What is Gua Lou Ren used for in Chinese medicine?
Gua Lou Ren is the seed of the trichosanthes fruit, and it is the most moistening and most lubricating of the trichosanthes medicinals: it clears and transforms Phlegm-Heat in the Lung, moistens dryness, and slides the stool through dry intestines. Sweet and cold, entering the Lung, Stomach and Large Intestine, it is chosen when heat has thickened the phlegm and dried the fluids at the same time.
Its traditional actions as recorded here:
- Transforms Phlegm-Heat, moistens the Lung, expands the chest — for cough with thick, sticky sputum that is difficult to expectorate, with chest pain, and for a stifling, oppressive sensation in the chest or diaphragm from Phlegm accumulating there.
- Moistens the Intestines — for dry stool and constipation arising from Stomach and Large Intestine heat, or in a pattern of Lung heat. This is the seed’s distinguishing action; it is oil-rich, and the lubricating effect is what the whole fruit gets from it.
- Promotes the healing of sores, eliminates pus, dissipates nodules — for breast and intestinal abscesses, carbuncles, and sores that have not yet suppurated.
The practical shape of the herb is that it does two things at once. A patient with a hot, tight, unproductive cough who is also constipated is exactly who this seed is for, because moistening the Lung and moistening the bowel are one movement in this medicinal rather than two.

What is the typical dosage of Gua Lou Ren?
Roughly 9–15 g in decoction, with 10–15 g the range recorded here from Chen & Chen and 9–15 g from Bensky — the two agree closely enough that the practical figure is about 9–15 g. The seeds are crushed before decocting; being oily, they give up little to the water whole.
- Crushed, not whole — the medicinal value sits in the seed oil and the kernel, so the seeds are broken before they go into the pot.
- Shuang (defatted) preparation — where the lubricating, stool-loosening effect is unwanted or the patient’s digestion is loose, a pressed, partly de-oiled form (Gua Lou Ren Shuang) is traditionally used instead. This is the standard way of keeping the Phlegm-transforming action while blunting the laxative one.
- Whole fruit versus seed — the fruit (Gua Lou) is dosed higher in the records here, up to 24–30 g crushed in Xiao Xian Xiong Tang. Dose figures are not transferable between the family members; see the comparison below.

Chinese Herb Contraindications & Cautions
Are there cautions for Gua Lou Ren? What is the Wu Tou incompatibility?
Two cautions are recorded: it is not used where the Spleen is weak with nausea or loose stools, and it is one of the classical Eighteen Incompatibilities — it is not combined with Wu Tou (aconite). The first follows from what the herb is: a cold, oily, moistening seed will worsen a cold, damp, loose digestion. The second is a formal prohibition from the classical literature.
- Spleen deficiency with loose stools or nausea — avoid. Where the Phlegm-Heat indication genuinely applies but the digestion is fragile, the de-oiled preparation (Gua Lou Ren Shuang) is the traditional way round the problem.
- The Eighteen Incompatibilities (shi ba fan) — the classical list of eighteen herb pairs held never to be combined. The trichosanthes group falls under the aconite clause. The pairings recorded on this site are explicit, and they cover every member of the family: Gua Lou, Gua Lou Ren, Gua Lou Pi and Tian Hua Fen are each recorded as incompatible with Fu Zi (prepared aconite lateral root), Chuan Wu (prepared Sichuan aconite) and Cao Wu (prepared wild aconite) — twelve recorded pairs in all. “Wu Tou” in the classical wording is the aconite main root, and in practice the prohibition is read as covering the aconite group as a whole, which is how the records here have it.
- Why it matters more than most cautions — the aconites are the most acutely toxic herbs in routine use, so a prohibition attaching to them is treated as absolute rather than relative. Prescribing that involves aconite at all is professional territory.
- Not for cold-Phlegm patterns — the herb is cold. Thin, white, watery phlegm from cold calls for a different category entirely.

Chinese Herb Toxicity & Overdose
Low as ordinarily used. Trichosanthes seed is not classified as a toxic herb here and carries no dose ceiling on that basis; the recorded 9–15 g range is a therapeutic range, not a safety limit. The commonest adverse effect reported for the seed is the predictable one — loose stools, abdominal discomfort or nausea — which follows from its oil content and cold nature rather than from a toxic constituent, and which is the reason for the Spleen-deficiency caution and for the de-oiled preparation.
One point deserves stating precisely, because it is easy to get wrong. The trichosanthes plant is the source of trichosanthin, a ribosome-inactivating protein that is a genuinely potent and well-studied toxic protein — and it is associated principally with the root (Tian Hua Fen), which is why abortifacient activity and injectable-preparation toxicity appear in the root literature. Ribosome-inactivating proteins have also been characterised from trichosanthes seeds. These are heat-labile proteins that do not survive decoction, so the finding belongs to the pharmacology of isolated compounds and injectable preparations, not to a boiled herbal decoction — but it is the reason the root and the seed should not be treated as interchangeable in any safety discussion.

Herb-Drug Interactions
No well-established clinical herb–drug interactions are documented for Gua Lou Ren. No interaction study, and no human pharmacokinetic or CYP-enzyme data, was located for the seed. Two points are worth practitioner awareness on general grounds rather than on evidence: an oily, stool-loosening seed used alongside pharmaceutical laxatives may be additive, and anyone taking it should be aware that the plant’s classical prohibition concerns another herb (aconite), not a drug. The absence of interaction reports reflects an absence of research, not a demonstration of safety. (Flagged for practitioner confirmation.)

