Medicinal mushrooms are sold as one category, and the evidence behind each is very different. Turkey tail has the most: a bound polysaccharide called PSK, used for decades in Japan as an addition to cancer chemotherapy, where meta-analyses of thousands of patients show a survival gain. Cordyceps has a handful of small, mixed exercise trials. Reishi has a low-quality signal as a cancer adjunct and a thin, mostly traditional case for sleep and calm.
Chaga sits almost entirely at animal and test-tube work, so it is the least established of the four. The shared idea is beta-glucan fibers that engage the immune system. What They Are
Medicinal mushrooms are a group of fungi sold as capsules, powders and extracts for the immune system, energy, calm and cancer support. Four are usually grouped together here, and this page covers them because the site has a separate page for lion's mane and its cognition research. Reishi (Ganoderma lucidum, Ling Zhi in Chinese medicine) is the classical tonic mushroom, dried and simmered for centuries. Cordyceps (Dong Chong Xia Cao, the caterpillar fungus, now mostly the cultivated strain Cs-4 or the species Cordyceps militaris) is the traditional lung and kidney tonic. Turkey tail (Trametes versicolor, Yun Zhi) is a bracket fungus that became the source of two licensed cancer drugs in Asia. Chaga (Inonotus obliquus) is a birch fungus from the northern forests, popular now and studied least.
These four do not share an evidence base:

- Chaga has laboratory and animal work and almost nothing in people.
Reading a claim proven for one as if it applied to all four is the main way this category gets oversold.
What The Evidence Shows
Each mushroom has been studied to a different depth, so a result for one says nothing about another. The four below run strongest first.
Turkey tail has the strongest evidence. Its purified polysaccharide PSK has been used in Japan since the 1980s as an addition to cancer chemotherapy. A meta-analysis of eight randomized trials and 8,009 patients found that adding PSK after surgery for gastric cancer improved survival, with a hazard ratio of 0.88 (Oba 2007). A later network meta-analysis of 23 trials and more than 10,000 patients with gastrointestinal cancers reached the same direction, with the gain concentrated in gastric and colorectal cancer when PSK was combined with chemotherapy (Ma 2017). A systematic review of turkey tail for lung cancer found a favorable signal too, though it leaned more on non-randomized studies (Fritz 2015).
One fact belongs alongside this, the strongest positive on the page. PSK is not a supplement. In Japan it is a licensed prescription drug called Krestin, manufactured by the Kureha Corporation and approved there since 1977, and the survival trials come almost entirely from that home market. What is sold as a turkey tail capsule elsewhere is a different, unstandardized product.
This survival benefit is specific. It comes from turkey tail extract added to chemotherapy, mostly in Asian trials. A small phase 1 trial in women recovering from breast cancer radiotherapy found that turkey tail raised immune cell counts and natural killer cell activity, an early immune signal, not a survival result (Torkelson 2012).
Turkey tail's survival data are for an extract added to chemotherapy under medical care. It is never a replacement for cancer treatment.
Cordyceps has small, mixed exercise data. A 12-week trial of the cultivated Cs-4 strain in 20 healthy older adults improved metabolic and ventilatory thresholds, markers of aerobic capacity, though VO2max itself did not move (Chen 2010). A three-week trial of a mushroom blend containing Cordyceps militaris in 28 younger adults improved VO2max and time to exhaustion after three weeks but not after one, and because it was a blend the cordyceps cannot be credited alone (Hirsch 2017). Across both, cordyceps shows a possible edge in exercise tolerance from small studies that do not fully agree, so keep expectations modest.
Reishi has a weak cancer signal and a thin sleep case. The Cochrane review found that patients who added reishi to chemotherapy or radiotherapy were more likely to respond to treatment and reported better quality of life, and that it raised some immune measures, but it graded the trials as low quality and concluded reishi could serve as a possible adjunct, not a first-line treatment (Jin 2016). The traditional use of reishi for sleep and calm is long, but the modern trials for those specific uses are few and small, so that claim rests mainly on the tradition, covered below.
Chaga is the least established. Its anticancer and antioxidant activity has been shown in cell and animal studies, and the human trials that would tell us whether any of it carries into people have not been done. On current evidence it is unproven in people.

