Bodywork: Moxibustion

My Plan

Moxibustion erwärmt einen Akupunkturpunkt, indem Beifuß in der Nähe der Haut verbrannt wird. Die am besten untersuchte Anwendung ist die Drehung eines Steißkindes. Das Erwärmen von BL67 am kleinen Zeh in den letzten Schwangerschaftswochen erhöhte in zusammengefassten Studien leicht die Wahrscheinlichkeit, dass sich das Baby kopfüber dreht.

Der übrige Teil der Evidenz ist dünner: vielversprechende, aber qualitativ niedrige Signale für Knie- und untere Rückenschmerzen, Menstruationskrämpfe und Chemotherapie-Nebenwirkungen. Die beiden wichtigsten Vorsichtsmaßnahmen sind Verbrennungen und Rauch.

What It Is

Moxa comes from mugwort, the dried and aged leaf of Artemisia vulgaris or Artemisia argyi. It is rolled into a stick or a small cone and burned so the heat warms an acupuncture point. Moxibustion heats the same map of points and channels a needle targets, and it is often paired with needles. The character for moxibustion, 灸 (jiu), means fire. It joins 針 (zhen), needle, to make zhen jiu, needle and fire, the single Chinese word for the discipline.

There are two broad ways to apply it. Direct moxibustion places a small cone of moxa on the skin over the point. Indirect moxibustion holds a lit stick a short distance away, or rests the moxa on a slice of ginger, salt, or garlic so the heat passes through. The best-known single use warms BL67, called Zhiyin, at the outer corner of the little toenail. That point ends the Bladder channel, and warming it is used to encourage a breech baby to turn.

Anatomy of the Practice

1The material and the point

Moxa is aged mugwort leaf, fluffed into a soft wool and pressed into a stick or a loose cone. It is burned over a chosen acupuncture point, most famously BL67 at the outer edge of the little toenail for breech presentation, or points on the lower abdomen and legs for cold-type period pain. The aim is steady, penetrating warmth without a flame touching the skin.

2What a session feels like

A stick or cone warms the point until it feels comfortably hot, then the heat is eased back before it stings. A stick is held a short distance from the skin and moved in small circles or lifted away when it gets too warm; an indirect cone warms more slowly. Sessions run several minutes per point and are often repeated daily over a couple of weeks for something like breech presentation.

3Warming where the pattern is cold

In practice moxibustion is chosen for patterns read as cold, deficient, or stagnant, where warmth is the aim of the treatment. That is why it clusters around cold period pain, some digestive complaints, and aching joints that ease with heat. It is also why a practitioner picks it over needling when the pattern reads as cold.

What It Does

One use stands out in the trials: turning a breech baby. In the 2023 Cochrane review of 13 trials in 2181 women, warming BL67 in the last weeks of pregnancy probably lowered the chance a baby stayed breech at birth. Across the pooled trials, about 13 percent fewer babies were still breech than in control groups, and a sham-controlled trial agreed. A European trial of 212 women found no effect, and moxibustion did not clearly lower the cesarean rate.

The pain and supportive-care uses sit below that, drawn from larger pools of lower-quality trials:

  • knee osteoarthritis, where 43 pooled trials pointed to a modest benefit;
  • chronic low back pain, across 10 trials, where it beat medication but not exercise;
  • period cramps, where warming timed to the days before a period raised the chance of relief;
  • chemotherapy side effects, added to cancer care.

Earlier signals point the same way for allergic rhinitis and ulcerative colitis.

These trials are mostly small and mostly from China. The certainty is graded low to moderate, so the true size of each effect is less settled than the response rates suggest.

