Measure: Blood Tests That Change a Decision

My Plan

A handful of blood markers change what you and a clinician decide to do. A few numbers on the standard panel are worth reading closely: ApoB and Lp(a) are among the most useful and the least ordered, and the giant full-body panel sold as catching everything mostly turns up findings that lead nowhere.

This page separates the markers that guide a decision from the ones that mostly generate follow-up, grades each at the strength the evidence supports, and adds the Chinese medicine view of reading the person as well as the number.

What It Is

A blood panel is a set of measurements drawn from one sample, and its value depends almost entirely on which measurements are on it and why. A small, well-chosen set reads back things that change a decision: whether to lower a cholesterol particle, whether blood sugar is drifting, whether the kidneys are under strain before any symptom shows. A large, undirected set, the kind marketed as a full-body or executive panel, reads back dozens of numbers in people with no symptoms, most of which were never going to change anything and some of which trigger a chain of follow-up even when nothing is wrong. Telling the two apart is the whole task, and it costs nothing: there is no product to buy and no test to order here, only the markers the evidence supports measuring and the ones it does not. Each is graded at the strength the research supports.

How to Read a Result

1A Reference Range Is Not the Same as Optimal

A laboratory reference range is simply where most results from a broadly healthy sample fall, usually the middle 95 percent. It is not the level that carries the lowest risk, and it is not a hard line between well and unwell. LDL cholesterol sits well inside the reference range at levels that trials still show benefit from lowering, so a value can read as normal on the report and still call for action.

2One Value Is a Snapshot, Retest Before Acting

Blood markers vary from day to day and with the laboratory, the time, illness, hydration and the last meal. A single number that is mildly off is a reason to retest, not a diagnosis. Many markers, from CRP to a borderline glucose, can move enough between two mornings to cross a threshold, so the trend across repeats carries more meaning than any one draw.

3A Number Is a Marker, Not the Whole Person

A result gains its meaning from context: your age, symptoms, family history, the other markers and the direction it is moving. The same ferritin means one thing in a tired menstruating woman and another in a well older man. A number read alone, without the person around it, is where both false alarms and false reassurance come from.

How It Works

Most useful markers are indirect readings of a process you cannot see. LDL cholesterol and ApoB are two ways of reading the same population of fatty particles that lodge in artery walls: LDL cholesterol measures how much cholesterol those particles carry, while ApoB counts how many particles there are, since each one carries a single ApoB molecule. When someone has small, cholesterol-poor particles, common in insulin resistance and high triglycerides, the two numbers disagree, and the particle count is the better read of risk. Blood sugar markers work the same way. HbA1c reflects the fraction of hemoglobin with glucose bound to it, built up over the red cell lifespan, so it averages roughly the past two to three months in a single draw, without fasting. Fasting insulin, folded together with fasting glucose into a value called HOMA-IR, estimates how hard the body is working to keep glucose in check, and can shift before HbA1c or fasting glucose move. What Changed

The common advice was to test everything every year to catch problems early, on the intuition that more information is always safer. The better evidence has revised that in two directions at once. Broad, undirected panels in people without symptoms have been tested in large randomized trials, and offering them made no measurable difference to how long people lived or to death from cancer or heart disease; the extra tests found more to label and treat without a payoff in outcomes.

At the same time, a few specific markers turn out to be more decision-changing than they are given credit for. ApoB reads lipid risk better than the standard cholesterol number when the two diverge, and Lp(a), an inherited particle that needs measuring only once in a lifetime, identifies people carrying extra heart risk that the usual panel misses entirely. So the change is toward focus: a short list of markers that guide prevention, and restraint about the rest.

The Markers That Change a Decision

Heart and Lipids

Heart and lipid markers are the highest-value group, because the evidence here points to cause, not just correlation, and to actions that lower risk.

