What It Is
Neuroplasticity is the brain's capacity to change its own structure and wiring in response to what you do. The brain holds on the order of tens of billions of neurons. The work happens at the synapses, the roughly 100 trillion junctions where one neuron passes a signal to the next. Those connections change constantly. Use a pathway and it strengthens; leave one idle and it weakens. This is the cellular basis of learning and memory, often summarized as the idea that neurons that fire together wire together.
The brain does this across your whole life, in four ways. The adult brain forms new connections, strengthens and weakens existing ones, remodels the insulation on its wiring, and reorganizes function around damaged tissue after an injury. What changes with age is how much effort and time it takes.
How the Brain Changes
1Synapses strengthen and weaken, within minutes
When two neurons fire together, the connection between them becomes more efficient. This is long-term potentiation. It can take hold within minutes and marks the physical start of a new memory. Its opposite, long-term depression, weakens the connections that stop carrying useful signals. The pathways you use are reinforced; the ones you leave idle fade. That is why repetition makes something stick.
2The brain grows and prunes structure, over weeks to months
Push a set of circuits hard enough and the brain adds gray matter there. Adults who spent three months learning to juggle enlarged the brain area that tracks visual motion, and that growth partly receded once they stopped practicing. The reversal shows the same process running both ways: structure grows where practice continues and recedes where it stops.
3Wiring gets insulated, making skills automatic
Myelin is the fatty sheath wrapped around the brain's wiring. Practicing a skill thickens the myelin along the circuits that carry it, so signals travel faster and more reliably. This is part of why a movement that felt clumsy becomes smooth and automatic once you have drilled it.
Across the Lifespan
Plasticity is greatest in childhood. A young brain starts overwired, then gets shaped by pruning. That is why children pick up language and motor skills so readily, and why circuits shaped in childhood are hardest to overwrite later. There are sensitive periods, windows when particular circuits are especially open to being shaped.
Those windows narrow with age but stay open through adulthood. Adults learn languages, instruments, and sports, recover speech and movement after brain injury, and reshape deeply worn habits. In older adults randomized to a year of brisk walking, the front of the hippocampus grew about 2% (Erickson 2011). The hippocampus is a core memory structure. That 2% reversed one to two years of age-related shrinkage, while a stretching comparison group kept declining.
Only one narrow question is genuinely disputed. Everyone agrees the adult brain reshapes the connections among its existing neurons. Whether it also grows brand-new ones is unsettled. Two careful 2018 studies of human hippocampal tissue reached opposite conclusions. One found new neurons into age 79. The other found almost none in adults. The split seems to hinge on how the tissue was handled. That question is open. The reshaping of existing connections is settled, and it covers most of what matters for learning and recovery.
What Drives Change
The brain changes the circuits you work hardest. So effort, aimed at the right skills, is what drives structural change. Four levers carry the most weight: effortful, novel learning at the edge of your ability, aerobic exercise, sleep, and, after injury, intensive rehabilitation. None of them costs anything.

Stroke Recovery

Three months learning to juggle enlarged the brain's motion-vision area, then it partly shrank back
People who spent three months learning to juggle grew the brain regions that track moving objects, and those regions shrank back partway once they stopped practicing.
In a randomized study, previously non-juggling adults were assigned to learn a three-ball cascade or to a control group. Structural MRI at baseline, after three months of practice, and after a further three months without practice showed transient bilateral expansion of gray matter in the middle temporal visual motion area (hMT/V5) and the left posterior intraparietal sulcus in the learners, changes that partly reversed after practice stopped. The study is small and the change is measured in structure, not in a real-world ability, so it demonstrates that focused practice reshapes the adult brain without quantifying a functional payoff.

Cabbies who passed London's 3-to-4-year street test grew the hippocampus; those who failed did not
Trainee London cab drivers who passed the years-long test of the city's streets grew the memory-and-navigation part of the brain, while those who did not pass, and people who never trained, did not.
A prospective study followed 79 trainee London taxi drivers and 31 controls with structural MRI and memory testing at baseline and three to four years later. The 39 trainees who qualified showed a selective increase in posterior hippocampal gray matter relative to non-qualifiers and controls, with no baseline difference between groups, which argues the structure changed with the learning, not reflecting who was drawn to the job. The same qualified drivers were worse at some other memory tasks, so the gain came with a tradeoff, not a free upgrade.
Who this may not transfer to:The trainee cohort was all male, which is the usual makeup of that workforce. There is no reason to expect the capacity for structural change to be male-specific, but this particular finding was measured only in men.

A year of brisk walking regrew the hippocampus about 2% in older adults
Older adults who walked briskly for a year grew the front of the hippocampus by about 2%, effectively reversing one to two years of age-related loss, while a stretching group kept shrinking as expected.
A one-year randomized trial in 120 community-dwelling older adults compared moderate-intensity aerobic walking with a stretching-and-toning control. The aerobic group increased anterior hippocampal volume by roughly 2%, against an expected age-related decline of 1 to 2% per year seen in the control group, which lost about 1.4%. Higher fitness gains and greater volume change tracked with better spatial memory, and the effect was associated with higher circulating BDNF. It is one well-run trial in older adults, so the size of the effect at other ages is not settled.

Brain-training games improve the trained game, not everyday thinking
You get better at the game you practise, and at things very like it, but the evidence does not support the promise that these games make your everyday thinking sharper.

- Effortful, novel learning drives the most change. The difficulty itself is what tells the brain to change. Coasting through something you already do well changes little. Working right at your limit reshapes structure. London taxi drivers, who spend years memorizing the city's streets, have an enlarged posterior hippocampus that grows with years on the job.
- Aerobic exercise reliably raises BDNF, a protein that helps build and strengthen connections. Across randomized trials it also lowers depression. Researchers think plasticity is the link between the two. To keep a brain adaptable, walk, do aerobic base work, and lift.
- Sleep after learning consolidates and reorganizes new memories. A night of sleep is part of the learning. That is why spacing practice across days beats cramming.
- Intensive rehabilitation after injury drives the brain to reorganize function around damaged tissue. Two weeks of constraint-induced movement therapy improved real-world use of the affected arm after a stroke, and the gain held at a year.
Commercial brain-training games are the exception. A 2016 review found players improve at the trained game and tasks very like it, with little carryover to everyday thinking (Simons 2016). That is the mechanism working as expected: the brain adapts the specific circuits you drill while leaving general thinking untouched.
When It Works Against You
The same mechanism strengthens useful and harmful circuits alike. The change that lets you learn can also lock in patterns you would rather lose.
Chronic pain is one medical example. In central sensitization, the pain-signaling neurons of the central nervous system become more responsive. This is the same use-dependent plasticity behind learning, now running in the pain system. This is one reason care for long-standing pain increasingly targets the nervous system alongside the tissue itself. Circuits that have learned to amplify pain can also be trained to quiet it. Teaching people how pain is produced, usually alongside active treatment, has improved pain and disability in trials.
It helps explain why some pain persists or spreads after the original injury has healed, and why normally harmless input can start to hurt.
Habits run on the same machinery. A repeated behavior builds an efficient, low-effort pathway. That is useful for the routines you want and stubborn for the ones you do not. Changing an entrenched habit is slow work. You build a competing pathway until the new one is easier to follow than the old. That is the biology underneath habit change, and the reason doing hard things reshapes you over time.
Two facts temper the hype. Meaningful change comes from deliberate practice over weeks to years. And the pace slows with age but never stops. Adults keep the capacity to learn and recover. Recovery after a brain injury can be substantial, though it depends heavily on the injury, its severity, and how intensive the rehabilitation is. Outcomes vary widely from person to person.
