Green tea is a pleasant daily drink with a few small, well-documented effects. Drunk regularly it lowers total cholesterol by about 7 mg/dL and systolic blood pressure by about 2 mmHg, both modest and both measured in short trials. Its most distinctive quality is that the leaf carries L-theanine and caffeine together, a combination that lifts attention while keeping it calm. The bigger claims are softer than the marketing: cancer prevention rests mostly on population studies, weight loss is minor, and blood sugar barely moves.
The one catch is the concentrated green tea EXTRACT, which has injured livers at high doses, while the brewed drink has not. Matcha is the same leaf, whole and stronger. Here is where each claim actually stands.

What It Is
Green tea is the leaf of Camellia sinensis, the same plant as black tea, dried without the oxidation that turns the leaf dark, which is why it keeps more of its pale green catechins. Those catechins, the chief of them EGCG, are the compounds most of the health research tracks. The leaf carries two others: caffeine and L-theanine, the amino acid behind tea's steady, calm lift. Matcha is the whole leaf ground to powder and whisked into water instead of steeped and strained, so a cup of it delivers more catechins and more caffeine than an ordinary brew.
Sold apart from the drink, the catechins are packed into concentrated green tea extract capsules, and that concentration is where the safety picture changes. A mug of green tea and a high-dose extract capsule are not the same exposure.
What It Does
Green tea's effects split by how well the evidence holds.
The dependable ones are small. Pooled across randomized trials, daily green tea lowers total cholesterol by about 7 mg/dL and systolic blood pressure by about 2 mmHg, with a larger drop in people who started high. The calm-alertness from the leaf's L-theanine and caffeine is quicker and just as measurable, clearest in the first hour or two after a cup. Trials can test these directly, which is why they are the claims to trust.
The bigger reputations rest on weaker ground. Cancer prevention comes mostly from population studies, which cannot separate the tea from the healthier lives of the people who drink it. Weight loss, measured over months, is small and not clinically important. Blood sugar barely moves. In one large Japanese cohort the heaviest drinkers had less heart-disease death and no less cancer death, an association and not a proven effect of the leaf.

Longevity And Mortality
All-cause and heart-disease death lower in the heaviest drinkers (women HR 0.77 and 0.69), no link to cancer death
In a large Japanese population, people who drank the most green tea were less likely to die of heart disease and of any cause, but no less likely to die of cancer.
Prospective cohort followed up to 11 years for all-cause and up to 7 years for cause-specific mortality (Kuriyama 2006). The inverse associations were stronger in women. This is an observational association, not a trial, and green tea intake in Japan tracks with other habits.

Weight And Fat Loss
Green tea did not produce meaningful weight loss (−0.09 lb (−0.04 kg) outside Japan)
Green tea leads to very little weight loss; in the more comparable non-Japanese trials the average change was essentially nothing.
Cochrane review of RCTs of at least 12 weeks (Jurgens 2012). Trials outside Japan pooled to essentially no effect; Japanese trials were too heterogeneous to pool and showed larger but inconsistent losses. The theanine and caffeine in tea give a small, short-lived lift to attention and alertness, most reliably when taken together as they are in the leaf.

Fasting glucose and HbA1c: down 1.6 mg/dL (0.09 mmol/L) and 0.30% in one meta-analysis, unchanged in two others
Green tea barely moves blood sugar. The largest review found tiny drops in fasting glucose and HbA1c, while two later reviews in people at risk of or living with type 2 diabetes found no change.
Liu 2013 pooled green tea beverages and extracts across healthy and at-risk adults and found small but statistically significant reductions in fasting glucose and HbA1c, with a fasting-insulin reduction only in the higher-quality subset. Wang 2014, restricted to people at risk of type 2 diabetes, and Li 2016, restricted to people with established type 2 diabetes, each found no significant change in fasting glucose or HbA1c, so the small pooled signal does not hold in the groups who would most want it.

No clear sign green tea prevents cancer across the trials
Despite the reputation, the trials do not show that green tea prevents cancer; most of the encouraging findings come from population studies that cannot prove the tea was responsible.
Cochrane systematic review (Filippini 2020) of observational and interventional studies; results varied by cancer site and study design, and the interventional evidence was sparse. The Ohsaki cohort (Kuriyama 2006) found lower cardiovascular but not cancer mortality, reinforcing that the cancer signal is weak.

How it works
24-hour energy burn raised 4% by catechins plus caffeine
Green tea catechins plus caffeine raised the body's calorie burn by a few percent over a day, more than caffeine alone accounted for.
Randomized crossover respiratory-chamber study in 10 healthy men (Dulloo 1999). The extract also raised urinary norepinephrine, suggesting sympathetic activation. This is a small mechanistic study; it demonstrates the thermogenic route, not clinically meaningful weight change.

How It Works
Green tea's small effects run through its catechins, EGCG chief among them, acting on the blood vessels and on metabolism. In the vessel lining the catechins help keep nitric oxide available, the signal that relaxes the artery wall, which is the credible route to the small blood-pressure drop, and they lower how much cholesterol the gut absorbs, which is the route to the lipid change. The caffeine and catechins together raise energy expenditure and fat oxidation by a few percent, the mechanism behind the weight and thermogenesis claims. Both routes are consistent and both are small.
The calm-alertness works by a separate route. L-theanine reaches the brain within about an hour and shifts the balance toward calm while the caffeine raises arousal, so the two do not cancel, and on an EEG the combination shows up as more alpha activity, the marker of a mind relaxed and awake at once. One distinction runs through all of it: the cup and the capsule are not the same exposure. A mug of green tea delivers perhaps 50 to 100 mg of catechins, matcha somewhat more, and a concentrated extract capsule can carry several hundred milligrams of EGCG in a single dose. Taking that capsule on an empty stomach makes the exposure larger still.
The cardiovascular and calm-alertness benefits belong to ordinary drinking amounts; the documented liver risk belongs to the high-dose extract, not the cup.
Reading a benefit measured in tea drinkers as a reason to swallow a strong extract is how this evidence gets misused most.