  • LDL cholesterol is the anchor. Across 26 randomized trials in about 170,000 people, each 39 mg/dL (1.0 mmol/L) drop lowered heart attacks and strokes by about 22 percent, and genetic, epidemiological and trial evidence together show LDL helps drive arterial disease, not merely accompany it.
  • ApoB refines that reading by counting particles, and is most informative when it disagrees with LDL.
  • Lp(a) is the underused one: largely set by genetics, stable across life, and measured a single time to flag a risk the standard panel does not show.
  • hs-CRP, a marker of low-grade inflammation, adds a modest, independent amount of information, best used to tip a decision near a threshold.
  • Blood pressure is not a blood test, but it belongs in the same conversation as one of the most decision-changing numbers you can track, and reading it correctly at home matters more than almost any lifestyle change made in response. Blood Sugar and Metabolism

HbA1c is the core blood-sugar marker: one draw, no fasting, and a read on the past few months that flags diabetes and prediabetes early enough to act. Fasting glucose adds a same-morning snapshot. HOMA-IR, built from fasting insulin and glucose, is an earlier and softer signal of insulin resistance that can move before the others, though insulin assays are not well standardized, so it is read as a personal trend across repeats rather than against a fixed cut-off. Together these numbers describe where someone sits on the progression from healthy metabolism toward diabetes, which is where most lifestyle change acts. Inflammation

hs-CRP is the practical inflammation marker. It rises with any infection, injury or flare, so a value taken when someone is unwell reflects that illness and not their baseline, and it is repeated when well. Read at baseline, higher hs-CRP comes with somewhat higher cardiovascular risk, independent of the usual factors but modest in size. It sharpens a decision already near a threshold and does not settle one alone.

Organs, Thyroid and Iron

Four more tests are worth ordering when there is a reason to look:

  • eGFR and urine albumin are two kidney tests, one from blood and one from urine. Each independently predicts long-term risk, and a normal eGFR alongside rising urine albumin still marks trouble, which is why both belong on the panel.
  • Liver enzymes (ALT, AST) screen for liver strain, and one raised value is read in context, since on its own it means little.
  • TSH is the right first test when symptoms suggest a thyroid problem, such as fatigue, weight change or cold intolerance; checking it routinely in people who feel well has a weaker case.
  • Ferritin, read with a full blood count, is the best single test for iron deficiency, and its meaning depends heavily on the person: low ferritin in a menstruating woman with fatigue is a different signal from a value in a well older adult, and ferritin also rises with inflammation, which can mask a true deficiency.

What You Probably Do Not Need

One strategy has the clearest evidence against it: the panel sold as peace of mind, dozens of markers ordered at once in someone with no symptoms and no specific reason. Randomized trials of exactly this strategy, pooled across 17 studies and 251,891 people, found it left overall mortality and death from cancer and heart disease unchanged, while increasing the number of diagnoses and prescriptions.

The problem is not the accuracy of the tests. Most abnormal-looking results in a low-risk person are either meaningless variation or findings that would never have caused harm, and each one can lead a healthy person into further tests, scans and procedures that carry their own risk. This is the position captured in the Choosing Wisely campaign and in preventive-services reviews of routine screening: a test ordered for a reason is worth having, a test ordered because it was on the list usually is not.

A marker is worth measuring when a plausible result would change what you do. If no result would change anything, the test generates follow-up, not information.

The Chinese Medicine View

Chinese medicine has no blood marker and does not reach for one, and forcing its categories onto a lab report does justice to neither. What it offers is a different unit of analysis. Where a panel isolates one variable at a time, the tradition reads a pattern, drawing many signs into a picture of a whole person in a particular state:

  • tongue and pulse
  • complexion and sleep
  • digestion, temperature, and energy through the day

A single sign read alone is treated as a fragment, and the classical texts are explicit that one finding out of context can mislead, which matches the modern caution that one abnormal value is not a diagnosis.

The tradition also holds the distinction between root and branch, ben and biao. A number on a report is usually a branch, a manifestation, and the questions a practitioner asks reach for the root behind it: the constitution and the imbalance producing the reading, and why this person in this season shows it. The two frames answer different questions and are strongest kept side by side. A lipid particle count and a diagnosis of, say, Spleen failing to transform Dampness are not translations of one another, and neither confirms the other; each is a way of deciding what to do, from a different angle, about the same person.

