Condition: Fatiga y falta de energía

My Plan

What It Is

About six everyday causes explain most tiredness that will not lift; fatigue is the symptom they share. Pinning down which one gets you most of the way there. A ferritin test, a question about snoring and daytime sleepiness, a thyroid panel, a question about mood, and a medication review together pinpoint the cause in most cases. Each cause has its own treatment, and treating the wrong thing is how tiredness drags on for months.

Poor or broken sleep tops the list, along with the sleep apnea that hides behind it. Loud snoring with pauses in breathing is the tell, and no habit or herb fixes tiredness caused by interrupted breathing. Low iron drains energy even before it becomes full anemia, most often in women with heavy periods, and a ferritin blood test settles it. An underactive thyroid, poorly controlled diabetes, and other conditions surface as fatigue too.

Low mood, depression, and anxiety lower energy and disturb sleep. Many everyday prescriptions carry tiredness as a side effect, so a medicine you already take can be the entire cause. Long stretches of inactivity leave the body tiring from activity it once handled easily, a state called deconditioning.

If even small physical or mental effort reliably makes you crash a day or two later, that pattern is post-exertional malaise, and it points to ME/CFS. Unlike every other cause here, it does not ease when you push harder.

What Helps

When a ferritin test shows iron stores are genuinely low, an iron course brings energy back. Treating low iron eases the feeling of fatigue. Iron only helps when the test says you are short. Unexplained low iron in a man or an older woman needs its underlying cause found before it is topped up.

When loud snoring and breathing pauses are the clue, energy returns once the apnea itself is treated, because the tiredness comes from breathing that keeps stopping through the night. No amount of sleep hygiene or supplements fixes that. For broken sleep, the behavioral program for insomnia restores the sleep that daytime energy depends on, and works better than sleeping pills.

True thyroid disease responds to treatment: when thyroid hormone is low and TSH is high, replacing the hormone lifts the fatigue. A mildly raised TSH with normal thyroid hormone, called subclinical hypothyroidism, is common in older people and often gets blamed for tiredness. Treating that borderline number on its own does not relieve it.

For the ordinary tiredness of being out of condition, the answer is to move more, and that alone raises energy as the body gets back in shape. The fix is to start easy and add a little each week.

In cancer-related fatigue, the evidence points toward activity scaled to what a person can manage, worked out with the care team, and medication does little for it.

Caffeine masks tiredness for a bad night or a night shift; it does not remove the cause. Taken late, it shortens and lightens the next night's sleep, so keep it to the morning.

A few supplements show early promise, but only in specific situations: vitamin D when you are tired and deficient, American ginseng for cancer-related fatigue, and standardized rhodiola for stress-related burnout. Each is reasonable to try in the right case, and none is a reliable fix.

In plain terms

In plain terms

In detail

Who this may not transfer to:Both trials enrolled only menstruating women, the group most prone to iron deficiency. Whether iron eases fatigue in men or postmenopausal women with low ferritin was not tested here, and unexplained low iron in a man or an older woman needs its cause found, not simply replaced.

How to use it

In plain terms

In detail

How to use it

In plain terms

In detail

In detail

In plain terms

In detail

How to use it

In plain terms

In detail

In plain terms

In detail

In plain terms

In detail

How to use it

The exception: a crash after effort points to ME/CFS

The defining feature of ME/CFS is post-exertional malaise: a heavy, delayed slump that lands a day or two after even a small effort. Building activity up, the right move for deconditioning, makes this worse, because pushing past your energy limit can leave you unable to function for days.

The defining feature of ME/CFS is post-exertional malaise: a heavy, delayed slump that lands a day or two after even a small effort. Building activity up, the right move for deconditioning, makes this worse, because pushing past your energy limit can leave you unable to function for days.

In 2021, the NICE guideline for ME/CFS dropped graded exercise therapy and advised against any program built on fixed, step-by-step increases in activity. The approach it backs instead is pacing: staying inside your energy envelope, worked out with a doctor who treats ME/CFS.

The trial evidence is genuinely contested. In chronic fatigue syndrome, structured exercise has reduced fatigue by the end of a program. But the trials could not settle whether the benefit lasts or whether it harms some people, and most were run before post-exertional malaise became the defining criterion. The large PACE trial once reported that graded exercise and cognitive behavioral therapy helped people recover; re-analyzed under its own original rules, those recovery gains largely vanished.

Cognitive behavioral therapy has moderate support for coping and for gradually rebuilding activity. It helps a person manage the illness while the tiredness itself stays real.

What the evidence does not support

'Adrenal fatigue' is the idea that everyday stress wears the adrenal glands down until they make too little cortisol. It is not a recognized diagnosis: a systematic review of 58 studies found no consistent support for it, and the saliva-cortisol tests sold to find it are read inconsistently. Stress-related tiredness is real; worn adrenal glands do not explain it. A genuinely underactive adrenal gland, Addison's disease, is a separate illness with its own diagnosis. Energy supplements and B12 shots do little unless a blood test shows a real shortfall of iron, vitamin D or thyroid hormone.

Working Through Your Own Tiredness

None of this replaces a proper work-up. Work through the treatable causes first, that is where the real gains are.

1
Get the cheap, high-yield checks firstFree to $Easy

Ask your doctor for a ferritin and thyroid panel, mention any loud snoring or daytime sleepiness so sleep apnea can be assessed, and be straight about your mood. These few checks find the cause in most people, and each cause has a treatment that targets the actual problem.

