What Acne Actually Is

Acne is a disorder of the oil gland and its follicle, the tiny pit each hair grows from. Four things go wrong inside the pore at once:

  • The gland makes more oil (sebum), of an altered kind.
  • The pore's lining over-produces sticky cells that plug the opening.
  • Cutibacterium acnes multiplies in the trapped oil.
  • The follicle wall inflames, turning a hidden plug into a red, sore spot.

A blocked but quiet pore is a whitehead or blackhead. Once it inflames it becomes a papule, a pustule, or, at the deep end, a painful nodule or cyst.

Hormones set the process off. Androgens, the group that includes testosterone, stimulate the oil gland. Insulin and IGF-1 amplify that signal. That is why acne arrives with puberty, and why the diet links below run through the same hormonal pathway. Acne also runs in families, and how strongly skin responds to normal hormone levels is largely inherited. Not one of these four causes is dirt, so acne forms below the surface and can't be scrubbed away.

What Clears It, In Order

In a network meta-analysis of 179 randomized trials, a retinoid combined with benzoyl peroxide cleared about 26% more lesions than placebo. A treatment needs eight to twelve weeks to show, and the usual reason one disappoints is quitting it too soon.

The Treatments

The topical backbone pairs a retinoid with benzoyl peroxide. Topical retinoids (adapalene, tretinoin, tazarotene) loosen the plug that clogs the pore, and dermatology guidelines back them most strongly. Benzoyl peroxide clears inflamed spots and kills C. acnes without breeding resistance. A Cochrane review of 120 trials and 29,592 people found it beat placebo (risk ratio 1.27, about 27% more people improved).

Antibiotics, whether cream or pill, ease inflamed acne, but the effect is short-lived and they should never be used on their own. More than half of C. acnes strains now resist topical antibiotics in many countries. Guidelines therefore pair any antibiotic with benzoyl peroxide to slow resistance. They also hold oral courses to about three months, while the retinoid and benzoyl peroxide take over the lasting work. An antibiotic taken alone for many months is the pattern that breeds resistance and eventually stops working.

For women, two tablets lower androgens at the source. The combined birth-control pill lowers that signal and clears red spots and blackheads better than placebo over a few cycles. Spironolactone blocks androgens: in the SAFA trial, 82% of women reported improvement by six months against 63% on placebo, with the benefit building slowly. The same androgen excess links acne to polycystic ovary syndrome in some women.

Azelaic acid, a gentler option, clears spots about as well as a retinoid (risk ratio 0.94 against tretinoin). It also fades the brown marks acne leaves behind, and it is one of the few acne treatments safe in pregnancy.

Isotretinoin works directly on the oil gland and dampens all four drivers at once. That is why it offers the best chance of lasting clearance, and often clears severe acne for good. Doctors reserve it for the severe end: deep, scarring acne, acne causing real distress, or acne that fails standard therapy.

Deep, nodular acne can scar permanently, and the scars are far harder to fix than the acne itself, so treat it early.

How It Works

Each proven treatment breaks a different link in the chain from oil to inflamed spot:

  • A retinoid loosens the sticky plug so pores stop clogging.
  • Benzoyl peroxide clears the trapped bacteria and settles the redness.
  • Antibiotics lower the bacterial and inflammatory load for a while.
  • The hormonal treatments turn down the androgen signal at the top of the chain, for women whose acne flares with the menstrual cycle.
  • Isotretinoin shrinks the oil gland itself and works on every driver at once, the one treatment that can end the process for good.

Where Diet Fits

A whole-foods, lower-glycemic way of eating works through the same insulin and IGF-1 pathway that hormones use. In a twelve-week trial of young men, a low-glycemic-load diet, lower in sugar and refined carbohydrate, cut acne spots by 23.5 on average. The ordinary diet cut them by 12.0. Most of that sugar and refined carbohydrate reaches people through ultra-processed food. It supplies about 57% of the calories the average American adult eats, the industrial diet behind much metabolic disease.

Milk shows a similar signal. Across observational studies, people who drink more milk have higher odds of acne. Skim milk shows the strongest association (odds ratio 1.82), while yogurt and cheese show none. Both effects are modest, so a lower-glycemic diet and less milk are low-risk things to try if your acne is stubborn.

Two more have early support. Zinc lowers inflamed spots, and people with acne tend to run low on it. In an early trial, 2,000 mg of omega-3 as EPA and DHA reduced acne lesions over ten weeks, probably by calming inflammation.

What Does Not Cause Acne

Some old suspects do not hold up. Chocolate and greasy food have not been shown to cause acne. The only dietary links that hold up in large reviews are glycemic load and dairy. The case against chocolate rests on small, conflicting studies, including one where 25 acne-prone men ate dark chocolate every day and their skin got worse. Someone who notices their own skin flaring after a particular food can act on that. The general suspicion of chocolate and fatty food does not match the evidence.

