What It Is
An enlarged prostate is the ordinary, non-cancerous growth of the prostate gland that comes with age, and its medical name is benign prostatic hyperplasia. The gland is about the size of a walnut, sits just below the bladder, and surrounds the urethra. As it grows, it presses on that tube and stiffens the muscle at the bladder neck, so the stream slows and weakens. A short form, the IPSS, scores how much that bothers you from 0 to 35. The narrowing itself shows up as a familiar cluster of urinary symptoms:
- a weak or stop-start stream
- hesitancy before the flow begins
- going more often, with a sudden urge
- dribbling at the end
- waking at night to urinate
- a sense the bladder never fully empties
The enlargement is benign and does not become cancer, though it can sit alongside cancer in the same man. The cancer question, the PSA blood test, and screening are covered on the Prostate Health page. The IPSS sorts symptoms into three bands: mild is 0 to 7, moderate 8 to 19, and severe 20 to 35 (Barry and colleagues, Journal of Urology, 1992). The score measures how much the symptoms bother you, so an overactive bladder or an infection can raise it even when the gland is small. A mild score usually means no rush; a moderate or severe score that disrupts your day is when treatment is worth starting.
What Helps
For mild symptoms, a sound choice is to check in regularly and hold off on medication unless things get worse. Several no-cost habits help at any stage, and they pair with any drug you add later.
Shift most of your fluids to earlier in the day, and ease off in the two or three hours before bed, so you wake less at night. Drinking too little backfires, because concentrated urine irritates the bladder.
Cut back on caffeine and alcohol for the same reason. One evening coffee or a couple of drinks makes more urine and irritates the bladder, so the stream weakens and you wake more at night. Take unhurried time on the toilet, and try double voiding (pause after you finish, then go again) to empty it more completely.
When symptoms become bothersome, medication is the usual next step. The two prostate-selective alpha-blockers, tamsulosin and silodosin, relax the muscle around the prostate and bladder neck, and the stream improves. They are the usual first medication when a weak stream is the main complaint, and they ease symptoms without shrinking the gland.
The two gland-shrinking drugs, finasteride and dutasteride, work on the gland itself. They block the hormone that drives prostate growth, so an enlarged gland shrinks and a sudden blockage becomes less likely. They suit a large gland.
For a large gland at high risk of progressing, the two kinds together outdo either one alone. The alpha-blocker eases symptoms quickly, and the gland-shrinking drug curbs the growth over the long run. In the MTOPS trial the alpha-blocker alone cut the risk of the condition worsening by 39 percent. Combining the two lowered that risk further (McConnell and colleagues, MTOPS, New England Journal of Medicine, 2003).

5-alpha-reductase inhibitors shrink the gland over months and cut progression 34%
Finasteride and dutasteride shrink an enlarged prostate over months. In MTOPS finasteride cut the risk of the condition getting worse by about a third and, unlike the alpha-blocker, lowered the chance of sudden retention and surgery, an effect that held up over six years in a follow-up study.
The 5-alpha-reductase inhibitors block conversion of testosterone to dihydrotestosterone, the hormone that drives prostate growth, so they shrink an enlarged gland, not only relaxing muscle. In MTOPS finasteride reduced overall clinical progression by 34% against placebo and was, along with combination therapy, the arm that significantly reduced acute urinary retention and the need for invasive treatment. The PLESS study confirmed that the lower rate of retention and surgery persisted across six years. Because they act on gland size, they help most when the prostate is clearly large and take months, not days, to work, which is why they are often paired with an alpha-blocker for early relief. They also lower measured PSA by about half, a fact any clinician ordering a screening test needs to know.
A 5-alpha-reductase inhibitor is the choice when the gland is large and the goal is to prevent retention or surgery, often started alongside an alpha-blocker so relief comes while the shrinkage catches up. Tell any clinician ordering a PSA that you take one, because the result has to be doubled to be read correctly.

Combining the two drugs cut progression 66%, more than either alone
Taking an alpha-blocker and a gland-shrinking drug together works better than either alone for men with a larger prostate. In MTOPS the combination cut the risk of progression by two thirds, and a four-year trial of dutasteride plus tamsulosin found the same pattern.
MTOPS showed that combining doxazosin and finasteride reduced overall clinical progression by 66% against placebo, better than doxazosin (39%) or finasteride (34%) alone, because the two drugs work by different mechanisms: one relaxes muscle for fast relief, the other shrinks the gland to change the trajectory. The CombAT trial then tested dutasteride plus tamsulosin over four years in 4,844 men with prostates of at least 30 mL and higher risk of progression, and found combination therapy superior to either monotherapy for symptom improvement and better than tamsulosin at reducing acute urinary retention and BPH-related surgery. The benefit is concentrated in men with larger glands, and combination brings the side effects of both classes, so it suits men at higher risk of progression, not everyone with symptoms.
Combination therapy makes the most sense when the gland is enlarged and the aim is both quick relief and preventing retention or surgery. For a small gland or mild symptoms, a single drug usually does the job with fewer side effects.