Is there modern research on Gua Lou Ren (Semen Trichosanthis)?
Yes, but most of it studies the seed as an oilseed rather than as a Phlegm-transforming herb, and none of it is clinical. Trichosanthes seed is roughly a third oil by weight, and the published work is dominated by lipid chemistry, antioxidant assays and animal lipid-metabolism models. The traditional indications — hot phlegm cough, chest oppression, dry stool — have essentially no trial literature behind them.
- The plant as a whole — a 2018 review of Trichosanthis Fructus catalogues around 162 identified compounds across terpenoids, phytosterols, flavonoids, nitrogenous compounds and lignans, and reports preclinical activity concentrated on the cardiovascular system (protection against myocardial ischaemia, calcium antagonism, endothelial protection, anti-platelet effects) alongside expectorant, anti-inflammatory and antioxidant findings. The same review notes explicitly that the different uses of the seeds, pericarps and fruits still need to be clarified — the modern literature has not caught up with the distinction the tradition already makes.
- Seed oil and lipids (animal) — a 2020 mouse study on a high-fat diet reported hypolipidemic and antioxidant activity for trichosanthes seed oil and its flavonoids, and a 2024 rat study reported similar effects alongside changes in gut microbiota and metabolomic profiles. Both are animal work on an isolated oil fraction.
- Antioxidant and neuroprotective screening — a 2024 comparative study in the nematode Caenorhabditis elegans examined seed oils from T. kirilowii and T. laceribractea. Screening-model evidence only.
- Ribosome-inactivating proteins — characterised from both the root tubers and the seeds of T. kirilowii and related species. Relevant to the plant’s protein chemistry and to injectable preparations, not to decoctions.
- The root, separately — a 2025 comprehensive review covers Trichosanthis Radix (Tian Hua Fen), including trichosanthin, cucurbitacins, and its documented abortifacient and toxicological profile. Cited here to mark what belongs to the root and does not transfer to the seed.

What is the difference between Gua Lou, Gua Lou Ren, Gua Lou Pi and Tian Hua Fen?
All four are the same plant — Trichosanthes kirilowii — held here as four separate medicinals because four different parts of it do four different jobs. Three are parts of the fruit; the fourth, Tian Hua Fen, is the root, which almost nobody guesses. Getting these straight is the single most useful thing to know about this group.
- Gua Lou (Fructus Trichosanthis Kirlowii) — the whole ripe fruit, peel and seed together. Sweet, cold; Lung, Stomach, Large Intestine. The broadest of the three fruit medicinals: clears Phlegm-Heat, opens and expands the chest, and moistens the bowel. It is the one used for chest oppression and chest pain (thoracic obstruction), and it is the form specified in Xiao Xian Xiong Tang and in Bei Mu Gua Lou San on this site.
- Gua Lou Ren (Semen Trichosanthis Kirlowii) — the seed (this entry). Same nature, flavour and channels as the whole fruit, but weighted toward moistening: it is the oily part, so it is the strongest of the family at loosening dry stool and at moistening a dry Lung. Chosen when Phlegm-Heat is accompanied by dryness or constipation.
- Gua Lou Pi (Pericarpium Trichosanthis Kirlowii) — the peel/rind. Same nature, flavour and channels again, but weighted the other way: it is the part that regulates Qi and opens the chest, and it is not lubricating. Chosen when the problem is chest constraint and Phlegm without dryness, or where a loosening effect on the stool would be unwelcome.
- Tian Hua Fen (Trichosanthis Kirlowii Radix) — the root. Bitter, slightly sweet, cold; Lung and Stomach only. This one is not really a Phlegm herb in the same sense: it clears heat and generates fluids for thirst and wasting-and-thirsting patterns, and it reduces swelling and expels pus for sores and abscesses. On this site it appears in Yu Ye Tang, Xian Fang Huo Ming Yin, Fu Yuan Huo Xue Tang and Bei Mu Gua Lou San.
The shortest version: peel opens the chest, seed moistens, whole fruit does both, root generates fluids. The first three sit together in the Phlegm-Heat category here; the root sits alongside them in the same category on this site but behaves clinically much more like a fluid-generating heat-clearing herb.
Which formulas contain Gua Lou Ren?
Two formula records on this site specify the seed by name rather than the whole fruit:
- Qing Qi Hua Tan Wan (Clear the Qi and Transform Phlegm Pill) — Gua Lou Ren 30 g, with Dan Nan Xing, Ban Xia, Huang Qin, Chen Pi, Xing Ren, Zhi Shi and Fu Ling. The seed is here for exactly its cooling, moistening, phlegm-loosening role inside an otherwise fairly drying formula.
- Ke Xue Fang (Coughing of Blood Formula) — Gua Lou Ren 9 g, with Qing Dai, Zhi Zi, Fu Hai Shi and He Zi.
The whole-fruit records account for the better-known chest formulas: Xiao Xian Xiong Tang (Gua Lou 24–30 g crushed, with Huang Lian and ginger-fried Ban Xia), Bei Mu Gua Lou San (Gua Lou with Chuan Bei Mu, Tian Hua Fen, Fu Ling, Ju Hong and Jie Geng — a formula that contains two members of this family at once), and the chest-opening decoction listed here as Gua Luo Xie Bai Bai Jiu Tang (Gua Lou 12 g with Xie Bai). Botanical naming: the plant is written Trichosanthes kirilowii Maxim. in current botanical usage; the Latin drug names recorded here use the spelling “Kirlowii”. The Chinese Pharmacopoeia also admits Trichosanthes rosthornii Harms as a source plant for the fruit, peel, seed and root, so commercial material may derive from either species — a species-substitution point rather than a conflict between sources.
