What It Is
Periodontitis is a long-running inflammation of the tissues that hold a tooth in its socket. It is set off by the plaque biofilm, the sticky bacterial film that gathers along the gumline. It has two stages. Gingivitis comes first: the gum reddens, puffs and bleeds when you brush or clean between the teeth. No bone is lost yet, and gingivitis reverses fully once the plaque is removed and kept off. In susceptible people the inflammation deepens into periodontitis. The gum pulls away from the tooth into a space called a pocket, and the body's response to the bacteria dissolves the bone that anchors the tooth. Lost bone does not grow back on its own, so teeth loosen, drift and some are lost, usually after years of near-painless progress.
Bringing the inflammation under control halts further bone loss.
What Helps
Daily cleaning and professional scaling control the disease; the products sold for gums add little on top. A brush along the gumline clears the biofilm from the outer and inner faces of the teeth, yet leaves the surfaces between them, where periodontitis often begins. That gap is why cleaning between the teeth matters. Across 35 trials, adding interdental cleaning to brushing tended to reduce gum inflammation. Interdental brushes worked at least as well as floss, and were easier for many people to use well. The trials were short and low in certainty, so the exact size of the benefit is unknown. But every trial pointed the same way, toward less inflammation, and it costs a few cents a day.
Below the gumline the deposits harden into a crust no brush can lift, and scraping it away, scaling and root planing, is the step that stops active disease. The tool and the number of visits did not matter: hand, sonic and ultrasonic all worked, and one long appointment did as well as several. Scaling also works better once a smoker quits. Smokers carried about 80% higher risk than people who had quit, and quitting brings the risk back toward a never-smoker's, with the gums recovering more afterward. Smoking narrows the small vessels in the gum and hides the bleeding that would signal inflammation, so the disease advances further before it is noticed.
What To Do First
None of this needs a specialist to begin, and the daily part determines the outcome. The order matters less than doing the first few steadily. Start with the cleaning a brush cannot do, book the professional clean below the gumline, and deal with smoking and blood sugar if they apply to you.
A brush misses the surfaces between the teeth. Use floss or an interdental brush once a day. This is the single lever most in your own hands.
A soft brush angled at the gumline for two minutes, twice a day, removes the biofilm that drives the disease. Keep the technique light and thorough.
Scaling and root planing lifts the hardened deposits from below the gumline, where no brush or floss reaches. It is the professional step the daily cleaning cannot replace.
Quitting lowers the risk sharply and makes the scaling work better. It is the hardest of these steps and the one that changes the disease most.
Gum disease and diabetes each affect the other, so manage them together. Treating the gums lowers blood sugar.
Regular hygienist visits lift the deposits a brush cannot reach and catch deepening pockets early. Steady maintenance is what keeps the teeth once the active disease is under control.
A few extras each add a little more benefit on top of that base. Low-dose doxycycline slows the enzymes that break down gum tissue. At 20 mg twice a day for three months, it improved attachment, the grip of gum and bone on the tooth, and pocket depth against a dummy pill. The dose sits below the level that acts as an antibiotic. A tetracycline-class antibiotic placed straight into a stubborn pocket added about half a millimeter of extra depth reduction over several months, with less gain in smokers. In the laboratory, cranberry acted against some gum bacteria but helped others cling to mouth cells, and its acids can erode enamel. The evidence is mixed, and cranberry has never been tested for gum disease in people.

The Diabetes and Heart Links
High blood sugar and gum disease feed each other. High blood sugar inflames the gums and makes periodontitis more likely and more severe, and the sore gums push blood sugar back up. Clearing the gum inflammation eases that loop, dropping long-term blood sugar about 0.43 percentage points, about what a mild glucose-lowering drug adds. Most type 2 diabetes is driven by diet and excess weight, so losing weight and eating whole, less-processed food lowers the same blood sugar that feeds the gum inflammation.
The heart link is only a correlation: gum disease occurs alongside heart disease, but treating the gums has not lowered the rate of heart attacks. People with periodontal disease carried about 24% higher risk of major cardiovascular events. Periodontitis and clogged arteries share the same major drivers (smoking, aging, diabetes and social circumstances) so much of the overlap may come from shared causes.

Treating gum disease during pregnancy did not clearly reduce preterm birth before 37 weeks, though it may lower the chance of a low-birthweight baby. The treatment is safe in pregnancy and does not bring on an early delivery, so it is done for the mouth's own sake.

