Iron is one of the clearest cases in nutrition of a supplement that works, but only for the people who need it. When your stores are low, iron corrects anemia, restores the blood's oxygen-carrying capacity, and can lift the fatigue of a shortfall, sometimes before anemia even shows. When your stores are already full, the same tablet does little for energy and brings side effects and, over years, the risk of storing too much.
For most people the safe first move needs no test: build your stores from iron-rich foods, with a vitamin C source at the meal to absorb more of them. Before you take tablets, though, test your ferritin, because supplementing when you are not low is where those side effects and the overload risk come from.

What It Is
Iron is the mineral your blood uses to carry oxygen. Most of it is locked into hemoglobin, the oxygen-carrying protein in red blood cells, and a smaller share sits in reserve as a storage protein called ferritin. A simple blood test reads that ferritin reserve, and the result tells you whether a supplement will help you at all.

1Where iron lives
About two-thirds of your iron is in hemoglobin inside red blood cells, carrying oxygen to the tissues. The rest is stored as ferritin, mostly in the liver, spleen and bone marrow.
2When stores run low
The reserve empties first. Ferritin falls while hemoglobin is still normal, so you can be iron-deficient without being anemic, and some people already feel it as fatigue, breathlessness on stairs, or feeling cold. Only when the reserve is exhausted does hemoglobin drop and anemia appear.
3Hepcidin limits absorption
A hormone called hepcidin limits how much iron you absorb from food and tablets. A single dose raises hepcidin for about a day, which reduces how much of the next dose you absorb. This is why spacing doses out gets more iron into you per tablet than taking them close together.
What It Does
Iron does one thing well: it replaces a shortage. The deeper the deficiency, the larger and more certain the benefit, and the cards below run in that order. Full iron-deficiency anemia sits at the top, where the trials are largest and the outcome is a change measured in the blood. Below it comes iron deficiency without anemia, where the gains are smaller and more subjective and rest on fewer trials. Once stores are full, the benefit is gone: when trials exclude the people whose iron was already fine, the improvements in fatigue and mood are no longer there.

Anemia And Iron
Daily iron raises hemoglobin about 5.3 g/L and cuts remaining anemia by about 60%
Taking iron every day fixes iron-deficiency anemia. It raises the blood's oxygen-carrying hemoglobin, leaves far fewer people anemic at the end, and lifts the tiredness and poor stamina that come with a shortfall.
A Cochrane meta-analysis of 67 randomized trials in 8,506 menstruating women (Low 2016) found daily oral iron raised endpoint hemoglobin by a mean of 5.30 g/L (95% CI 4.14 to 6.45; 51 trials, 6,861 women, high-quality evidence) and reduced the risk of anemia (RR 0.39, 95% CI 0.25 to 0.60) and of iron deficiency (RR 0.62, 95% CI 0.50 to 0.76). It improved maximal and submaximal exercise performance and reduced symptomatic fatigue. The same review found iron increased gastrointestinal side effects (constipation RR 2.07, loose stools RR 2.13).
Who this may not transfer to:Correcting iron-deficiency anemia is general physiology and applies to men and postmenopausal women, but this pooled evidence was gathered only in menstruating women.
This is the clearest use of iron: refilling a documented low store corrects the anemia and its symptoms; it is a course to a target, not a permanent daily habit.

Energy And Fatigue
Iron eases fatigue in iron-deficient non-anemic adults, though measured exercise capacity does not change
People who are low in iron but not yet anemic felt less tired on iron, a small-to-moderate improvement. Their measured exercise performance did not change, so the effect was on how they felt more than on what their body could do.
A systematic review and meta-analysis of 18 RCTs in 1,170 iron-deficient non-anemic adults (Houston 2018) found iron reduced self-reported fatigue (SMD -0.38, 95% CI -0.52 to -0.23; I2 0%; 4 trials, 714 participants) and raised hemoglobin (MD 4.01 g/L) and ferritin, but did not improve objective physical capacity, including maximal oxygen consumption (SMD 0.11, 95% CI -0.15 to 0.37; 9 trials) or timed exercise tests.
Who this may not transfer to:Measured in iron-deficient non-anemic adults of both sexes.
If you are iron-deficient without anemia and tired, iron is a reasonable trial for the fatigue itself, but do not expect it to raise athletic or exercise performance, which did not move in these trials.

