What It Is
Apple cider vinegar is the juice of pressed apples fermented twice, first into a cider and then into vinegar, which is where the sourness comes from. The active ingredient behind the metabolic interest is acetic acid, the same acid in ordinary table vinegar, usually around 5% to 6% in a bottle. The cloudy strands some bottles carry, sold as the mother, are cellulose and bacteria left from fermentation, and they are not the source of the studied effect. The marketing sells it as a fat burner and a cure-all. The evidence supports something narrower: with a carbohydrate meal, vinegar lowers the blood-sugar rise that meal would otherwise produce.
What It Does
The best-supported thing vinegar does is blunt the post-meal (postprandial) glucose and insulin rise when it is taken with a carbohydrate meal. A meta-analysis of small clinical trials found a consistent reduction in the glucose and insulin area under the curve compared with the meal alone, which makes this the most repeatable metabolic effect vinegar has. It works meal by meal, when vinegar comes before the starch, and fades once the meal is digested.

What the Marketing Overclaims
Apple cider vinegar has been sold for weight loss for a long time, and online it spread as a daily ritual credited with far more than the data supports. The weight-loss claim rests largely on one 12-week trial of obese Japanese adults, where 15 to 30 mL of vinegar a day produced about 2–4 lb (1 to 2 kilograms) of loss that drifted back toward baseline after people stopped. That is one small study, in a single population, with changes of a kilo or two that did not last.
A 2024 randomized trial in Lebanese adolescents and young adults circulated widely as fresh support, then was retracted by the journal in 2025 over statistical and data concerns, so it carries no weight either way. What is left is a modest single-study signal and a plausible mechanism. On its own that does not support expecting weight loss from a spoonful of vinegar.
The detox and cure-all framing is thinner still. Vinegar does not flush toxins, dissolve fat, or reset anything, and there is no sign of the sweeping benefits the marketing attaches to it. The one defensible use is the post-meal blood-sugar effect above. Reading that reliable acute effect as proof of a weight-loss cure is the most common way this evidence gets stretched.
How It Works
The mechanism is acetic acid, and it appears to act in more than one place. In the gut, acetic acid slows how fast the stomach empties and dampens the enzymes that break starch into sugar, so a given plate of carbohydrate is absorbed more gradually and the blood-sugar peak is flatter. Beyond the gut, it may increase glucose uptake into muscle and raise fat breakdown, pathways mapped partly in animal and cell work. A plausible mechanism is only a starting point. In people, the post-meal glucose and insulin rise does fall, and it does so repeatably.

Ways to Do It
If you want to use it, match what you take to what you want from it. The food-first version costs nothing and carries no risk. The pre-meal drink is where the glucose evidence sits, and it goes into water with a carbohydrate meal, never neat. The pills give the least active ingredient and carry a swallowing risk of their own.
Splash cider vinegar on salads, cooked greens or beans as ordinary seasoning. This gives you the sour flavor and a little acetic acid inside a meal, with none of the dental or throat risk that comes from drinking it neat. For most people this is the sensible amount and the fair starting point.
Stir one to two tablespoons (15 to 30 mL) of cider vinegar into a large glass of water and drink it just before a carbohydrate-heavy meal. This is the setup and the dose behind the post-meal glucose trials. Always dilute it; undiluted the acid is hard on tooth enamel and the throat, so sip it through a straw and rinse with plain water afterward.
Nearly all of the harm from vinegar falls on the enamel and the throat. Always dilute it well, drink through a straw, rinse with water, and wait at least thirty minutes before brushing so you are not scrubbing acid-softened enamel. Anyone with reflux, a hiatus hernia or a history of throat trouble should skip the drink.

Buy an ordinary bottle of cider vinegar and measure one to two tablespoons into water before a carbohydrate meal. This is the exact intervention the trials used, it costs pennies a serving, and you control the dilution and the freshness.
Cider vinegar gummies and capsules are the packaged option, and the weakest one. A gummy carries very little acetic acid next to a spoon of the liquid, often with added sugar, and swallowed tablets have lodged in the throat and burned it. You pay more for less of the active ingredient.

The Chinese Medicine View
Chinese medicine does know vinegar, which sets it apart from many modern supplements. Vinegar (cu) is a recognized substance in the tradition, sour in flavor and warm in nature, entering the Liver and Stomach. It is used to move Blood and break up stasis, to astringe and to stop some kinds of bleeding, and to relieve certain kinds of food stagnation, classically in small culinary and medicinal amounts.
Vinegar is also a classic processing agent: herbs fried in vinegar are understood to be guided more strongly into the Liver channel. Apple cider vinegar is a Western product, and the tradition describes rice vinegar, so any reading here is interpretation laid over the modern item, held apart from the trial data above.
The lens the tradition offers is the sour flavor itself. Sour is astringent and gathering: it draws in and holds, which is why it is turned to for leakage and for softening the movement of Blood, and why in excess it is understood to over-constrain. A large sour drink is read as taxing to the Spleen and Stomach, so someone with a weak or cold digestion, loose stools, or a sensitive stomach is exactly the person the tradition would tell to go gently or leave it alone, and a big sour drink on an empty stomach fits that warning. Classical dietary texts also treat too much sour as something that can, over time, bind and stiffen the sinews.
The two views sit near each other, without merging, on amount and on the person. A tradition that treats a strong sour agent as something to match to the constitution would not hand everyone the same daily glass, any more than it hands everyone the same herb. It asks who is in front of you before deciding whether vinegar helps or burdens them. That instinct, that the right amount depends on the person and the digestion, aligns with a modern reading where the benefit is modest, the dose matters, and the acid does measurable harm to teeth and throat. Reflux, hiatus hernia and swallowing trouble
