What It Is

Vitamin C, or ascorbic acid, is a water-soluble vitamin the body cannot make for itself, so it has to arrive from food every few days. Most animals build their own supply; humans are among the few that lost the enzyme, which is why a shortfall shows up as bleeding gums or slow wound healing. It also works as an antioxidant. The uses beyond dispute are two: it prevents scurvy, the deficiency disease, and it covers a small daily requirement, near 90 milligrams for men and 75 for women. A diet with any fruit and vegetables clears that without effort.

What It Does

A body with enough vitamin C does four things reliably. It prevents and cures scurvy, builds the collagen that holds skin and blood vessels together, and keeps infection-fighting immune cells stocked. It also raises how much iron you absorb from plant foods eaten in the same meal. Each of these follows from having enough, and a normal diet with produce supplies it. More than the body needs adds none of them.

The cold reputation is real but smaller than its fame. Pooled trials in more than 11,000 people found that taking vitamin C every day did not lower how often people in ordinary life caught colds. Taken regularly it did shorten a cold that arrived anyway. Colds ran about 8 percent shorter in adults and 14 percent shorter in children. That is roughly half a day off a week-long cold.

One situation flips the result. Under short bursts of extreme physical stress, such as marathon runners and soldiers on cold-weather military exercises, regular vitamin C roughly halved the risk of catching a cold.

A clinical review (Gandhi 2023) describes scurvy as a nutritional deficiency of vitamin C that impairs connective-tissue synthesis, producing bleeding gums, joint aches, poor wound healing, skin changes and perifollicular hemorrhage. It is fully preventable and treatable with vitamin C. Although rare in modern times because of widespread dietary vitamin C, sporadic cases still occur, mainly in the elderly and malnourished and in association with alcohol dependence, low socio-economic status and poor diet. This is the mechanistic anchor of the whole topic: vitamin C is essential because the body cannot make it, and the clear, unarguable use is preventing the deficiency.

Ascorbic acid has a pronounced enhancing effect on non-heme iron absorption when measured from single test meals in fasting subjects. Cook and Reddy (2001) tested whether this holds across a complete diet by measuring iron absorption over five days in 12 subjects at daily vitamin C intakes ranging from 51 to 247 mg. Iron absorption correlated positively with ascorbic acid (P = 0.0069) but the overall facilitating effect from a complete diet was far less pronounced than from single meals, which the authors noted explains why prolonged vitamin C supplementation has a negligible effect on iron status. So the practical lever is pairing vitamin C foods with plant iron at a meal, not supplementing to raise iron over time.

A pharmacokinetic model built from seven healthy volunteers undergoing vitamin C depletion and gradual repletion (Graumlich 1997, from Levine's group) found saturable absorption, tissue distribution and renal tubular reabsorption. At doses of 200 to 2500 mg/day the plateau in pre-dose plasma concentrations was largely due to apparent saturation of tissue uptake. The tissue stores fill and stay full, so intake beyond that point mostly leaves in the urine. This is the mechanistic reason grams-per-day dosing does not keep raising vitamin C status the way the dose suggests.

The Cochrane review of vitamin C for the common cold (Hemila 2013) pooled 29 trial comparisons in 11,306 participants and found a risk ratio for developing a cold of 0.97 (95% CI 0.94 to 1.00) in general-community trials involving 10,708 people. That confidence interval sits against no effect, so regular supplementation does not lower the chance of catching a cold in the general population. The reviewers concluded that routine vitamin C supplementation to prevent colds is not justified for most people. This is the finding that most directly contradicts the popular belief that a daily dose builds a shield against colds.

A double-blind randomized trial (Audera 2001) assigned 400 healthy volunteers to vitamin C at 0.03 g (the low-dose comparator), 1 g, 3 g, or 3 g with additives, taken at the onset of a cold and for the following two days. Across 184 cold episodes there were no significant differences in any measure of duration or severity between the groups. If anything the low-dose group had numerically the shortest symptoms. This is the direct test of the popular practice of loading up on vitamin C when you feel a cold coming on, and it did not work.

Deficiency results in impaired immunity and higher susceptibility to infection, and infection in turn drains vitamin C. This explains why correcting a shortfall restores immune function, and why the popular idea of vitamin C as an immune booster holds up as a description of adequacy, not of megadosing a replete body past normal.

