What It Does

Static holds and PNF, the contract-then-relax method, build more range than ballistic bouncing, and across those same 77 trials women tended to gain more than men. For one specific problem the aim is exact: a plantar-fascia stretch eases plantar heel pain when the fascia itself is the tight tissue.

This review examined what actually changes when range of motion improves with stretching. The weight of evidence favored increased stretch tolerance, a change in how the stretch is sensed, over a lasting increase in muscle length or a change in passive tissue stiffness. Because the adaptation is largely sensory, the gains are not permanent structural changes, and they recede once the stimulus is removed. This explains both why flexibility builds fairly quickly and why it fades without maintenance.

How to use it

Treat flexibility like fitness, not a one-time fix: keep a little regular stretching or full-range work going, because range that is won and then dropped drifts back over weeks.

Balance And Falls

Flexibility work does not reduce falls in older adultsModerate · no effect

Stretching is not what prevents falls. The exercise that reliably cuts falls in older adults is balance and functional training, and Tai Chi; flexibility work on its own has not been shown to help.

This large Cochrane review graded the evidence for different exercise types in preventing falls among community-dwelling older adults. Balance and functional exercise carried high-certainty evidence of a reduction in the rate of falls, and Tai Chi also reduced falls. Flexibility exercise did not have the evidence to be counted as an effective fall-prevention approach. Stretching does no harm here; it simply should not stand in for the balance-focused training that lowers fall risk.

How to use it

For steadiness on the feet, the practice with the evidence is balance and functional training, covered on the balance and falls page. Keep mobility work for comfortable range, and pair it with the balance work, not substituting it.

How It Works

Stretching does not build or lengthen muscle. Across 25 studies its effect on muscle size was trivial, appearing only with holds far longer than anyone uses in practice. What changes instead is how the pull is sensed, moving on three timescales.

Anatomy of the Practice

1During the stretch

You take a joint toward the end of its range and hold. The pull you feel is your nervous system sensing tension. Within a held stretch that signal eases, so the same position feels less intense after thirty seconds.

2Over the first weeks

Practiced most days, your range increases. Most of that early gain is a greater tolerance to the stretch: your nervous system permits a position it used to guard, so the joint travels further.

3When you stop

Because the gain is largely in how the stretch is sensed, it fades without upkeep. Keep practicing and the range holds. Stop, and it drifts back over weeks.

What Stretching Does Not Do

Three things stretching was long prescribed for do not hold up in the trials:

  • Injury. It does not lower sports-injury rates. In the same kind of pooled trials, strength training cut injuries to about a third.
  • Soreness. A Cochrane review of stretching done before or after exercise found no drop in next-day muscle soreness.
  • Falls. In older adults it does not lower falls. Balance and functional training and tai chi and qi gong are what the evidence backs, covered under balance and fall prevention.

When to Stretch, and When Not To

Held in the minutes before a heavy lift or sprint, a long static stretch briefly weakens that effort, about a 5% drop in strength across 104 studies. The loss scales with the length of the hold and all but disappears below about 45 seconds.

A moving warm-up raises range without that penalty.

Building Range You Keep

The tool that builds range and keeps it usable is loading the joint through its whole path. Across 55 trials, strength training through a full range improved flexibility about as much as stretching did, with no significant difference between them. It builds strength at the same time. Full-range work also builds more muscle than partial reps. The range you can control under load is the range that stays.

The Chinese Medicine View

The tradition's name for deliberate therapeutic stretching is 導引 (daoyin), guiding and pulling, in the same family as tai chi and qi gong. Chinese medicine files tendons, ligaments and the quality of movement under the sinews (筋). The sinews belong to the Liver (肝主筋). Their suppleness depends on Liver Blood to moisten and nourish them, so the tradition reads stiffness, cramp and a narrow range as a matter of Liver Blood. Movement and gentle stretching move Qi and Blood through the channels, 經絡; where they flow freely the sinews stay soft and the joints move well.

The dose is set by a person's energy reserve. The governing phrase is 形勞而不倦, the body labors without becoming exhausted, so a stretch is eased to a comfortable limit and held there, never pushed past pain. The tradition does not treat more range as good for everyone. A Liver Blood deficiency picture (night cramps, dry eyes, floaters, brittle nails and tight tendons) is helped more by nourishing Blood, gentle frequent movement and keeping warm. In this tradition cold is held to contract the sinews. Someone whose joints are already very loose is not a candidate for still more opening; the answer depends on your pattern, so ask a practitioner.

Do not force a cold or painful stretch

A stretch should reach a firm resistance, stopping short of pain. Forcing past a sharp pain, or reaching hard into the range while the muscles are cold, is how a strain happens. Move around for a few minutes first so the tissue is warm, then ease into the hold and stop where it pulls without hurting.

Bouncing at the end range

Ballistic stretching, repeated hard bouncing to push past the current limit, carries more risk than a slow held stretch and, for building range, works less well. Controlled movement to warm up and steady holds to build flexibility do the job with less to go wrong.

If your joints already move beyond the normal range

Some people are naturally hypermobile, with joints that bend well past the usual limit. For them more flexibility is not the aim; strength and control around the joints is the useful work.

A fresh injury or an inflamed joint

Stretching into an acute muscle tear, a swollen joint, or one that was recently repaired can set healing back. Give an acute injury time, and after a repair or a joint replacement let a clinician guide the range and timing; a general routine does not apply here.

I've never stretched, where do I start?

Work the ladder above from the first rung, warming up with movement before you hold anything, and add the other rungs as they fit your week.

Do I need to stretch a lot to get more flexible?

No. In the pooled trials, piling on more stretching volume did not clearly add more range, so a short, steady dose most days does the work.

Should I stretch before or after exercise?

For general flexibility the placement matters little. For the effort right after, placement matters: avoid a long hold in the minutes before heavy or explosive effort; a short hold is fine.

Do I have to stretch at all, or is lifting enough?

For many people, full-range strength work covers general flexibility on its own. A separate stretching block is then optional. Save a targeted stretch for a spot that stays specifically tight.