Turkey tail PSK added to chemotherapy after gastric cancer surgery improved survival, hazard ratio 0.88 across 8,009 patients
When a turkey tail mushroom extract was added to chemotherapy after stomach cancer surgery, slightly more people were still alive later than with chemotherapy alone.
Pooling eight randomized trials of 8,009 patients, adding the turkey tail polysaccharide PSK to chemotherapy after curative gastric cancer surgery improved overall survival with a hazard ratio of 0.88 (95% CI 0.79 to 0.98). A later network meta-analysis of 23 gastrointestinal-cancer trials and more than 10,000 patients found the same direction, strongest in gastric and colorectal cancer when PSK was combined with chemotherapy. Measured in: Adults with resected gastric and other gastrointestinal cancers, mostly in Japanese trials. The benefit is for a standardized extract added to chemotherapy, mostly in Asian trials, not for turkey tail taken instead of cancer treatment, and the pooled trials vary in method and era.

Turkey tail PSK looked favorable for survival and symptoms in lung cancer across 28 studies, mostly weaker designs
In lung cancer, adding turkey tail extract looked helpful for survival and for symptoms like fatigue, though many of the studies were weaker in design.
A systematic review of 28 studies of PSK and other Trametes versicolor extracts for lung cancer found favorable effects on survival, immune parameters, performance status and tumor-related symptoms such as fatigue and anorexia, alongside preclinical support for immunomodulation. Measured in: Lung cancer patients across randomized and non-randomized trials. The favorable signal leaned heavily on non-randomized controlled trials and preclinical work, not on large randomized trials, so it is weaker than the gastric cancer data.

Turkey tail up to 9 grams a day raised immune cell counts and natural killer activity in a phase 1 breast cancer trial
In a small early trial, women recovering from breast cancer treatment who took turkey tail showed signs of a stronger immune response.
In a phase 1 dose-escalation trial, women recovering from breast cancer radiotherapy who took up to 9 grams a day of a Trametes versicolor preparation for six weeks showed trends toward higher lymphocyte counts and increased natural killer cell activity, and the preparation was well tolerated. Measured in: Nine to eleven women recovering from breast cancer radiotherapy. This was a safety and dose-finding study of nine completing participants with no control group, measuring immune markers, not any survival or clinical outcome.

Cordyceps Cs-4 raised aerobic thresholds 10.5% in 20 older adults, with no change in VO2max
Older adults who took cordyceps for three months could exercise a bit harder before tiring, though their peak oxygen use did not improve.
In a 12-week double-blind placebo-controlled trial of 20 healthy adults aged 50 to 75, cordyceps Cs-4 at 1 gram a day raised the metabolic threshold by 10.5% and the ventilatory threshold by 8.5%, both markers of aerobic capacity, while VO2max did not change in either group. Measured in: Twenty healthy adults aged 50 to 75. A pilot trial of only 20 people in which the main measure of fitness, VO2max, did not change, so the effect is small and needs larger confirmation.

