What It Is
PEMF applies a magnetic field that switches on and off many times a second, and its pulses pass through skin and bone without being felt. Under one name are two very different products. One is an FDA-cleared clinical stimulator held over a single bone. The other is a consumer mat that spreads a weak, diffuse field across the whole body. These mats differ from one another by roughly 100-fold in what they emit.
The evidence that a bone stimulator heals a fracture does not carry over to a full-body mat. They deliver different fields to different places, so a trial on one says nothing about the other.
What It Does
The strongest evidence is in bone. The largest pooled analysis of PEMF as an adjunct after spinal fusion surgery gathered 941 patients across seven trials. A meta-analysis found that adding electrical stimulation, PEMF among the methods, raised the odds of a successful fusion about two and a half times.
Bone-growth stimulators are cleared as well for fractures that have stalled or failed to knit. Direct evidence for the hardest cases is thinner: a Cochrane review could pool only 125 patients.
Osteoarthritis pain, studied mostly in the knee and hip, shows a small, mixed benefit over control arms. A 2026 analysis of 457 patients rated those trials at high risk of bias and judged the gains possibly too small to notice.
The full-body mats have the weakest evidence. A crossover trial in 108 women with fibromyalgia found the active mat matched a sham exactly: both arms improved by the same amount, the pattern you expect from a placebo. A review of whole-body consumer mats turned up only small trials, none with a positive result that a second study confirmed. The energy, recovery, sleep and wellness claims printed on the mats are ones no trial has tested. One device is routinely confused with these mats: rTMS, repetitive transcranial magnetic stimulation. It aims a magnetic coil at a single brain region across a course of clinic visits, and it is cleared for major depression.

Bone Density
About two and a half times the odds of a successful spinal fusion
After spinal fusion surgery, wearing a device that delivers electrical or magnetic stimulation made the bones more likely to fuse successfully, about two and a half times the odds seen without it, across the pooled trials.
A systematic review and meta-analysis of seven randomized controlled trials (941 patients) found that post-operative electrical stimulation increased the odds of a successful radiographic spinal fusion about two and a half times relative to control or sham (odds ratio 2.53, 95% confidence interval 1.86 to 3.43), with the evidence graded moderate quality. The stimulation methods pooled together included pulsed electromagnetic fields alongside direct current and capacitive coupling, so the estimate is for electrical stimulation as a class, not for PEMF isolated. Measured in: Adults undergoing spinal fusion surgery, seven randomized controlled trials (941 patients), 487 stimulated and 454 control or sham. The meta-analysis pooled pulsed electromagnetic fields with direct current and capacitive coupling, so the two-and-a-half-fold effect is for electrical stimulation as a class, not for PEMF measured on its own.

FDA-cleared to help fractures that have stopped healing (nonunion) knit
For a broken bone that has stalled and will not knit back together, a pulsed magnetic field held over the site can help it heal, and this use is cleared by the FDA. It is the strongest thing PEMF is used for.
The US Food and Drug Administration has cleared pulsed electromagnetic field (PEMF) stimulators as a treatment for nonunion of bone, a fracture that has stopped healing. A review of the mechanism describes how the field acts at cell-membrane adenosine receptors to push bone-forming cells toward building matrix and toward reducing inflammation, giving a cellular rationale for the clinical clearance. This is the best-established indication for PEMF, though it rests on a regulatory clearance and a worked-out mechanism more than on large modern randomized trials. Measured in: Adults with nonunion of bone; mechanism established in cell and animal work with clinical use in mixed adult populations. The clearance and the cellular mechanism are well established, but the direct randomized-trial evidence for the hardest fracture cases is limited, so the strength of the clinical proof is less than the clearance alone suggests.

Only four small trials, 125 patients, too few to settle non-healing fractures
When researchers gathered the best controlled studies on using magnetic fields for fractures that heal slowly or not at all, they found only a handful of small ones and could not say for sure whether it works. The regulatory clearance is real, but the trial evidence behind it for these cases is thin.
A Cochrane systematic review of electromagnetic field stimulation for delayed union and non-union of long-bone fractures in adults identified only four small randomized placebo-controlled trials, together enrolling 125 participants, each with methodological limitations and most data relating to non-union of the tibia. The reviewers concluded the available evidence was insufficient to establish whether the treatment changes the proportion of fractures that heal. This is the counterweight to the FDA clearance: the direct trial base for the hardest cases is small and uncertain. Measured in: Adults with delayed union or non-union of long bones, across four randomized placebo-controlled trials (125 participants). Only four small trials with methodological limitations were available, and most data concerned tibial non-union, so the finding is about the limits of the current trial base, not a demonstration that the treatment fails.