Cancer Risk And Outcome

Vitamin C supplements show no reliable cancer-survival benefitEmerging · mixed

A systematic review of trials of vitamins C and E and cancer survival (Mohseni 2022) found the association inconsistent across studies, without a reliable survival benefit emerging. Separately, the interest in vitamin C for cancer centers on intravenous pharmacologic dosing, which reaches blood concentrations that oral intake cannot and is being studied in early trials; that line of work is investigational and should not be read as support for taking oral vitamin C supplements to treat or prevent cancer. Oral vitamin C and intravenous vitamin C are different interventions and the evidence for one does not carry to the other.

How It Works

Vitamin C works by giving up electrons. That makes it useful in two ways: a cofactor that lets specific enzymes run, and an antioxidant that neutralizes reactive molecules. The enzyme role is the load-bearing one. The same electron-donor chemistry changes how iron behaves in the gut, so more of the iron in plant foods gets absorbed alongside it.

Anatomy of the Practice

1Building collagen and tissue

Vitamin C is a required cofactor for the enzymes that hydroxylate collagen, the protein that gives skin, blood vessels, bone and gums their structure. Without enough, collagen is made poorly, vessels and gums become fragile, and wounds heal slowly. This is the mechanism behind scurvy, and it is why the deficiency use is the one that is beyond argument.

2Supporting immune defense

Vitamin C concentrates in white blood cells, especially neutrophils, and supports the skin barrier and helps them engulf, kill, and clear microbes. Being deficient weakens these defenses. Topping up someone who is already replete does not raise immunity above normal. The immune system runs on it, but past the amount it needs, extra does nothing.

3Why the extra is excreted

Absorption in the gut and uptake into tissues are saturable. Below the daily requirement the body holds onto vitamin C efficiently. Past a few hundred milligrams a day the tissues are full, blood levels stop rising, and the rest leaves in urine. That physiology is why gram-a-day doses give little added return.

Getting It Right

Food is the whole strategy for almost everyone. A supplement earns its place only when the diet is short, and grams a day belong to a specific clinical reason.

The dose that helps is small, around 100 to 200 mg a day. The rule underneath all of it is that more is not better.

Get it from food first$Easy

Citrus fruit, kiwi, strawberries, bell peppers, broccoli, Brussels sprouts, tomatoes and potatoes are all rich sources. A single orange, or a serving of peppers or broccoli, covers most of a day's need. A mixed diet of produce covers it on its own, and food brings fiber and other nutrients a pill leaves out.

Pair vitamin C foods with plant iron if your iron runs low$Easy

Vitamin C boosts absorption of the non-heme iron in plants when eaten in the same meal. A squeeze of lemon over lentils or spinach, or fruit with an iron-fortified breakfast, is a free way to get more iron from that food. Time it with the iron-containing food, since the effect happens at the meal. It is most useful for people on plant-based diets or with low iron. People with iron overload should be careful for the same reason.

Add a modest supplement only if your diet is short$Easy

A small daily supplement helps a few groups: people who eat very little produce, some older adults, and those whose health limits how much they absorb. Heavy smokers need about 35 mg more a day than others. For everyone else, an ordinary diet already supplies the requirement.

If you already take vitamin C daily, the small duration benefit comes from that continuous habit, so carry on. Starting grams of it once a cold begins did not shorten or soften colds in a randomized trial. Fluids, rest and time do more.

Grams a day give nothing back, and they are not free of risk. High-dose supplements raise kidney-stone risk in men who already form oxalate stones. A clinician can advise where a specific reason exists.

The Chinese Medicine View

Vitamin C was isolated and identified only in the 20th century, so it never entered the classical Chinese materia medica. It has no channel, no assigned temperature or flavor, and no traditional preparation. Even so, two of the tradition's categories describe functions that overlap with the vitamin's.

Wei Qi, the defensive Qi that guards the body's surface, is the tradition's name for frontline defense. Vitamin C works in the same immune cells and skin barrier that Wei Qi is said to defend. The flesh, the blood vessels, and the healing of wounds belong to the Spleen and to the movement of Blood. Many of the richest vitamin C foods are the sour, fresh plant foods the tradition already prized.

A practitioner would not call vitamin C a tonic in any classical sense. In the tradition a substance earns its place by what it moves or warms or supports in the body, and an isolated vitamin fits none of those roles.