Saw palmetto did not beat placebo, a 0.04-point difference out of 35
The common advice to take saw palmetto for an enlarged prostate has not held up. In good placebo-controlled trials, including one that pushed the dose to three times standard, it worked no better than a dummy pill, and the current Cochrane review agrees.
Saw palmetto is the most popular prostate supplement and was long recommended as an alternative to prescription drugs. Better trials revised that. The STEP trial randomized 225 men to saw palmetto or placebo for a year and found a difference in the symptom score of 0.04 points out of 35, with no difference in flow rate, prostate size, or residual volume. The CAMUS trial then took 369 men up to three times the standard 320 mg daily dose and still found no benefit over placebo. The 2023 Cochrane review concludes Serenoa repens does not improve urinary symptoms compared with placebo. The early positive reports came from smaller, lower-quality studies, which is where the reputation was built.
If saw palmetto is already part of a routine it is not dangerous, but it is not doing the work its reputation claims. Money and hope are better spent on the measures that do move symptoms, and on a conversation about the drugs proven to prevent retention and surgery.

Die Lesart ist, dass im durch Belastung geprägten Mittelbereich der Erkrankung der Zeitpunkt der Behandlung eine Frage der Präferenz ist und ein Aufschub für viele Männer wenig kostet, was abwartendes Beobachten zu einem aktiven Überwachungsplan macht, nicht zu Vernachlässigung.
Wenn die Symptome mäßig sind und Sie damit leben können, ist abwartendes Beobachten mit regelmäßiger Kontrolle sinnvoll, und Sie behalten jede Behandlungsoption offen. Two office-based procedures, done under local or light anesthesia, sit between tablets and full surgery. The prostatic urethral lift pins the enlarged lobes open with small implants; water vapor therapy uses steam to shrink the blocking tissue. Both ease symptoms while preserving ejaculation, and both suit smaller-to-moderate glands.
For a large gland, or symptoms that tablets cannot control, two operations relieve the obstruction the most. Transurethral resection of the prostate, or TURP, cuts away the blocking tissue; for a very large gland, enucleation removes it whole. A urologist weighs the choice against the gland's size and your priorities.

In the STEP trial it beat placebo by 0.04 points out of 35 on the symptom score (Bent and colleagues, New England Journal of Medicine, 2006). The CAMUS trial pushed the dose to three times the standard and still found nothing over placebo. A 2023 Cochrane review, pooling the higher-quality trials, concludes it gives little to no benefit (Franco and colleagues, Cochrane Database of Systematic Reviews, 2023). It is well tolerated, so a man already taking it comes to no harm.
Two plant compounds did better than saw palmetto in trials, but the benefit is small and the evidence is thin. Beta-sitosterol improved symptom scores by about 4.9 points on the IPSS-type 0-to-35 score, and peak urine flow by about 3.9 milliliters per second, over placebo. That came from a Cochrane review of four short trials, and it did not shrink the gland.
Pygeum, an extract of African plum-tree bark, modestly improved symptoms and flow across a review of 18 small trials. Men made about a fifth fewer nighttime trips, with roughly a 23 percent rise in peak flow.
The same caveats apply to both compounds. The trials were small, ran only a few weeks, and used inconsistent preparations. None tested whether these products prevent the blockages and surgery the prescription drugs are proven to prevent, so at best they help a little in the short term. Prostate products sold online also have a documented record of being spiked with undeclared drugs, so the source matters.

