thailand, Cambodia and Vietnam) is characterised by benzoic acid derivatives and a vanilla-like note; Sumatra benzoin (Styrax benzoin and Styrax paralleloneurus) additionally contains cinnamic acid. Chromatographic methods have been published specifically to tell the two apart.

    • Adulteration is documented — a 2023 molecular study of commercially available benzoin found that about 30% of samples were mixed with plant tissue from genera other than Styrax. Because benzoin is a hardened resin, its species cannot be judged by eye, and DNA had to be recovered from bark residues to identify it.