In non-anemic women, iron cut unexplained fatigue 29% versus 13% on placebo
In non-anemic women with unexplained tiredness and low or borderline iron stores, four weeks of iron eased fatigue more than placebo. Women whose ferritin was already in a healthy range did not benefit.
A double-blind placebo-controlled RCT randomized 144 women aged 18 to 55 with unexplained fatigue to 80 mg/day of elemental iron as ferrous sulfate or placebo for four weeks (Verdon 2003). Fatigue on a 10-point scale fell by 1.82 points (29%) on iron versus 0.85 points (13%) on placebo (difference 0.95, 95% CI 0.32 to 1.62; P=0.004). A subgroup analysis found the benefit was confined to women with serum ferritin at or below 50 micrograms/L.

No benefit for mood, fatigue or cognition once iron stores are already normal
The gains from iron showed up in people who were actually low in iron. When the analysis dropped everyone who was not iron-deficient, the benefit was no longer there, so iron is not a general energy boost for people whose stores are fine.
In the meta-analysis of non-anemic people (Fiani 2025), subgroup analyzes were consistent for iron-deficient participants but found supplementation effects were absent when excluding iron-deficient participants. The signal tracks iron deficiency, not iron intake in general, which is the mechanistic reason to test ferritin before supplementing rather than taking iron for tiredness alone.

How it works
Alternate-day single doses absorb more iron than daily, 21.8% versus 16.3%
Taking iron every other day as a single morning dose absorbed more iron per tablet than taking it daily, and taking it twice a day was no better than once. Spacing doses out absorbs more iron than taking them close together.
Two open-label RCTs using stable iron isotopes in iron-depleted women (Stoffel 2017) found alternate-day dosing produced higher cumulative fractional absorption than consecutive-day dosing (geometric mean 21.8% vs 16.3%, P=0.0013) and greater total absorption (175.3 vs 131.0 mg). A second study found no significant absorption gain from splitting a daily dose into two, while twice-daily dosing raised serum hepcidin. Providing iron on alternate days as single morning doses optimized absorption.

A sixfold larger iron dose adds only threefold more absorbed, and twice-daily adds none
A single iron dose raises the hormone hepcidin for about a day, which blocks the next dose. So doubling up, whether a bigger tablet or a second same-day dose, gives diminishing returns; much of the extra iron is simply not absorbed.
Stable-isotope studies in 54 non-anemic women with ferritin 20 micrograms/L or below (Moretti 2015) found that 24 hours after doses of 60 mg or more, serum hepcidin rose and fractional iron absorption fell by 35% to 45%. Increasing the dose sixfold (40 to 240 mg) raised absolute iron absorbed only threefold (6.7 to 18.1 mg), and total iron absorbed from three doses across a day was not significantly greater than from two morning doses. Lower doses (40 to 80 mg) and avoiding twice-daily dosing maximized fractional absorption.

A vitamin C tablet added to iron did not speed hemoglobin recovery, 2.00 versus 1.84 g/dL
For people being treated for iron-deficiency anemia, adding a vitamin C tablet to their iron did not raise hemoglobin any faster than iron on its own. The iron alone raised it, and the extra vitamin C tablet was not needed.
An open-label equivalence RCT randomized 440 adults with iron-deficiency anemia, 96.8% of them women, to a 100 mg iron tablet plus 200 mg vitamin C or the iron tablet alone every 8 hours for 3 months (Li 2020). Hemoglobin rose 2.00 g/dL with vitamin C and 1.84 g/dL without (difference 0.16 g/dL, 95% CI -0.03 to 0.35), meeting the pre-set equivalence margin; ferritin recovery and adverse-event rates were also equivalent. Vitamin C still aids non-heme iron solubility at a meal, but a separate supplement added nothing clinically here.