Everything to be aware of is here, in one place. This practice suits most healthy people; a few situations call for real care.

A borderline or incidental result sets off cascades of more testing, harm reported by 991 physicians

Ganguli and colleagues surveyed a national sample of 991 US physicians about cascades of care following incidental findings. Almost all had experienced them, and majorities reported resulting harms including physical harm from downstream procedures, psychological harm and anxiety for patients, wasted time and financial cost, and personal frustration. Read Results With a Clinician Who Knows Your History

The markers here gain their meaning from your age, symptoms, family history and the other numbers alongside them. A result is information for a conversation with a clinician who has your full picture, not a verdict to reach on your own.

One Abnormal Value Is Not a Diagnosis

Blood markers vary between days, laboratories and circumstances. A single value sitting a little outside the range is a reason to repeat it before drawing any conclusion, and a diagnosis rests on repeated measurements read in context, not on one surprising number.

Do Not Change Treatment Off a Single Number

A home or one-off reading is a prompt to ask a question, not a basis for starting, stopping or adjusting a medication. Take the results and the trend to a prescriber before acting on a value alone.

Common Questions

Which blood tests are actually worth getting?

The short list is the one whose results would change a decision. For heart risk that means a lipid panel, ApoB where it can be had, and Lp(a) once in a lifetime; hs-CRP refines the picture near a decision point. For metabolism it means HbA1c and fasting glucose, with fasting insulin and HOMA-IR as an earlier signal. Kidney function through eGFR and urine albumin, and thyroid through TSH when symptoms suggest it, round out the set that guides real action. Blood pressure, though not a blood test, belongs alongside them. Beyond that, added tests earn their place for a specific reason, not by default.

Do I need a full-body or executive blood panel every year?

The evidence does not support it for people without symptoms. Across 17 randomized trials and 251,891 participants, offering broad general health checks made no difference to overall mortality or to death from cancer or heart disease, while increasing diagnoses and prescriptions. A focused set of markers chosen for your age and risk, repeated at sensible intervals, carries the information; the dozens-of-tests annual panel mostly adds follow-up.

What is Lp(a), and why is it measured only once?

Lipoprotein(a) is an LDL-like particle whose blood level is roughly 90 percent inherited and stays remarkably stable across adult life. That is why one measurement classifies your risk for good. A high level adds cardiovascular risk on top of your other lipids and is independent of them, which makes it useful, and it is still ordered too rarely. There is not yet a widely approved treatment that lowers Lp(a) and is shown to change outcomes, so a high result is used to sharpen overall risk and to focus attention on the factors that can be changed.

My clinician will not order ApoB or Lp(a). Can I get them another way?

Yes. Both are available through direct-to-consumer lab testing, where you order and pay for the test yourself without a clinician's requisition, which is an option when your own clinician will not order a marker you have reason to want. Lp(a) in particular needs measuring only once, so a single self-ordered test settles it for life. Take any abnormal result back to a clinician who knows your history, since what you do about a high number is a decision to make together.

What is the difference between ApoB and LDL cholesterol?

They read the same population of particles two ways. LDL cholesterol measures how much cholesterol the particles carry; ApoB counts how many particles there are, since each carries one ApoB molecule. When someone has many small, cholesterol-poor particles, common in insulin resistance and high triglycerides, LDL cholesterol can look reassuring while the particle count is high. In that situation ApoB is the better guide to risk. Where the two agree, either number tells you the same thing.

My result came back slightly abnormal. Should I worry?

A single value a little outside the reference range is usually a reason to retest rather than a diagnosis, because markers vary with the day, the laboratory, hydration, recent illness and the last meal. Reference ranges describe where most people fall, not a hard line into disease. The sensible next step is to repeat the test, look at the trend, and read it with a clinician who knows your history and the rest of your numbers, before acting on one reading alone.