Anatomy of the Practice
1The shared ingredient: beta-glucan
The compound the four have in common is beta-glucan, a fiber in the mushroom cell wall. Immune cells carry receptors that recognize these fibers as the marker of a fungus, and binding them switches those cells toward activity. This is the mechanism behind the immune claims, and it is why a product that states a measured beta-glucan content tells you more than one that does not.
2From gut to immune system
Beta-glucans are not digested like ordinary starch. They pass to the gut-associated immune tissue and to the immune cells lining the gut, which is where much of the signaling is thought to begin. This route is worked out mostly in cells and animals. It shows how the effect could happen; it does not prove a clinical result.
3Where the four diverge
Turkey tail PSK was purified and tested as an addition to chemotherapy, so its data are about survival in cancer patients. Cordyceps was studied for oxygen use in exercise. Reishi carries the immune and quality-of-life work plus its sleep tradition. Chaga has been taken mostly as far as animal models. It is the same fiber family, studied to very different depths in people.
The immune mechanism is the one most of the marketing rests on. Beta-glucans from the mushroom cell wall bind receptors on immune cells such as macrophages, natural killer cells and dendritic cells, and shift them toward activity. Turkey tail PSK, the most studied preparation, is a protein-bound beta-glucan, and the working theory for its cancer results is that it helps the immune system recognize and act against tumor cells while chemotherapy acts separately (Chan 2009). That mechanism is established in the laboratory. Whether it changes an outcome that matters has to be measured directly in people, and only turkey tail has been measured that far.
What is actually in the product varies across every species. The active material sits in the fruiting body, the mushroom itself, and in cultivated mycelium, the branching network usually grown on grain. Many cheaper supplements are mycelium dried together with the grain it grew on, which can leave a capsule that is largely starch, with little mushroom and little beta-glucan. A product that names the fruiting body, or states a measured beta-glucan percentage, shows there is mushroom inside. Check that before choosing between species.
Choosing and Using Them
Ways to Do It
Match the mushroom to the goal, and read the Cautions section first, especially if you take a blood thinner or an immune-suppressing drug. For a healthy person, food-grade mushrooms and a decent diet come before any capsule. The higher rungs are specific and, in the cancer case, belong with an oncologist, not with self-treatment.
The fair starting point for most people. Shiitake, maitake, oyster and enoki mushrooms carry the same beta-glucan family and are food, not a supplement. This will not deliver the concentrated turkey tail dose used in cancer trials, and it is a sound, low-cost way to get the fiber and eat well while you decide whether any capsule is worth it.
If you want a supplement, the practical buy is a fruiting-body extract from a brand that gives a measured beta-glucan percentage, not just a milligram weight of powder. That number is the closest thing to a quality signal on the shelf. Reishi and cordyceps are the usual choices for general immune and energy support at this rung.
The exercise and immune signals in the trials built over weeks, not from a single dose, so take it daily and judge it against how you were before you started. If nothing has shifted in the studied window, there is no case for continuing to pay for it.
Mushrooms concentrate metals and other contaminants from where they grow, and chaga in particular can carry a lot of oxalate. A product that carries independent testing for heavy metals and states the result is worth the extra cost, and it is the paid end of the quality ladder. Grain-grown mycelium sold with the grain still in is the thing to avoid.
The survival data are for a standardized turkey tail extract added to chemotherapy under medical care, mostly in Asian trials. This is a decision to make with the oncologist running the treatment, weighed against interactions and the specifics of the cancer, and it is never a replacement for that treatment. It is included here because it is the one mushroom with human outcome data, not as a self-start option.

Immune-suppressing drugs and autoimmune conditions
These mushrooms are taken partly to stimulate the immune system, which is the concern for anyone on an immunosuppressant drug, including after an organ transplant or for an autoimmune condition. Adding an immune stimulant could in theory work against the medication or stir an autoimmune process. If you take such a drug or have an autoimmune condition, treat these as something to clear with the clinician managing it before starting.
Blood thinners, bleeding and surgery
Reishi in particular has been reported to have a mild blood-thinning action, and there are case reports of bleeding at high doses. If you take an anticoagulant or antiplatelet drug, or have surgery coming up, tell your clinician you are taking a mushroom supplement, since it is simple to pause it beforehand.
Discuss with your oncologist, never substitute
The turkey tail survival data are for an extract added to chemotherapy as part of cancer care, not for a mushroom taken in place of treatment. Some mushroom compounds could also interact with chemotherapy or affect the immune system in ways that matter during treatment. Anyone in cancer care should raise any mushroom supplement with their oncologist before starting it.
Product quality and what is in the capsule
Mushroom supplements are loosely regulated and vary a great deal. Many are mycelium grown on grain and sold with the grain still in, which can mean mostly starch and little active mushroom, and the label does not always make this clear. Mushrooms also concentrate heavy metals, and chaga is high in oxalate, which is a concern for anyone prone to kidney stones. A fruiting-body product that states a measured beta-glucan content and carries third-party testing is the simplest way to know what you are getting.
Pregnancy, breastfeeding and allergy
There is not enough safety data on concentrated mushroom supplements during pregnancy or breastfeeding to judge them, so the cautious course is to keep to culinary mushrooms as food during those times. As with any mushroom, allergic reactions are possible, so start with a small amount the first time and stop if you develop a rash, itching or any difficulty breathing.