Die Evidenz dazu, ob sie den Bedarf an äußerer Wendung senkt, war sehr unsicher (RR 0.62; 95%-KI 0.32 bis 1.21), und unerwünschte Ereignisse wurden in den meisten Studien unzureichend berichtet. How It Works

The mechanism comes down to heat delivered to a specific point. Warming the skin and the tissue beneath it raises local blood flow. The sensory nerves that carry warmth feed into the same spinal pathways that modulate pain, so a warm point can ease an aching joint or a cramping abdomen. For the breech use, the leading idea is that heating BL67 raises fetal movement, and more movement gives a breech baby more chances to turn. The exact pathway is not settled, and the trial evidence is mixed.

The main limit lies in the study design, the same one that constrains acupuncture research. A person feels the heat, so a sham that produces no warmth cannot convince anyone it is the active treatment. A practice you can feel resists blinding, which caps how strong the evidence can look even when the effect is real; better-designed trials are what the field is missing here. Without a matched sham, the placebo response and the specific effect cannot be separated.

How It Is Done

Most moxibustion is given by a licensed acupuncturist, who chooses the point and the form to match the pattern. The breech protocol is the main use taught for careful practice at home.

The Chinese Medicine View

A classical line holds that where needles and herbs fail, moxa may still work. In the tradition its action is warming. It is used to:

  • warm the channels and dispel Cold and Damp;
  • move Qi and Blood where they have grown sluggish and cold;
  • support Yang where it is depleted.

That is the frame behind its clustering around cold-type period pain, aching joints that ease with heat, and the tired, cold, deficient patterns. In each, a practitioner reaches for warmth instead of a draining technique.

Mugwort, ai ye, is classically a warming, Blood-moving herb, and burning it is held to carry that warming, unblocking action into the point. Warming BL67 at the little toe to turn a breech baby sits inside this logic. It is read as moving Qi downward and outward, giving the baby room and impetus to turn. Moxibustion suits a cold or deficient pattern. It is used with more care, or avoided, where there is marked Heat, since adding warmth to a hot pattern is held to make it worse.

Burns are the main harm, with allergic reactions and infections also reported

The 2010 systematic review searched 14 databases and gathered adverse events from randomized and controlled trials, observational studies and case reports. The most common were burns, allergic reactions, and infections including cellulitis and hepatitis C, with trials also reporting blistering, itching, discomfort from smoke, and burns. In pregnant women, reports included premature rupture of membranes and bleeding from excess pressure on an anterior placenta. A separate clinical image documents a patterned moxibustion burn. No clear lung-function change in acupuncturists exposed to moxa smoke, though rhinitis (23%) was common

The 2019 study surveyed acupuncturists and ran retrospective lung-function testing comparing exposed and less-exposed practitioners. It found no significant difference in most large and small airway function indexes, and concluded moxa smoke had no significant effect on the respiratory health of acupuncturists. At the same time, expectoration (18.7%) and rhinitis (22.7%) were the commonest symptoms, and rhinitis and shortness of breath rose with exposure time. Burns are the main risk

The method works by heat held close to or on the skin, so the commonest problem is a burn or a blister, and infections have followed deeper burns. Keep the sensation warm, never painful, watch the skin the whole time, and treat direct moxibustion placed on the skin as a practitioner technique. Take extra care around thin skin and near the face.

Smoke needs ventilation

Burning moxa produces smoke that irritates the airways at close range; in a survey of acupuncturists working around it, a runny or blocked nose was the commonest complaint, at about 23 percent. Burn it in a well-ventilated room, keep sessions short, and use smokeless moxa sticks, or avoid it if you have asthma or another airway condition.

Not over numb or insensate skin

Do not apply heat where feeling is reduced, since the ordinary warning that the point has grown too hot is the sensation itself. Avoid moxibustion over numb or insensate skin, over reduced sensation from diabetes or nerve damage, and over broken skin, and skip it over varicose veins where heat is unhelpful.

Pregnancy only under supervision

The breech use is done from about 34 weeks and belongs under a midwife or a qualified practitioner, alongside usual maternity care, not in place of it. Do not self-treat in pregnancy, and never let moxibustion replace standard antenatal checks or a recommended external cephalic version.
