What is Ban Mao used for in Chinese medicine?

Ban Mao (the dried, defatted body of the blister beetle, Mylabris) is an acrid, hot, and extremely toxic substance used traditionally, in exceedingly small processed doses, to break up blood stasis and attack toxic accumulations — classically for stubborn masses and nodules and for blood-stasis amenorrhea, and, applied topically, as a vesicant/counter-irritant for tenacious skin lesions such as ringworm and warts. In contemporary integrative oncology, cantharidin and its synthetic derivative norcantharidin are researched and, in China, formulated into marketed anticancer products.

Its documented traditional actions:

  • Breaks blood stasis and disperses masses — classically for amenorrhea due to blood stasis and for stubborn abdominal masses/nodules; used internally, when at all, only in minute processed doses.
  • Attacks toxicity and disperses nodules — scrofula, carbuncles, boils, and skin ulcers, traditionally addressed both by trace internal doses and by topical application.
  • Corrodes flesh / vesicant counter-irritant (topical) — applied externally to raise a blister over stubborn dermatoses such as tinea (ringworm) and warts; the same corrosive, blistering action that makes it dangerous is what is being harnessed, under professional control only.
  • Modern integrative-oncology context — cantharidin and norcantharidin (a lower-toxicity synthetic derivative) are studied against liver, gastric, colorectal, pancreatic, lung, breast, prostate, and bladder cancers and leukemia, and are formulated into several China-marketed anticancer products (see Research). This is pharmaceutical-grade, protocol-based use, not a traditional dosing practice, and not a treatment recommendation.

Discrepancy (nature/channels): the current Sacred Lotus sources & references lists Ban Mao’s nature as “Spicy, Cold, Toxic” and its channels as Large Intestine, Liver, Small Intestine. Materia medica consensus (Bensky-tradition references) and a 2025 peer-reviewed pharmacology review instead describe Ban Mao as acrid/pungent, hot, and extremely toxic, entering the Liver, Stomach, and Kidney channels (Bensky-derived sources more often give Large Intestine, Liver, Stomach). We found no independent source supporting a “cold” nature or a Small Intestine channel; correcting the identity fields is outside the scope of this content update, so they are left unchanged pending owner review.

Chinese Herb Dosage

  • What is the dosage of Ban Mao, and is it used internally?

    Internal use is exceptional and expert-only; what follows is reference information about documented dose levels, not usage guidance. Materia medica sources cite internal reference doses on the order of roughly 0.03–0.06 g (30–60 mg) of the processed substance — an extraordinarily small figure reflecting extreme toxicity — while a 2025 pharmacology review separately reports an average documented toxic dose of about 3.9 g in poisoning cases (range 0.1–60 g). The gap between those two figures illustrates how narrow the margin is between a traditional reference dose and a dangerous one.

    Reference facts, not instructions:

    • Processing is mandatory before any internal use — the wings, legs, and head are removed and the body is defatted/roasted (traditionally often stir-fried with glutinous rice) to reduce toxicity and irritation; the raw insect is never used internally.
    • Internal use is rare in contemporary practice — where cantharidin-related compounds are used against cancer in China, this is done as standardized pharmaceutical products (Kangsaidi capsules, Aidi injection, DCVB6 injection — see Research), not as general-practice decoction dosing.
    • Topical (vesicant) use is also professional-only — for example, the FDA-approved cantharidin topical solution (Ycanth, 0.7%) for molluscum contagiosum is applied only by a health-care provider using a controlled single-use applicator, never self-administered.

Chinese Herb Contraindications & Cautions

  • Are there cautions or contraindications for Ban Mao?

    Ban Mao is contraindicated in pregnancy and, more broadly, is one of the most toxic substances in the Chinese materia medica: a professional-only, exceptional-use substance, never a general or self-administered herb. The site’s existing reference note states it has been shown to cause teratogenic (birth-defect) effects, consistent with its absolute pregnancy contraindication.

    • Absolute contraindication in pregnancy — teratogenic and abortifacient effects are documented; Ban Mao must never be used, internally or topically, during pregnancy or when pregnancy is possible.
    • Contraindicated with pre-existing renal, urinary-tract, or GI mucosal disease — cantharidin is directly irritant/corrosive to the GI tract and is concentrated in and irritates the kidneys and urinary tract on clearance, so pre-existing impairment of either system sharply raises risk.
    • Never for self-use — the margin between a documented reference dose and a toxic dose is extremely narrow (see Dosage); any use belongs only to a licensed practitioner or physician working within a regulated protocol.
    • Skin/mucosal contact caution even topically — as a vesicant, uncontrolled contact (including with unaffected skin, eyes, or mucous membranes) can itself cause blistering and chemical injury; professional topical use is carefully localized and dosed (see the FDA-approved Ycanth precedent).