2
Review your medication with a prescriberFreeEasy

Many common medicines list tiredness among their effects. Before adding anything, ask whether something you already take could be the cause, since a change there can lift the fatigue at no extra cost.

3
Protect sleep and a steady body clockFreeModerate

Keep regular hours, get daylight early, and move caffeine to earlier in the day. If broken nights are the real problem, the behavioral program for insomnia repairs them better than sleeping pills do.

4
Build activity back gradually, unless you crash after effortFreeModerate

For ordinary, out-of-condition tiredness, start with gentle regular activity and build slowly. If even small effort reliably triggers a delayed crash, pace within your limits instead and get assessed for ME/CFS.

5
Eat and drink for steady energyFree to $Moderate

Regular whole-food meals blunt the blood-sugar swings that leave energy flat, and cutting back on alcohol removes a common drag. This does more for steady energy than any single energy food or drink.

6
Test before adding a supplementFree to $Easy

Supplements help mainly when a blood test shows you are short of something. Correcting a real iron or vitamin D deficiency raises energy; energy blends, B12 shots and adrenal-fatigue protocols do little unless a test shows a genuine deficiency.

Go Deeper

Four everyday habits lift baseline energy once the treatable causes are handled:

The Chinese Medicine View

Chinese medicine sorts tiredness into several patterns, each read from the whole picture: build, digestion, sleep, mood and pulse.

The Chinese Medicine View

Chinese medicine never treated tiredness as one thing. It reads it by pattern, asking which system is depleted and what the rest of the picture shows, and the pattern points to what supports it. This is an interpretive lens, and a practitioner is the right way to work with it: reading the pulse and tongue is part of the diagnosis. It runs alongside the medical work-up. A fatigue read as Spleen Qi deficiency may also be low iron on a blood test, and both can be true.

Spleen Qi Deficiency

The classic tired pattern: heavy fatigue that is worse after eating, poor appetite, loose stools, a pale swollen tongue with teeth marks. The Spleen is read as the source of Qi from food, so when it is weak, energy is not made. The direction is to strengthen the Spleen and support Qi.

Qi And Blood Deficiency

Tiredness with a pale complexion, dizziness on standing, palpitations, poor memory and, in women, light or late periods. Read as too little of both Qi, the energy, and Blood, the substance it moves through the body, a picture that overlaps with what Western testing calls anemia.

Kidney Deficiency

A deeper, constitutional exhaustion with lower back and knee weakness, low drive, and either cold limbs and frequent pale urination (Yang) or night sweats and heat in the palms (Yin). Read as depletion of the deepest reserve the body holds, often with age, overwork or long illness.

Liver Qi Stagnation

Tiredness that comes with tension, irritability, a sense of pressure, and energy that rises and falls with stress. Read as Qi that is stuck, so the direction is to move and soothe. It is the tiredness of strain with no outlet, easing once the strain lifts; tonifying alone does not address it.

Damp Encumbering The Spleen

A foggy, heavy tiredness with a muzzy head, a feeling of heaviness in the limbs, poor appetite and a thick tongue coat, often with rich food, sweet food or humidity in the picture. Read as dampness weighing the Spleen down, and the direction is to dry damp and lighten the diet. Rich tonics tend to make it worse.

Cautions

Extra restraint

Everything to be aware of is here, in one place. This practice suits most healthy people; a few situations call for real care.

Common Questions

Why am I tired all the time even though I sleep?

Eight hours in bed do not guarantee restorative sleep. If your nights are broken, or an underactive thyroid, low iron, a medicine, or low mood is behind it, more hours will not touch it. The tiredness of low mood in particular often turns up before you notice the mood itself.

Should I get my iron checked, and will an iron supplement help?

In trials of women who were tired but not anemic, iron eased fatigue more than a placebo, but only when their ferritin was at or below 50 micrograms per liter. Even then, it did not raise measured exercise capacity. Iron taken on a hunch can irritate the gut and build up when stores are already full. So a ferritin test comes first, iron only if it reads low, and a recheck after about six weeks.

I crash for a day or two after any effort. Is that different?

Yes. That delayed crash after light effort is post-exertional malaise, the hallmark of ME/CFS. Pacing inside your energy limits is the safer course, and pushing through can set you back for days.

When does tiredness mean I should see a doctor?

Most tiredness is ordinary, but a few pairings send you to a professional. Weight loss you cannot explain, alongside tiredness, is the one doctors act on soonest. Breathlessness, chest pain or a very pale look, heavy thirst with a lot of urine, a persistent low mood, or fatigue that is getting worse fast are the rest.

When to See Someone

Most fatigue is not dangerous and traces to one of the everyday causes. See a doctor, and urgently where noted, if you have:

  • Fatigue with unexplained weight loss, fevers, drenching night sweats, or a new lump. Tiredness with weight loss in particular is a combination doctors investigate promptly(seek urgent care)
  • Breathlessness, chest pain, or looking very pale. These can point to anemia or the heart(seek urgent care)
  • Fatigue with heavy thirst and passing a lot of urine. This can point to diabetes
  • New severe fatigue, or tiredness that keeps worsening quickly
  • Fatigue alongside persistent low mood, loss of interest, or any thoughts of harming yourself(seek urgent care)
  • A delayed crash lasting a day or more after even small physical or mental effort (post-exertional malaise). This points to ME/CFS, where pushing through can make things worse, so it is managed by pacing

None of this is meant to alarm you. Tiredness usually means something ordinary is needed: more sleep, more iron, more movement, or less strain. The list is only to catch the few times it points to something more. Start slow, get the simple things checked, and see someone if any of the above fit.