The sexual side effects of finasteride and dutasteride deserve a closer look. For most men, lower libido and trouble getting erections ease over time, or reverse once the drug is stopped. A pooled analysis found that the drugs eased symptoms but did cause these sexual side effects at a real, measurable rate (Kim and colleagues, PLoS One, 2018). A minority report trouble that persists after stopping, more often younger men who took the drug for longer.
That finding rests on medical-record data, and its true frequency is disputed (Kiguradze and colleagues, PeerJ, 2017). Raise any lasting change with the prescriber early. Chinese medicine reads difficult or blocked urination through the organs of the lower burner, the region that governs urination and reproduction. The classical name for a poor, dribbling, or obstructed stream is Long Bi (癃閉). Treatment depends on the presentation. Warming herbs suit a cold, deficient Kidney pattern and are wrong for a hot, damp one. The aging behind most enlargement is read as deficiency that calls for tonifying. Herbal treatment belongs with a practitioner.
This is the ordinary aging that lies behind most prostate enlargement. Yang deficiency shows as a weak, dribbling stream, frequent pale urination, feeling cold, and low back and knee weakness; Yin deficiency as scanty dark urine, night sweats, and a dry mouth. The direction is to tonify the Kidneys, warming the Yang or nourishing the Yin as the presentation calls for.
Burning, urgent, frequent or difficult urination, sometimes cloudy urine or a heavy feeling below, with a yellow greasy tongue coat. This is the pattern that usually maps to irritation or infection. The direction is to clear heat and drain damp.
A stream that starts and stops, a feeling of incomplete emptying, and a fixed, distending discomfort low in the abdomen, on a dusky or purplish tongue. Long-standing obstruction is read as qi and blood no longer moving freely. The direction is to move qi and invigorate blood to open the flow.

Alpha-blockers often reduce ejaculate, and tamsulosin can cause floppy iris in cataract surgery
Reduced or absent ejaculate is one of the most common effects of tamsulosin and silodosin, reported across the large BPH trials, and it reverses when the drug is stopped. Separately, tamsulosin is the alpha-blocker most tied to intraoperative floppy iris syndrome, where the iris billows and the pupil constricts during cataract surgery, raising the risk of complications. A prospective masked comparison graded the syndrome as more severe in men taking tamsulosin than in those on the less selective alfuzosin or on nothing. The gland-shrinking drugs commonly lower libido and cause erectile difficulty, reversible for most men
A meta-analysis of 5-alpha-reductase inhibitor monotherapy in BPH found a statistically clear but small benefit over placebo, alongside a high rate of adverse events including reduced libido, erectile difficulty, and reduced ejaculate volume. For most men these ease over time or reverse when the drug is stopped. A separate analysis of medical records reported that a subset of men, more often younger men with longer exposure, developed erectile dysfunction that persisted after stopping, the basis of what some call post-finasteride syndrome. That persistent picture rests on observational data and its true frequency is disputed, so it belongs in the decision as a possibility to weigh, not an established rate. The gland-shrinking drugs halve the PSA reading
Finasteride and dutasteride lower the measured PSA by about half within several months, so a screening result taken while on one has to be roughly doubled to be read correctly. Tell any clinician ordering a PSA that you take one, since an unadjusted value can look reassuringly low when it is not.
Alpha-blockers can cause dizziness on standing at first
The older non-selective alpha-blockers in particular can lower blood pressure and cause light-headedness on standing in the first days, so standing up slowly helps while the body adjusts. The prostate-selective drugs tamsulosin and silodosin cause less of this. If you are due for cataract surgery, tell the eye surgeon you take one before they operate.
Living with an enlarged prostate is mostly worked out steadily at your own pace. The habits above (shifting fluids earlier, cutting evening caffeine and alcohol, double voiding) are yours to start now. Which medication or procedure fits, and any change to one, is a decision to make with your doctor.
When to See Someone
A few situations change the picture and need attention sooner. These are the signs to get seen about:
- A sudden and painful inability to pass urine, with a full, tense bladder. This is acute urinary retention, a urological emergency (Pinar and colleagues, French Journal of Urology, 2025). The bladder must be drained with a catheter that day; it does not resolve on its own, and the back-pressure can harm the kidneys(seek urgent care)
- Fever with painful, frequent or difficult urination. This can mean a urinary or prostate infection and needs prompt antibiotics(seek urgent care)
- Visible blood in the urine. It often has a harmless cause, but because it can be the first sign of a bladder, kidney or prostate cancer it needs to be assessed(seek urgent care)
- Urinary symptoms that worsen quickly over days, when the usual change is slow over months
- Recurrent urinary infections, bladder stones, or a constant sense the bladder never empties, all of which point to obstruction that a urologist should assess
- Leaking because the bladder stays full and overflows, or new or worsening kidney results. Both need prompt review
None of this is a reason to worry about ordinary urinary symptoms; they are common and very treatable. The few signs to know are the ones that mean things are moving faster than the usual slow change over months. A sudden complete blockage is the one true emergency here. It can. Many contain one of two common decongestants, pseudoephedrine or phenylephrine, and both tighten the muscle at the bladder neck. For mild symptoms, yes. In the trial that set watchful waiting against surgery, most men who waited stayed stable through the years of follow-up. Regular reviews catch the minority whose symptoms build, so treatment can start if the picture changes.
