What It Is
A wellness IV drip is a bag of saline with vitamins and minerals dissolved in it, run into a vein through a small cannula over 30 to 60 minutes. The original recipe, the Myers cocktail, mixes magnesium, calcium, several B vitamins and vitamin C. Newer menus add higher-dose vitamin C, amino acids, or NAD+, sold by name for energy, immunity, hangovers, skin and recovery.
These drips are used in two different ways. One is the medical use of intravenous vitamins for a documented deficiency, or a gut that cannot absorb nutrients. The other is the wellness market, selling the same infusions to people who are already well-nourished. The evidence for each is different.
How It Works
An intravenous drip routes nutrients straight into the blood, past the gut, which limits how much of a nutrient the body takes up. The route matters in three ways.
Anatomy
1Oral versus intravenous
The gut caps how much of a water-soluble vitamin the body absorbs and holds, so a pill raises blood and tissue levels to a ceiling and no higher. An IV bypasses that cap and reaches blood levels many times higher. That higher level is the one pharmacological difference. In a person who is already replete, that rise is brief: the surplus above what tissues can hold is filtered by the kidneys and excreted.
2What is in a wellness drip
Most drips are saline plus water-soluble vitamins, chiefly B vitamins and vitamin C, sometimes with magnesium. A drip sold for immunity or radiance is usually the same mixture under a brand name.
3What NAD+ is
NAD+ is a coenzyme every cell uses to convert food into usable energy and to run repair enzymes. Its level tends to fall with age, which is the basis for the whole NAD+ category. People raise it with oral precursors, nicotinamide riboside and nicotinamide mononucleotide, or with an intravenous NAD+ infusion, the slow and costly premium option.
How high the blood level goes is what matters. For high-dose vitamin C studied in cancer, the very high level is the point: concentrations several times anything a diet supplies produce effects ordinary amounts do not. For a wellness drip in a well-nourished person, the same high level does little, because tissues are already full.
Raising a blood marker is separate from improving an outcome. An infusion can lift a number on a lab report. Each trial still has to show the drip changes how people feel, perform or age.
What Changed
The idea behind these drips is a plausible piece of physiology: more of a nutrient in the blood should mean more benefit. Whether it does turns on a single question: was the body short of that nutrient to begin with? Ranked by the evidence behind them:

- NAD+ precursors: strong on the marker, unconfirmed on benefit.
- High-dose vitamin C in cancer: investigational.
- Wellness vitamin drips, the weakest evidence.
Where The Research Stands
Wellness Vitamin Drips
The one randomized test of the Myers cocktail was run in fibromyalgia, and it found no benefit over a placebo infusion. Both groups improved. The lift came from the infusion experience and the attention around it, not the vitamins.
No controlled trial supports the hangover claim. In people who are not deficient, a measurable benefit over placebo has not been shown, and no mechanism predicts one in a person who is already replete.
High-Dose IV Vitamin C In Cancer
Early-phase trials have paired high-dose IV vitamin C with chemotherapy. These report it is generally well tolerated, with preliminary signals on quality of life and fatigue.
A randomized controlled trial in pancreatic cancer, PACMAN, reported a promising survival signal when the high dose was added to chemotherapy. The result is preliminary and needs confirmation in larger trials. As it stands, high-dose IV vitamin C is an experimental adjunct; no trial has shown it extends life.
NAD+, And The Precursors NR And NMN
Oral NR and NMN reliably raise NAD+ and its metabolites in the blood, a result shown across several randomized trials. One trial in postmenopausal women with prediabetes found NMN improved insulin sensitivity in muscle, a lead worth following.
A review of the trials found the compounds move the biomarker, but everyday energy, physical performance and slower aging stayed unconfirmed. The trials were small, short and inconsistent. The one study that tracked an intravenous NAD+ infusion measured only its chemistry; no controlled trial has tested whether an NAD+ drip changes any clinical outcome.
Legitimate Medical IV Vitamin Use
When a person has a documented deficiency, or a condition that stops them absorbing nutrients, an intravenous vitamin corrects a shortfall no pill can reach. That covers gut disorders, bariatric surgery and severe malnutrition, three settings where absorption fails. It is how diagnosed scurvy, pernicious anemia, and iron-deficiency anemia that cannot be managed by mouth are treated.

How it works
Extra water-soluble vitamins are excreted, not stored, in people who are already replete
In someone who is already well-nourished, tissues are full, so pushing in extra water-soluble vitamins mostly ends up in the urine, not raising anything the body was short of.
The Levine group mapped vitamin C pharmacokinetics in healthy volunteers: absorption and tissue uptake are saturable, so oral doses drive blood and tissue levels up to a ceiling and no further, with the excess excreted. An IV route can reach blood concentrations many times higher than any pill because it bypasses the gut cap, which is the pharmacological difference behind high-dose IV vitamin C. For the water-soluble vitamins in a wellness drip given to a replete person, that higher blood level is transient and cleared by the kidneys, which is why raising a level briefly is not the same as correcting a shortfall.

