What It Is

Choline is an essential nutrient, close to the B vitamins in how it works, formally recognized by the Institute of Medicine in 1998. The liver makes some through an enzyme called PEMT, but never enough, so the rest has to come from food. Its work touches three systems: the brain, the liver, and the body's methylation chemistry.

What It Does

The best-established job is protecting the liver, and the recommended intake (425 mg a day for women, 550 mg for men) was set to guard it. In tightly controlled feeding studies, volunteers whose diets were stripped of choline developed a fatty liver, or muscle injury, within weeks. The damage cleared once choline came back.

Pregnancy is the second role with real support. Large amounts of choline cross the placenta, and demand outruns what the mother can make, so the extra goes to forming the fetal brain. In a controlled trial, mothers took roughly double the recommended amount during the third trimester. Their infants processed information faster than the infants of mothers at the standard amount.

For adult memory, the controlled-trial signal is thin. Community studies have found people with higher choline intake scoring a little better on memory tests, but that is an association. Heavier choline eaters tend to eat well across the board. A systematic review of controlled trials found no consistent proof that supplementing a well-fed adult sharpens thinking. The firm cognitive evidence is in the developing fetal brain.

In detail

Fischer and colleagues at the University of North Carolina fed 57 adults (26 men, 16 premenopausal women, 15 postmenopausal women) a fixed 550 mg per 70 kg per day for 10 days, then a depletion diet of under 50 mg per 70 kg per day, with or without a folic acid supplement, for up to 6 weeks. Liver fat was measured by MRI and muscle damage by serum creatine phosphokinase. Overall, 77% of men and 80% of postmenopausal women developed fatty liver or muscle damage on depletion, compared with 44% of premenopausal women, and 6 men showed signs even while consuming the Adequate Intake. Damage reversed when choline was restored. Folic acid supplementation did not change the response.

How to use it

This is why choline counts as an essential nutrient and why the recommended intake exists. It also explains why the requirement is not the same for everyone: men and postmenopausal women are more vulnerable to a shortfall than premenopausal women, whose estrogen drives more internal choline synthesis.

Its metabolic roles are well characterized: it is phosphorylated and converted to phosphatidylcholine (lecithin), the predominant membrane phospholipid and a required component of VLDL; it is the precursor of acetylcholine; and it is oxidized in the liver and kidney to betaine, an irreversible step that supplies methyl groups to convert homocysteine to methionine. Choline, folate and methionine metabolism interconnect, so a shortfall in folate raises the demand on choline as a methyl donor.

How to use it

The mechanism explains why a deficiency shows up first as fatty liver (blocked fat export) and why choline interacts with folate status. It also cautions against reading too much into it: an established biochemical role is not the same as a measured benefit from taking more.

Resseguie and colleagues showed that estrogen regulates PEMT in an isoform-specific way and mapped the estrogen-receptor binding across the gene. The polymorphism most strongly linked to choline deficiency syndrome, rs12325817, sits within 1 kb of a critical estrogen response element, and the risk allele failed to bind the estrogen receptor or the pioneer factor FOXA1. That loss of hormonal activation reduces the liver's ability to synthesize choline and raises the dietary requirement, which is why some people, including women carrying the variant, develop organ dysfunction on a low-choline diet while others do not.

How to use it

It means the recommended intake is an average, not a personal number, and a minority need more. Genetic testing for these variants is available direct to consumer, but for most people the practical takeaway is simpler: get choline reliably from food, not assume you synthesize enough.

Wallace and Fulgoni applied the National Cancer Institute method to NHANES intake data. About 8.03% of adults and 8.51% of pregnant women met the Adequate Intake; children aged 2 to 8 were most likely to meet it. Adults who ate eggs met their target 57.3% of the time versus 2.43% for non-consumers, and egg eaters averaged 525 mg/day versus 294 mg/day. The authors concluded it is very difficult to meet the Adequate Intake without eating eggs or taking a supplement. A companion analysis put mean intakes at about 250 to 280 mg/day for women and 387 to 407 mg/day for men.

How to use it

Below the Adequate Intake is common and mostly not dangerous in a varied diet, since the intake is set with a margin and frank deficiency is rare. Tang, Hazen and colleagues at the Cleveland Clinic combined a controlled human challenge with a prospective cohort. The challenge confirmed TMAO is produced by intestinal microbiota from dietary phosphatidylcholine. In the cohort of 4,007 patients undergoing elective coronary angiography, higher fasting TMAO predicted death, myocardial infarction or stroke over 3 years (highest vs lowest quartile HR 2.54), holding after adjustment for traditional risk factors.