Mood & stress
Iron eased anxiety and improved memory in non-anemic iron deficiency, though attention and depression did not shift
Even before anemia sets in, low iron can worsen mood and thinking. Pooling trials, iron eased anxiety and fatigue and modestly improved memory and some cognitive scores, though it did not shift attention or depression.
A meta-analysis of 18 studies (12 RCTs and 6 pre-post) in 1,408 non-anemic people, 1,176 of them menstruating adults (Fiani 2025), found that in the RCTs iron improved anxiety (d=0.34), fatigue (d=0.34), physical well-being (d=0.42), cognitive intelligence (d=0.46) and short-term memory (d=0.53), but not attention or depression. The uncontrolled pre-post studies reported larger effects on depression and fatigue, which that design tends to overstate.

Iron improved restless legs severity about 3.78 points on a 40-point scale
For restless legs syndrome, iron eased the uncomfortable urge-to-move symptoms compared with placebo, a modest improvement on the standard severity scale. It helped whether or not blood iron was clearly low.
A Cochrane meta-analysis of 10 trials (428 participants, followed 2 to 16 weeks) found iron improved International Restless Legs Scale scores versus placebo (MD -3.78 on the 0 to 40 scale, 95% CI -6.25 to -1.31; I2 66%; 7 trials, 345 participants), rated moderate certainty on GRADE (Trotti 2019). Quality of life on continuous scales improved, while sleep quality and periodic limb movements did not clearly change. A single trial found iron comparable to the dopamine agonist pramipexole.

How It Works
Every dose of iron has to cross the gut wall, and how much gets through is set by hepcidin, the hormone that rises after a dose and blocks absorption for about a day. So a large tablet or a second same-day dose gives back much less than its size suggests. The dietary forms differ too. Heme iron, from meat, liver, and shellfish, is absorbed well regardless of the rest of the meal. Non-heme iron, from beans, lentils, tofu, dark leafy greens, and fortified grains, is absorbed less efficiently, and its uptake depends on the meal around it: vitamin C helps it dissolve, while tea, coffee, and calcium at the same sitting cut it down. How to Take Iron
Iron is a course to refill a low reserve; once ferritin is back in range, most people stop. The steps below run in order, and the first one determines whether the rest applies to you at all.
Test your ferritin before you start iron: it helps when your stores are low, and only adds risk when they are already full.
Ways to Do It
A ferritin test comes first. From there, food is the gentle way to build stores, and if you supplement, the dose and timing matter more than the total amount.
A ferritin blood test, usually with a full blood count, tells you whether your stores are low. Iron helps when ferritin is low or borderline and does little when it is already in a healthy range, so this one test settles whether a supplement is worth taking at all. Low iron also has causes worth finding, from heavy periods to gut bleeding. Take a low result as a prompt to look for the cause and to treat it.
Heme iron from red meat, liver, and shellfish is absorbed well and is the most reliable dietary source. Non-heme iron from beans, lentils, tofu, dark leafy greens, and fortified grains is absorbed less efficiently but still counts, especially alongside a vitamin C source such as citrus, peppers or tomatoes. Tea, coffee, and calcium-rich foods at the same meal blunt absorption, so keep them for between meals if you are trying to build stores.
A modest dose, around 60 mg of elemental iron, taken as a single morning dose on alternate days, absorbs a higher fraction per tablet than large or twice-daily dosing and tends to be gentler on the gut. Take it with water or a little vitamin C, and away from tea, coffee, dairy, and calcium supplements. Bigger and more frequent gives diminishing returns: the hepcidin your last dose raised is still blocking the next one.
Refilling iron stores takes weeks to a few months. Recheck ferritin after about eight to twelve weeks and stop, or drop to food, once it is back in range. Iron is not a permanent daily supplement for most people, and staying on it past the point of need is where the risk of storing too much starts.