Oral nicotinamide riboside 1,000 mg a day raises blood NAD+, a biomarker not an outcome
The pill form of an NAD+ booster really does raise NAD+ in the blood. That part is settled. The trial measured the blood marker, not whether people felt or functioned any better.
This crossover RCT is one of several showing oral NAD+ precursors move the biomarker. Twenty-four healthy adults aged 55 to 79 took nicotinamide riboside 1,000 mg daily for six weeks and placebo for six weeks, in random order, double-blind. NAD+ metabolism was clearly stimulated, most visibly a large rise in the precursor NAAD, confirming the compound is absorbed and used. The exploratory clinical readouts, a trend toward lower blood pressure and arterial stiffness in those who started elevated, did not reach significance for the group. So the trial establishes the mechanism, raising the marker, and not a clinical outcome.

An IV NAD+ drip takes about two hours to appear in blood; the study measured chemistry only
The one study that watched what an IV NAD+ drip actually does found the body takes hours to show any blood NAD+ and then quickly breaks it down and clears it. It measured chemistry, not whether anyone felt or functioned better.
Intravenous NAD+ is promoted for energy, recovery and anti-aging, and it is uncomfortable and expensive, typically infused slowly over hours because faster rates provoke symptoms. This pilot tracked the plasma and urine NAD+ metabolome across a 6-hour infusion and found NAD+ was not detectable in plasma until roughly two hours in, after which it was rapidly metabolized and its breakdown products rose in blood and urine. That maps the pharmacokinetics and nothing more: there is essentially no controlled trial measuring whether IV NAD+ improves any clinical outcome, so its marketed benefits rest on mechanism and testimonial, not outcome data.

Oral NMN improved muscle insulin sensitivity in 25 prediabetic women in one trial
In one careful trial, an oral NAD+ precursor made the muscles of prediabetic women respond better to insulin. It was a single small study, it did not change weight or the other usual markers, and it used a pill, not a drip.
This is the strongest single human outcome signal among the NAD+ trials. Twenty-five postmenopausal women with prediabetes who were overweight or obese took nicotinamide mononucleotide or placebo for ten weeks, double-blind. Insulin-stimulated glucose disposal in skeletal muscle, assessed by hyperinsulinemic-euglycemic clamp, rose with NMN and not with placebo, alongside changes in muscle insulin signaling and remodeling gene expression. Body weight, blood pressure, liver fat and circulating metabolic markers were largely unchanged. It is one small trial in one group, so it is a lead to follow, not a settled effect.
Who this may not transfer to:Measured only in postmenopausal women with prediabetes. Whether the same muscle insulin-sensitivity effect occurs in men, in younger women or in people without prediabetes is untested, so it should not be assumed to carry across.

Pain
Myers-cocktail drips no better than placebo for fibromyalgia in a 34-person trial
In the one controlled trial, a weekly vitamin drip worked no better than a placebo drip for fibromyalgia. People in both groups felt somewhat better, which points to the effect of receiving an infusion and attention, not the vitamins.
This is the closest thing to a head-to-head test of the popular Myers cocktail. Thirty-four adults with fibromyalgia were randomized to eight weekly intravenous micronutrient infusions or matching placebo, double-blind. On the primary tender-point measure and on the secondary pain, tenderness, depression and quality-of-life measures, there was no statistically significant difference between the infusion group and placebo at 8 or 16 weeks. Both groups improved from baseline, so the trial supports the drip being safe and feasible while leaving its benefit over placebo unestablished.

Energy And Fatigue
NAD+ precursors raise the blood marker but have not improved energy, function, or aging
Across the human studies, NAD+ boosters raise the blood level dependably but have not been shown to make people more energetic, stronger, sharper or younger. The marker moves; the payoff people are sold has not.
Summarizing the controlled human data on nicotinamide riboside and nicotinamide mononucleotide, this review separates two things: the compounds elevate NAD+ and its metabolites consistently, and the functional or clinical benefits the category is marketed on, more energy, better physical performance, healthier aging, are not established, because the trials are small, brief, use varied outcomes and do not converge on a benefit. The gap between a moving biomarker and an unchanged outcome is what a reader weighs before paying for these compounds.

High-dose IV vitamin C with chemotherapy stayed investigational in a 14-patient study
In early studies, high-dose vitamin C given by drip alongside chemotherapy was tolerated, and a small number of people said they felt better. It is investigational. It has not been shown to shrink cancer or help people live longer, and it is not a cancer treatment.
High-dose IV vitamin C reaches blood levels no pill can, which is why it is studied as an experimental adjunct in oncology, not as a wellness drip. In this single-arm phase I-II study, 14 patients with advanced cancer received it alongside chemotherapy; it was safe and generally tolerated, with transient infusion-related effects, and three patients had transient stable disease with increased energy and functional improvement. The investigators explicitly say the work neither settles nor rules out a role for IV vitamin C in cancer, positioning it as feasibility research. No survival benefit has been demonstrated, and nothing here supports using it in place of standard treatment.

