Qian Dan is a lead compound and is treated as unsafe for internal use in modern practice. Its historical internal applications were abandoned once lead's toxicity was understood, and contemporary Chinese medicine restricts it to external, professionally supervised use only — in small amounts, for limited periods, and never on broken skin, mucous membranes, or large body-surface areas, because lead can be absorbed systemically through the skin.
- Pregnancy and lactation — avoided outright; lead crosses the placenta and is excreted in breast milk, and even low-level exposure is linked to adverse fetal/infant neurodevelopment.
- Children — considered especially vulnerable; pediatric lead exposure is associated with neurodevelopmental effects, including at blood-lead levels once considered background/normal.

Chinese Herb Toxicity & Overdose
Lead tetroxide is systemically toxic, and no safe exposure level has been established. Its traditional external use in Chinese medicine is a documented real-world pathway to clinical lead poisoning: absorption occurs through broken or diseased skin (the ulcers/sores Qian Dan was traditionally applied to are themselves higher-risk absorption sites), and lead accumulates in blood, soft tissue, and bone.
- Documented case — a patient using a lead-containing herbal patch (517 mg lead per gram of patch) on a chronic leg ulcer for about three months developed anorexia, weight loss, headache, dizziness, nausea, vomiting, constipation, weakness, and anemia, with a blood lead concentration of 226 µg/dL; the patient recovered after chelation therapy (EDTA and DMSA).

Qian Dan is produced by controlled heating and oxidation of metallic lead, yielding an orange-red to orange-yellow heavy mineral powder — lead tetroxide (Pb3O4), known industrially as minium or red lead. It belongs to a small group of toxic mineral substances in the traditional pharmacopeia — alongside mercury-based agents (e.g., calomel/Hong Fen) and arsenic-based agents (e.g., realgar/Xiong Huang) — all restricted to careful, limited external use because of heavy-metal toxicity.