3Over years
Whether the drink adds up to less disease and longer life has been watched in large populations, where regular drinkers have less heart disease. That is an association and not a proven effect of the leaf; the cancer signal is weaker still, and the long-term payoff is less certain than the quick numbers suggest.
How to Drink It
Ways to Do It
The benefit belongs to the drink, so the ladder starts free and stays close to the leaf. The extract comes last, and it is the one rung that carries a safety caution.
This is the food-first option and the amount behind almost all the evidence. Two or three cups spread through the day supply the catechins, the L-theanine and a gentle amount of caffeine, and it is the intake that tracks with the cardiovascular findings. Steep at slightly below boiling for a minute or two; water that is too hot makes it bitter without adding benefit. If you rely on plant iron, drink it between meals so it does not blunt iron absorption.
Matcha is the whole leaf ground and whisked in, so you swallow the leaf instead of straining it out, and a serving carries more catechins and more caffeine than a steeped cup. It is a reasonable way to get a stronger dose from the drink itself. Treat the extra caffeine seriously if you are sensitive to it or drinking it late.
Green tea is one source of flavan-3-ol catechins among several; cocoa, apples and berries carry relatives of the same family. A varied diet covers the compound class without leaning on any single source, and it is the diet pattern the population studies described.
A concentrated green tea extract is the one way to take EGCG far above what the drink provides, and it is the one option with a documented liver risk. If there is a specific reason to use it, take it with food, keep the daily EGCG modest, avoid it if you have any liver problem, and treat it as a decision to make with a clinician. The Chinese Medicine View
Tea is one of the few subjects on these pages that the Chinese tradition reasoned about for well over a thousand years. 綠茶 lǜ chá, green tea, has a settled place in Chinese dietary and medical thought, and the fair reading holds long use and the modern trials side by side without letting either stand in for the other.
Tea is classed as cool in nature, bitter and sweet in flavor, entering the Heart, Lung, and Stomach. Its recorded actions read close to what a modern drinker wants from it:
- clears heat, and clears the head and the eyes
- lifts and settles the mind, so thinking is sharp without agitation
- transforms phlegm
- quenches thirst
- promotes urination
- helps resolve food stagnation after a rich meal
The clearing-the-head quality, an unclouded alertness, is the traditional description that sits nearest to the calm-alertness that L-theanine and caffeine produce. The closeness is there, and it does not make the old account a prediction of the chemistry: the tradition reasoned about a leaf, not a catechin, and reading it as if it anticipated EGCG would be a mistake.
The tradition is at its most useful in saying plainly that tea does not suit everyone. Its cool, downward-moving nature is wrong for someone who runs cold, and its bitterness and downward action can burden a weak digestion. A person with a cold, damp constitution or a delicate stomach is who the tradition would tell to go easy, to keep the tea weak, and never to drink it strong on an empty stomach. Drunk to excess or late, a cooling stimulant is understood to unsettle the Shen, the spirit that should quiet at night.
Two descriptive languages are laid over the same leaf, and the instinct they share is useful: the right amount depends on the person, not on a fixed daily dose. If you work with a practitioner, this is a reasonable thing to ask about.

Liver injury from concentrated extract, not from the drink
This is the least-advertised risk here. A United States Pharmacopeia review of the case reports and trials concluded that high-dose green tea EXTRACT can cause liver injury, with reported EGCG amounts running from about 140 mg to roughly 1,000 mg a day and susceptibility differing widely between people, and that taking it on an empty stomach raises the exposure. In the Minnesota Green Tea Trial, 6.7% of women on 843 mg of EGCG daily had raised liver enzymes against 0.7% on placebo. The brewed drink is not implicated. If you use an extract, take it with food, keep the dose modest, avoid it entirely if you have a liver problem, and stop and see a clinician if you notice abdominal pain, dark urine, or yellowing of the skin or eyes.
The caffeine, and more of it in matcha
Green tea carries caffeine, and matcha carries more because you consume the whole leaf. That is enough to affect sleep, reflux, anxiety or heart rhythm in people it applies to, and enough to matter if you are already limiting caffeine. Iron absorption at meals
The tannins in tea reduce how much non-heme iron the gut absorbs from plant foods, which matters mainly for people who are iron-deficient, anemic, or relying on plant iron. The simple fix is to drink tea between meals instead of with them, and to keep it away from an iron supplement.
Medications and blood thinners
Green tea contains vitamin K and its catechins can affect how some drugs are handled, so if you take warfarin or another anticoagulant, or a medication with a narrow margin, keep your intake steady and mention it to the clinician who manages the drug. A concentrated extract is more likely to interact than the occasional cup.
Pregnancy, breastfeeding and high-dose use
In pregnancy the usual advice is about tea’s caffeine, and moderate drinking is generally within limits, but a concentrated green tea extract is a different matter and is not advisable without a clinician’s say-so. For any high-dose or supplement use, in pregnancy or alongside other treatment, weigh it with someone who knows your situation instead of self-starting.

Only a little. Barely, and not dependably. The largest meta-analysis, 17 trials in 1,133 people, found fasting glucose down 1.6 mg/dL (0.09 mmol/L) and HbA1c down 0.30%, both small. Two later reviews found no change at all: one in people at risk of type 2 diabetes and one in people who already have it, the groups who would most want an effect. Green tea is a fine drink for someone watching blood sugar, and not a treatment for it.