Anatomy of the Practice
1What the field does in tissue
A pulsed magnetic field passes through the body painlessly and induces small electrical currents in the tissue it crosses. Bone and cartilage cells respond to these currents, and that response is what the clinical devices are designed to trigger. The strength and pulse pattern of the field set how much signal reaches the cells.
2Inside the cell
Lab work traces the effect to cell-membrane receptors, chiefly the A2A and A3 adenosine receptors. Through them the field pushes bone and cartilage cells to build more matrix and produce fewer inflammatory signals. This comes largely from cell and animal studies. It gives a plausible reason the bone devices work, though it does not by itself prove that any given consumer product does anything.
3Over a course of use
Where PEMF has a clinical effect (on a stalled fracture, or on bone after fusion surgery) it works over weeks to months of daily wear, not one session. The dose only counts if an adequate field reaches the right spot.
How To Do It
Ways to Do It
PEMF splits into a medical decision and a consumer purchase, so decide which one you are in first. The clinical uses belong with a clinician and a specific device; the consumer mats are best approached with modest expectations. A field's strength, pulse frequency, placement and duration determine whether it does anything, so a device vague about all four cannot tell you the dose you are getting.
The evidenced uses are clinical: a stalled or non-healing fracture, or bone healing after spinal fusion, both handled with an orthopedic team and a specific cleared device. For knee or hip arthritis pain, expect little. The energy, recovery and general-wellness claims are the least supported. Decide what you want it for first.
If a fracture is slow to heal, or you are recovering from spinal fusion, raise PEMF with the surgeon. They can tell you whether a cleared bone-growth stimulator fits your case and arrange the device and course.
Consumer mats and pads vary widely. Some state their field strength and pulse frequency; others publish almost nothing. If you are buying a home device, favor one that states its output. The measured benefit beyond bone is small, so expect it to help joint pain modestly, if at all.
Where PEMF works it works over weeks, so judge a joint over a couple of months, not a session or two. Expect a gradual change at most, and be ready to conclude it did little for you. The claim that a wellness mat recharges your cells is one no trial has shown.

The Chinese Medicine View
In Chinese medicine, pain and slow healing read as stagnation, a place where the movement of Qi and Blood has become stuck. Yang is the warming, activating principle behind repair. A device that stimulates local repair loosely echoes the classical goal of moving Qi and Blood and warming Yang in one spot. Classical medicine never described electromagnetic therapy, so this is a rough modern analogy, not a claim the tradition makes.
The same lens raises a caution. Empty heat shows as the night sweats, dry mouth and restlessness of Yin deficiency. In that pattern a tradition-minded practitioner grows more careful with anything framed as adding activity or heat. A practitioner weighs the person, their constitution and their complaint, then tailors the approach to the individual.

Not with a pacemaker or implanted electronic device
A pulsed magnetic field can interfere with the electronics of a pacemaker, implantable defibrillator, neurostimulator, or an insulin pump. Anyone with an implanted electronic device should not use PEMF unless a cardiologist or the relevant specialist has cleared the specific device and situation. This is the single firmest contraindication, and it applies to consumer mats as much as to clinical units.
Caution in pregnancy
The effect of PEMF on a developing pregnancy has not been studied, so the field is best avoided over or near the abdomen during pregnancy. If you are pregnant or may be, treat any PEMF device with caution and raise it with your clinician before use.
Metal implants and active cancer, discuss first
People with a metal implant, or with active cancer in the area being treated, are usually kept out of the trials, so the effect in those situations is not established. If either applies to you, discuss PEMF with the clinician managing that condition before starting; do not assume a consumer device is neutral.
The main risk is money, not injury
Reviews of whole-body PEMF mats report no acute adverse effects, though effects from long-term use have not been studied. For most people the risk is money spent on a device, or on a wellness claim the evidence does not support. A mat that is vague about its output may deliver too little to do anything, or simply less than its marketing implies.

Common Questions
Can a full-body mat heal a broken bone?
No. The cleared bone devices are targeted stimulators an orthopedic team arranges, held over the exact spot and worn steadily through a course of treatment. A wellness mat covers the whole body with a weak field instead. A stalled fracture is a question for the surgeon.
How is a PEMF mat different from a TENS or muscle-stimulation unit?
They put current into tissue by different routes. A muscle-stimulation or TENS pad runs current straight through skin electrodes to contract a muscle or mask pain. PEMF induces a similar current with a magnetic field and no electrodes, and it is aimed at bone and cartilage. One works on muscle and nerve, the other on the skeleton.
Is a mat worth trying for knee or hip arthritis?
The measured pain relief is modest, so keep expectations low if you try one. For arthritis the strongest lever is not a device at all: regular exercise does more for joint pain than any mat, and it costs nothing. If you buy a mat, keep exercise as the main effort and add the mat as an extra.
Is PEMF safe to use?
For most people PEMF is low-risk. The one firm exception is an implanted electronic device: see the pacemaker caution above. Pregnancy, a metal implant and active cancer each call for a clinician's go-ahead first, also noted above.