For the everyday immunity and wellness reasons the drips are sold, the controlled evidence of benefit is thin and inconsistent, and the review that looked also documented risks to weigh.
Pulling together the controlled literature on high-dose IV vitamin C, this systematic review reports that benefit signals are limited, inconsistent and largely confined to particular clinical contexts, not the general immune-boost and wellness uses marketed by drip clinics, and it documents the risks alongside. Combined with the physiology of repletion, that is why an immunity drip for a well-nourished person is not supported by controlled outcome data, even though the infusion itself briefly raises a blood level.

The Chinese Medicine View
An IV line and an isolated coenzyme are two products of modern medicine, so the classical Chinese pharmacopoeia has no entry for either. No historical text assigns a channel, a temperature or a flavor to a vitamin drip, and none describes a preparation that resembles an infusion.
Tonifying, bu, is the family of methods for building up one of four deficiencies, Qi, Blood, Yin or Yang. Its whole art is judging whether a deficiency is truly present, and whether the person can assimilate what is given.
One central teaching holds that the Spleen governs transformation: nourishment has to be transformed and absorbed before it becomes part of the body. Give more than the system can transform, and the surplus does not build reserve; it creates stagnation and damp. The evidence for any drip rests on the trials that tested it.

The IV line itself can cause infection, vein inflammation, and rarely air embolism
In a survey of complementary and alternative medicine practitioners who administer IV vitamin C, most reported few problems, but adverse events did occur, including infusion-site reactions, and the report notes serious harm in patients who were given high doses without appropriate screening. That documents that harms happen in real-world practice. Separately, placing a cannula and running fluid into a vein carries its own well-recognized hazards independent of the contents: phlebitis, local and bloodstream infection, extravasation, fluid and electrolyte disturbance, and the rare but serious risk of air embolism. High-dose IV vitamin C can trigger severe red-cell breakdown in G6PD deficiency
G6PD is the enzyme that protects red blood cells from oxidative stress. In people who inherit a deficiency, a large oxidative load can rupture red cells, causing acute hemolytic anemia. Published case reports describe exactly this after high-dose intravenous ascorbic acid, including a classic report in the BMJ and later cases presenting with dark urine and a falling blood count. The mechanism is well understood and the reaction can be severe, which is why screening for G6PD deficiency is standard before any high-dose IV vitamin C. The IV line itself
Placing a cannula and running fluid into a vein carries risks independent of what is in the bag: vein inflammation, infection at the site or in the blood, fluid leaking into tissue, and rarely an air embolism. An air embolism is a dangerous air bubble in the circulation. These events are uncommon, and most infusions pass without incident, but they apply to every drip.
G6PD deficiency and high-dose vitamin C
People with the inherited enzyme condition G6PD deficiency can suffer sudden, severe breakdown of red blood cells after high-dose intravenous vitamin C. The mechanism is well understood and the reaction can be dangerous. Reputable clinics screen for G6PD deficiency before any high-dose infusion. If you have G6PD deficiency, avoid a high-dose vitamin C drip and raise it with your clinician.
Iron overload with intravenous iron
Intravenous iron treats iron-deficiency anemia. Given to someone who is not iron-deficient, it pushes iron stores too high. Repeated infusions without a clear need risk iron overload. Iron overload harms the liver, heart and other organs. Iron by vein belongs with a documented deficiency and a monitoring plan.
Unregulated clinics and self-treatment
Many wellness drips are given in storefront or mobile settings that sit outside the checks a medical facility carries. Sterile technique, dosing, screening and emergency readiness are not guaranteed. A drip does not diagnose the underlying problem. Using one to fix persistent fatigue or a recurring illness can delay finding the cause. If a symptom keeps returning, it needs a proper diagnosis.
When a clinician's input helps
In a well-nourished person, a wellness drip has no established benefit. If you are considering an intravenous vitamin, ask a clinician to test for a deficiency first. If a test finds one, the infusion may help. Let a blood test guide the decision.

Do vitamin IV drips give you more energy?
For most people who eat a reasonable diet, no controlled trial has shown they do. What a drip reliably delivers is fluid and a rest, and that alone can feel refreshing.
Does high-dose vitamin C treat cancer?
No. It is tested only as an add-on to standard cancer treatment, and those trials are preliminary. In dose and hospital supervision it has nothing in common with a wellness drip, so it is no reason to delay or replace standard care.
Do NAD+ boosters like NR and NMN work for anti-aging or energy?
For anti-aging and energy, no. In people, more energy, better performance and slower aging are still unconfirmed. Any anti-aging claim on the label is still an untested hypothesis.