The Chinese Medicine View
Iron as an isolated mineral, and hemoglobin as a measurable protein, belong to modern chemistry and sit outside the classical Chinese pharmacopoeia. There is no traditional entry for elemental iron, no channel it enters, no flavor or temperature assigned to it by any historical text. What the tradition does have is a pattern that overlaps, clinically, with what a modern clinician would call iron deficiency. This framing is offered as a lens, not as evidence.
The relevant pattern is Blood deficiency, Xue Xu, and its signs track the symptoms of low iron closely:
- a pale or sallow complexion
- fatigue, and dizziness on standing
- palpitations
- poor concentration and memory
- in women, scanty or pale menstruation with a pale tongue
In the tradition, Blood is generated by the Spleen from food and stored by the Liver, so Blood deficiency is treated by strengthening that transformation and by nourishing Blood directly, with foods and formulas such as those built around Dang Gui, Shu Di Huang, E Jiao, and longan, and with the everyday advice to eat liver, red meat, dark leafy greens and dates. Those iron-rich foods are, by modern measure, exactly what rebuilds an iron store.
The lens has a clear limit. Blood deficiency in Chinese medicine is a whole pattern, not a lab value: a person can show it with normal iron, and can be iron-deficient without the full pattern. A practitioner reads the tongue, the pulse and the whole presentation, and would not equate the two. The framing is here to connect a modern mineral to a way of thinking the tradition has used for a long time, and the evidence for iron itself rests on the trials above.

Ferrous sulfate iron causes gut side effects two to three times as often as placebo
A systematic review and meta-analysis of 43 RCTs in 6,831 adults (Tolkien 2015) found ferrous sulfate significantly increased gastrointestinal side effects versus placebo (OR 2.32, 95% CI 1.74 to 3.08) and versus intravenous iron (OR 3.05, 95% CI 2.07 to 4.48). Iron supplements worsen iron overload in hereditary hemochromatosis
Hemochromatosis is systemic iron overload of genetic origin, most often from homozygous C282Y mutations in the HFE gene, caused by reduced hepcidin so the body keeps absorbing iron (Brissot 2018). Iron accumulates in hepatocytes, pancreatic cells and cardiomyocytes, risking cirrhosis, diabetes and cardiomyopathy. The mainstay treatment is phlebotomy, drawing blood to remove iron; supplemental iron works directly against that and accelerates loading. A before-and-after analysis of US poison-control data (Tenenbein 2005), covering the 10 years before and 5 years after the 1997 unit-dose packaging requirement, found iron-ingestion calls for children under 6 fell from 2.99 to 1.91 per 1,000 calls, and deaths from iron poisoning fell from 29 to 1. Do not supplement iron without testing first
Iron helps when your stores are low. Taken when your iron is already fine it does little for energy, commonly causes gut side effects, and over time adds the risk of storing too much. A ferritin test settles whether you need it, and a low result is also a reason to investigate the cause before treating.
Hereditary hemochromatosis and iron overload
Some people are genetically predisposed to absorb and store too much iron. In hereditary hemochromatosis, the most common such disorder, iron builds up in the liver, heart and pancreas, and the treatment is removing iron by regular blood draws. Supplemental iron works directly against that. If you have hemochromatosis or a family history of iron overload, do not take iron unless a clinician directs it.
Keep iron away from children
Iron overdose is one of the leading causes of poisoning death in young children, and a handful of adult tablets can be lethal to a toddler. Keep iron in its child-resistant packaging, stored out of reach, and if a child may have swallowed any, call poison control or emergency services immediately.
Gut side effects, and how to ease them
Nausea, constipation, and stomach upset are common with standard ferrous salts and are the usual reason people abandon iron. A lower dose, a single morning dose taken on alternate days, and taking it with a little food ease most of it without losing much absorption. The effects settle on stopping or lowering the dose.
Timing, interactions, and unexplained deficiency
Iron interferes with the absorption of some medicines, including levothyroxine and certain antibiotics, so separate them by a few hours and tell your clinician what you take. Iron deficiency that turns up in a man or a postmenopausal woman calls for finding the cause, since gastrointestinal bleeding is an important one to rule out.

Does iron give you energy if you are tired?
Only when the tiredness comes from low iron. In people who are iron-deficient, even before anemia sets in, iron reduces fatigue and can improve mood and some memory scores. In people whose iron stores are already fine, it does nothing measurable for energy. A ferritin blood test tells you which group you are in.
Do I need a blood test before taking iron?