How to use it

This is the origin of the eggs-and-heart worry, but it measures a blood metabolite, not a menu. TMAO also rises from fish and red meat, is higher when kidney function is low, and this cohort was selected for heart disease, so a high TMAO reading is a marker to interpret cautiously, not a reason to avoid choline-rich food.

Wilcox, Tang, Hazen and colleagues randomized healthy volunteers (41% male, median age 28) with normal renal function to one of five daily interventions for 4 weeks: hardboiled eggs, choline bitartrate, eggs plus choline bitartrate, egg whites plus choline bitartrate, or phosphatidylcholine supplements. Every arm containing choline bitartrate raised fasting TMAO significantly, along with platelet responsiveness, whereas whole eggs alone did not. An earlier crossover study in young adults similarly found egg intake raised plasma choline without the TMAO rise seen with supplemental choline.

How to use it

This is the practical resolution of the eggs-and-TMAO scare: get choline from food, and treat concentrated choline or phosphatidylcholine supplements as the form that meaningfully raises TMAO, most relevant for people with reduced kidney function. What the raised marker means for real heart events was not measured here.

Caudill and colleagues at Cornell randomized 26 women entering their third trimester to a tightly controlled 480 mg per day (n=13) or 930 mg per day (n=13) of choline until delivery, with intake of choline and other nutrients held constant. Infant information processing speed and visuospatial memory were tested at 4, 7, 10 and 13 months in 24 infants. Mean reaction time across the four ages was significantly faster in the 930 mg group. Within the 480 mg group, infants exposed to the higher intake for longer also showed faster reaction times, hinting that even modest increases may help.

Who this may not transfer to:Mothers were women by definition; infant outcomes were measured in both sexes. The sample is small (24 infants) and the outcome is a laboratory measure of processing speed.

How to use it

This supports aiming for at least the pregnancy requirement of 450 mg a day, and possibly higher, through food or a choline-containing prenatal. It is a small study of a surrogate measure (reaction time), not proof of lasting IQ gains, so it is a reason to meet the target, not to megadose.

Poly and colleagues analyzed 1,391 non-demented Framingham Offspring participants (744 women, 647 men, ages 36 to 83) who completed food-frequency questionnaires at two exams and underwent neuropsychological testing and brain MRI. Higher concurrent choline intake was associated with better verbal memory (0.60 per unit; 95% CI 0.29 to 0.91) and visual memory (0.66; 95% CI 0.19 to 1.13). Higher remote intake was inversely associated with white-matter hyperintensity volume and with the presence of large white-matter lesions (OR 0.56; 95% CI 0.34 to 0.92).

How to use it

Read this as a reason to eat enough choline as part of a good overall diet, not as evidence that a choline pill sharpens an adult brain. The controlled trials that could test that have not shown a consistent benefit.

Across life stages the findings were mixed, and the review did not find consistent evidence that choline intake or supplementation improves cognition in healthy adults, in contrast to the more coherent prenatal signal.

The TMAO Question

Gut bacteria convert some dietary choline, and the carnitine in red meat, into trimethylamine, a gas the liver oxidizes to TMAO (trimethylamine-N-oxide). Higher blood TMAO has been linked in several studies to more heart attacks and strokes, so eggs, a leading choline source, became the focus of a heart-risk question.

The link between blood TMAO and cardiovascular events is real as an association, but it is drawn largely from patients already referred for heart testing. Fish breaks the link: oily fish raises TMAO more than eggs, yet fish eaters have less heart disease, so a high TMAO reading is a poor signal of harm. Tested head to head, whole eggs barely moved fasting TMAO while an equal dose of a choline supplement raised it, along with markers of platelet stickiness.

The evidence sorts into three statements:

  • Blood TMAO is associated with cardiovascular events: consistent, but observational and confounded.
  • Whole eggs raising TMAO enough to matter: not supported in the feeding trials.
  • Choline supplements raising TMAO: shown, with the effect on actual heart events unmeasured.

How It Works

Genes and diet together set how much choline a person needs. The liver's PEMT enzyme, switched on by estrogen, lets premenopausal women make more choline and tolerate a low intake better. Men and postmenopausal women make less, and are the most vulnerable when intake drops. A common variant near the PEMT gene blunts the estrogen response, so a subset of people, including some women, need more choline from food than average.