Vitamin C supplements about doubled kidney-stone risk in men

A prospective study of 48,850 men in the Cohort of Swedish Men followed for 11 years (Thomas 2013) found that men who reported taking ascorbic acid supplements had an approximately two-fold higher risk of incident kidney stones, with the commonly reported adjusted hazard ratio near 1.9, compared with non-users. No such association was seen for multivitamin users, consistent with the much smaller vitamin C dose in a multivitamin. The mechanism is plausible: vitamin C is partly metabolized to oxalate, a component of the most common kidney stones. High-dose vitamin C can trigger dangerous hemolysis in G6PD deficiency

A case report and literature review (Wang 2024) described two colorectal cancer patients who developed severe hemolytic anemia after high-dose vitamin C treatment at 1 g/kg, with hemoglobin falling below 50 g/L. Reviewing prior cases, the authors found that all reported patients with G6PD deficiency developed hemolysis after high-dose vitamin C, and concluded G6PD deficiency should be considered a contraindication to it, alongside caution in bone-marrow suppression, moderate-to-severe anemia and impaired liver or kidney function. The doses involved are far above anything taken orally, but the mechanism is why anyone with G6PD deficiency should be careful with large vitamin C doses.Wang 2024, World J Clin Cases

High-dose vitamin C made two of three glucose meters read falsely high

A laboratory evaluation (Katzman 2021) tested ascorbic acid across the range from standard to the very high concentrations used in some cancer protocols on three hospital glucose meters. For two of the three devices, rising ascorbic acid caused false elevations in the glucose reading; the third gave error messages above a threshold, not a wrong value. The interference is strongest at the high concentrations reached with intravenous dosing, but it is the reason a person on large vitamin C doses, or their clinician, should know the reading can be unreliable and should not adjust insulin on a suspect value.Katzman 2021, J Diabetes Sci Technol

Kidney stones in susceptible people

Vitamin C is partly converted in the body to oxalate, a building block of the most common kidney stones. In a large study of men, those taking high-dose vitamin C supplements developed stones about twice as often as non-users, while multivitamin users did not. If you have a history of oxalate kidney stones, get your vitamin C from food, hold supplements to modest amounts, and raise it with your clinician. The vitamin C in food is not the concern.

Iron overload: hemochromatosis

Because vitamin C increases iron absorption and helps mobilize stored iron, high doses can worsen iron overload. Anyone with hereditary hemochromatosis or another iron-overload condition should avoid vitamin C supplements. Do not deliberately pair large doses with iron, and follow the clinician managing your iron.

G6PD deficiency

In people with G6PD deficiency, very high doses of vitamin C can trigger severe red-blood-cell breakdown. The risk is greatest with the intravenous gram-per-kilogram doses used in some settings. If you have G6PD deficiency, treat large vitamin C doses as something to avoid and to discuss with your clinician. Ordinary dietary and modest supplemental amounts are not implicated.

It can throw off a glucose meter, and upset the stomach

High concentrations of vitamin C can make some blood-glucose meters read falsely high. If you take large doses and monitor glucose, treat a reading as unreliable and do not adjust insulin on it. Doses into the grams commonly cause loose stools, stomach cramps, and nausea. These settle once the dose drops. If a test or a medication decision depends on a glucose reading, tell whoever orders it that you take vitamin C.

Common Questions

Does vitamin C prevent colds?

For everyday colds, no. A daily habit does not change how often you get sick in ordinary life. The single exception is people pushed to a physical extreme. Even there, the benefit came from taking it daily before the strain; starting it after symptoms appeared did nothing.

Does it help once I have a cold?

Not once symptoms have started. The small shortening seen with regular use comes only from a dose already in your system day to day. Beginning it at the first sign is too late to matter. Once a cold is underway, a large dose does not change its course.

How much do I need, and can I get it from food?

Yes, easily. The daily need is small, and common foods clear it well before you reach it, so most people never need a pill. Only a few groups are the exception.

Can high doses cause kidney stones?

Only the high-dose supplement form, and mainly in men already prone to oxalate stones. Vitamin C from food does not carry that risk. The concern is high-dose supplements, 1,000 mg or more, not the amount on your plate or in a multivitamin.

Does vitamin C prevent or treat cancer?

Not from an ordinary supplement. Swallowed vitamin C has not been shown to improve cancer survival, and the trials disagree with one another. High-dose intravenous vitamin C is a separate therapy: given by drip, it reaches blood levels no pill can, and it is still being tested in early hospital trials.