Washing harder does the opposite of what people expect. A six-week trial compared washing once, twice, or four times a day:

  • Overall there was no difference between the groups.
  • Washing twice a day helped blackheads and whiteheads most.
  • Washing only once a day left more redness and inflamed spots.

Scrubbing and over-washing strip and irritate the skin, and that irritation inflames acne. Gentle cleansing twice a day is the amount that helps. Picking and squeezing drive inflammation and scarring. Home remedies such as toothpaste and lemon juice have not been shown to help and can irritate the skin.

Isotretinoin and pregnancy: the one non-negotiable

Isotretinoin causes severe birth defects. Anyone who could become pregnant takes it only under a formal pregnancy-prevention program (iPLEDGE in the United States). The program requires reliable contraception plus pregnancy tests before, during, and after treatment. It also calls for blood-test monitoring of the liver and blood fats. Dry lips and skin are near-universal and manageable. Mood changes are worth watching for and reporting, so a dermatologist prescribes and follows it. Serious adverse events are rare, and most effects reverse when the course ends.

The hormonal tablets are for women and avoided in pregnancy

The combined pill and spironolactone both act on androgens and are not used in pregnancy. The pill carries the usual considerations, including a small clot risk. Spironolactone can raise potassium and increase urination at higher doses. Both are chosen and monitored with a prescriber.

Topicals irritate before they settle

Retinoids and benzoyl peroxide commonly dry, redden, or sting the skin in the first few weeks. Starting every other night, using a lower strength, and adding a plain moisturizer keeps most people on them. Retinoids increase sun sensitivity, so daytime sunscreen goes with them. Benzoyl peroxide bleaches fabric and marks towels and pillowcases.

When to See Someone

Most acne is handled with the topicals and habits above and never needs more. These are the situations where a professional, usually a dermatologist, is the right next step, and two of them need prompt care:

  • Deep, painful nodules or cysts, or acne already leaving scars or brown marks. Nodular acne can leave permanent scars that are much tougher to treat than the acne, so start early. Isotretinoin works best before the scars form.
  • Acne that has not improved at all after about three months of over-the-counter treatment, which usually means a prescription regimen is the next step.
  • In a woman, acne together with irregular periods, excess facial or body hair, or scalp thinning, which can point to polycystic ovary syndrome or another hormonal cause worth investigating.
  • A sudden, severe onset with a deepening voice or rapid new hair growth (virilization), which needs a hormonal work-up; a skin cream will not address it.(seek urgent care)
  • A very sudden, severe, ulcerating breakout with fever or aching joints. This can be acne fulminans and needs prompt specialist care.(seek urgent care)

Acne is common and treatable, and most people never need more than one or two well-chosen treatments and the patience to let them work. For deep or scarring acne, a dermatologist can start isotretinoin before scars form.

What is the best treatment for acne?

For most people it is a topical retinoid combined with benzoyl peroxide, used together every day and given eight to twelve weeks to work. The retinoid unclogs pores while benzoyl peroxide settles inflamed spots and kills the acne bacterium. Together they outperformed placebo by about 26% more lesions cleared in a network meta-analysis of 179 trials. If that is not enough, the next steps are a short antibiotic course alongside benzoyl peroxide, a hormonal option for women, or a dermatologist referral for the severe end.

Does chocolate cause acne?

Chocolate has never been shown to cause acne. Across systematic reviews the diet signals that hold up are high glycemic load and milk. Chocolate rests on only small, mixed studies, including one trial of 25 acne-prone men eating dark chocolate daily that reported worse scores. Greasy food has not been shown to drive it either. If your own skin reacts to a particular food, you can act on that, but the general suspicion of chocolate does not match the evidence.

Do I need to wash my face more often?

No. When a six-week trial had people cleanse once, twice, or four times daily, it found no overall difference. Cleansing a single time each day left the most redness and inflamed spots, while four washes added nothing. Scrubbing and over-washing irritate the skin and can worsen acne. Washing gently twice daily with a mild cleanser is enough, and acne is not caused by a dirty face, so scrubbing harder does nothing for it.

Does diet affect acne?

A little, and in specific ways. A low-glycemic-load diet cut acne lesions by 23.5 against 12.0 on a control diet over twelve weeks in young men. Drinking more milk, skim milk in particular (odds ratio 1.82), is linked to more acne, while yogurt and cheese are not. These are modest effects working through insulin and IGF-1. Lowering refined carbohydrate and trying less milk are reasonable things to test if your acne is stubborn, alongside the topical treatments as add-ons.

How long does acne treatment take to work?

Eight to twelve weeks for most treatments before you can judge them.