Anatomy of the Practice

1As a methyl donor and a membrane brick

Most choline is converted to phosphatidylcholine, the main phospholipid in cell membranes. Some is oxidized to betaine, which donates a methyl group to recycle homocysteine into methionine. This overlaps with folate, so when folate is low, choline is pulled harder into methyl duty and the requirement rises. This is established human biochemistry.

2In the liver, packaging fat for export

The liver needs phosphatidylcholine to build the VLDL particles that carry fat out into the blood. Without enough choline, that export stalls and fat accumulates in the liver.

3In the brain and the developing nerves

Choline is the raw material for acetylcholine, a neurotransmitter for memory and muscle control. It also supplies the methylation that shapes the fetal brain and its hippocampal memory circuits. A controlled prenatal trial measured this effect on the child.

Ways to Do It

For most people, eggs and a varied diet make the target easy to hit from food. A mostly plant diet and pregnancy are where the requirement is hardest to meet from food alone.

The Chinese Medicine View

In Chinese dietary therapy, egg nourishes Yin and Blood and moistens dryness. It is also used to settle a pregnancy, the same place the modern evidence is strongest, though the tradition arrived there on its own terms. Animal liver is read as building Liver Blood and benefiting the eyes, used for pallor, depletion, and poor night vision. Soy and its products are seen as cooling and supplementing.

Egg yolk, liver, and heavier soy preparations are read as rich and moistening. They nourish someone dry, depleted, or worn down, but burden someone already damp or phlegm-laden, where more richness feeds the accumulation and dulls digestion. A Spleen that is weak and cold is thought to struggle with cold, raw soy foods in particular. Both agree the right amount depends on the person: the tradition on constitution, the modern data on diet, pregnancy, and genes.

Cautions For This Practice

Everything to be aware of is here, in one place. This practice suits most healthy people; a few situations call for real care.

Doses at and above the 3.5 g daily upper limit can cause low blood pressure, sweating, and a fishy body odor

The Institute of Medicine set the adult upper limit at 3,500 mg/day, based primarily on hypotension and secondarily on the fishy body odor caused by increased trimethylamine excretion. Case reports of very high intakes (roughly 10 to 16 g/day) describe fishy odor, sweating, hypersalivation, vomiting and diarrhea. Very high doses can lower blood pressure and cause a fishy odor

The tolerable upper limit is 3.5 g a day for adults, set because doses around and above it can lower blood pressure. Much larger intakes, in the range of 10 to 16 g a day, have caused a fishy body odor, from excess trimethylamine, and gastrointestinal upset. These are effects of concentrated supplements, not of food; you cannot reach them by eating eggs. Keep everyday supplemental choline well below the upper limit.

The fishy-odor risk is higher for a few people

People with the inherited condition trimethylaminuria, who cannot fully clear trimethylamine, can develop the fishy body odor at ordinary supplement doses. Liver disease, kidney disease, Parkinson's, or depression can also raise sensitivity to high-dose choline. Be most cautious near the upper limit.

Supplements raise TMAO more than food, which matters most with kidney disease

TMAO is cleared by the kidneys, so a concentrated choline supplement, which raises TMAO more than food does, matters most for someone with reduced kidney function. If your kidneys are impaired, meet your needs from food and be careful with high-dose supplements.

Read prenatal labels rather than assume they cover it

Pregnancy raises the choline requirement while most prenatal vitamins supply little or none. Check whether yours contains choline. If not, make up the 450 mg target with eggs or a choline-containing product.

Start low and check with a clinician if you have concerns

For a healthy adult, meeting the intake from food carries no meaningful risk. If you have a liver or kidney condition, or you are weighing a high dose, start low and check with a licensed practitioner.

Do I need a choline supplement?

In the United States most intakes fall below the Adequate Intake, yet frank deficiency stays rare on a varied omnivorous diet with eggs, fish, meat, or soy. Two groups reliably fall short: people on a mostly plant diet with no eggs, and pregnant women.

Which supplement form should I look for?

Choline bitartrate is the cheapest and simply raises total choline. Phosphatidylcholine, sold as lecithin from soy or sunflower, delivers choline as the membrane phospholipid. Alpha-GPC and CDP-choline (citicoline) cost more and are the forms marketed for cognitive support. All four count toward the same daily total.

Does cooking destroy choline?

No. Choline is heat-stable, so boiling, frying, or baking does not meaningfully lower it. The roughly 250 mg in two eggs survives cooking largely intact, unlike heat-sensitive nutrients such as folate.