Chinese Herb Toxicity & Overdose

  • Very high — Ban Mao is among the most toxic substances in the Chinese materia medica, and its active constituent cantharidin has caused fatal poisoning at very low doses. Cantharidin is a potent vesicant that inhibits serine/threonine protein phosphatases (notably PP2A) at the cellular level, which underlies both its corrosive/blistering action and its toxicity to the GI tract, kidneys, urinary tract, liver, and heart.

    Documented toxicity pattern:

    • Gastrointestinal — burning of the mouth and throat, nausea, vomiting, abdominal pain, and hematemesis (vomiting blood) are typically the earliest signs after ingestion.
    • Renal / urinary tract — cantharidin is concentrated in and cleared via the kidneys and irritates the urinary tract as it passes, producing hematuria, proteinuria, and dysuria; case series report acute kidney injury with dialysis required in a majority of severe cases.
    • Systemic / multi-organ — severe poisoning has produced multi-organ dysfunction (a 2024 case report followed ingestion of 10 g of cantharidin powder), and a case series described a “Guillain-Barré-like” pattern of cranial-nerve palsies and ascending muscle weakness alongside renal failure and respiratory compromise requiring ventilation; 4 of 10 patients in that series died.
    • Fatal dose is low — fatalities have been reported at crude-beetle doses as low as roughly 1.3 g, far above any documented traditional reference dose, illustrating how narrow the safety margin is.
    • No specific antidote — management reported in the literature is supportive (fluids, hemodynamic support, and in severe cases hemofiltration/hemoperfusion); any suspected exposure is a medical emergency.

Herb-Drug Interactions

  • No well-established clinical herb–drug interaction data exist for Ban Mao — the toxicology literature is dominated by acute-poisoning case reports rather than controlled interaction studies. Given its documented nephrotoxicity and GI corrosiveness, concurrent use with other nephrotoxic agents (e.g., NSAIDs, aminoglycosides, cisplatin-class chemotherapy) or GI-irritant/ulcerogenic drugs is a reasonable theoretical concern, and any use in an integrative-oncology context should be coordinated by the treating physician.   • Is there modern research on Ban Mao / cantharidin?

    Yes — cantharidin and its synthetic derivative norcantharidin are actively researched as anticancer agents, with several products already marketed in China and one purified-cantharidin drug now FDA-approved in the U.S. for a non-cancer skin condition; controlled human cancer-efficacy data remain limited. Preclinical work spans liver, gastric, colorectal, pancreatic, lung, breast, prostate, and bladder cancers and leukemia.

    • Mechanism (preclinical) — cantharidin/norcantharidin inhibit serine/threonine protein phosphatases (notably PP2A), driving G2/M cell-cycle arrest, apoptosis via Bcl-2-family modulation, anti-angiogenic effects, and reversal of drug resistance in cancer cell lines and animal models.
    • Norcantharidin vs. cantharidin — norcantharidin (NCTD), a demethylated synthetic analogue, is approved in China as a cancer therapeutic and shows a better safety margin than cantharidin itself, though a 2023 review of its hepatocellular-carcinoma research notes that most supporting data are still from cell and animal studies, with “very little” from clinical trials.

  • How is Ban Mao different from other toxic externally-applied substances like Chan Su and Xiong Huang?

    Ban Mao, Chan Su, and Xiong Huang are all classed among the toxic “substances for external application,” but they come from different sources and act by different mechanisms. Ban Mao is an insect-derived vesicant that breaks blood stasis and blisters the skin; Chan Su is a toad-derived cardiotonic/detoxifying substance; Xiong Huang (realgar) is an arsenic-sulfide mineral used to attack toxicity and kill parasites.

    • vs Chan Su (toad venom) — both are toxic substances researched as sources of modern Chinese anticancer pharmaceuticals (cantharidin/norcantharidin for Ban Mao; bufadienolide-based cinobufacini/huachansu for Chan Su), but Chan Su is sweet, acrid, and warm and works mainly through cardiotonic and detoxifying bufadienolides, whereas Ban Mao is acrid/hot and works through the corrosive, blood-moving terpenoid cantharidin.

    Ban Mao vs. its pharmaceutical derivatives: the crude, processed herb is not the same product as the purified or synthetic compounds derived from it.