Chlorhexidine stains the teeth with long use
Chlorhexidine mouthrinse cuts plaque sharply, but used for four weeks or longer it stains the teeth and can dull taste. It is usually given as a short course around treatment, on the schedule your dentist sets.
Antibiotic tablets are held back for the aggressive cases
A course of antibiotic tablets alongside scaling adds only a small extra benefit in ordinary chronic periodontitis, and taking them routinely for a small gain drives antibiotic resistance. They are reserved for fast-progressing or aggressive disease, decided with the dentist.
A hard brush and heavy scrubbing wear the gum back
The plaque comes off with a soft brush and a gentle, thorough technique. Scrubbing hard with a stiff brush abrades the gum and the root and drives recession over the years, so a light, regular technique is the aim.

The Chinese Medicine View
In Chinese medicine the mouth belongs to more than one organ. The soft gum is governed mainly by the Stomach, the hard tooth by the Kidney, so a bleeding gum and a loosening tooth can point to two different imbalances.

The Stomach channel crosses the upper gum, the Large Intestine channel the lower. Red, swollen, bleeding gums read as Stomach heat. Rich, greasy, spicy or over-sweet food is thought to generate this heat, which then rises along the Stomach channel to the mouth. The teeth are read differently. They are seen as an extension of the Kidney: grown from the same essence, or Jing, that builds bone and marrow. So teeth that loosen or lengthen with age read as declining essence as much as a local problem. Loose teeth with a pale tongue and a weak low back point to Kidney decline, and the aim is to build the body up. The swollen, throbbing gum of an abscess is a matter for a dentist first, whatever the pattern.
The inflamed, active picture: red, swollen, bleeding, sometimes painful gums, bad breath, thirst, a red tongue with a yellow coat. The direction is to clear Stomach heat and cool the blood, and the dietary heat from rich, sweet and greasy food is addressed alongside it.
The chronic, low-grade picture: gums a dull, dark red, mild recession, a dry mouth, heat felt in the palms and at night, and a red tongue with little coat. Read as Stomach and Kidney Yin too weak to hold the fire down, so the aim is to nourish Yin and settle the deficient heat.
The slow, deficient picture common with age or depletion: teeth that loosen or lengthen as the gum recedes, weak low back and knees, a pale tongue. Since the teeth are read as an extension of the Kidney and the essence that governs bone, the direction is to tonify the Kidney and support that essence.
Common Questions
Can periodontitis be reversed?
Partly. Gingivitis, the early stage, heals completely once you clear the plaque and keep it off. Bone, once lost, does not grow back, so treatment ends the active disease without rebuilding lost bone. Pooled across nine trials, scaling lowered pocket depth about 1.4 mm and returned roughly three-quarters of the pockets to a healthy depth (Suvan, J Clin Periodontol 2020).
What is the single best thing I can do?
Cleaning between the teeth daily and, if you smoke, stopping are the two biggest levers you control. Quitting moves the disease more than anything on a shelf, and a former smoker's risk drops close to that of someone who never smoked (Leite, Nicotine Tob Res 2019).
Does treating my gums help my diabetes?
Yes, measurably. In people with both conditions, gum treatment brought long-term blood sugar down at three to four months, on moderate-certainty evidence (Simpson, Cochrane 2022). Better blood sugar eases the gum inflammation in return, so the two are managed together.
Do mouthwashes and supplements work?
Some help a little, on top of the daily basics. Oral probiotics, mostly Lactobacillus strains, improved pockets and bleeding across 12 small trials, but only while they were being taken. Vitamin D helped mainly people who were deficient, bringing their level back above 30 ng/mL. Oil pulling did no better than an ordinary rinse across five short trials (James, Cochrane 2017).
Does gum disease affect the heart?
Gum disease and heart disease appear together, but one has not been shown to cause the other. People with gum disease had a higher rate of heart attacks, strokes and death across 39 studies of more than four million people (Guo, PLoS One 2023). In trials, treating the gums has not reduced those events.
Is a swollen, painful gum an emergency?
Sometimes, yes. A rapidly swelling, throbbing patch of gum with pus, sometimes with a bad taste or a suddenly loose tooth, can be a periodontal abscess that needs same-day dental care. If the swelling reaches the face or neck, or you struggle to swallow or breathe, treat it as an emergency.
When to See Someone
Most of managing periodontitis is steady and in your own hands. A few situations need same-day care. The signs to act on:
- A fast-swelling, throbbing area of gum with pus, a bad taste, or a suddenly loose tooth. This can be a periodontal abscess needing same-day drainage(seek urgent care)
- Swelling spreading to the face, under the jaw or the floor of the mouth, difficulty swallowing or opening the mouth, or any trouble breathing. This is a spreading infection and an emergency(seek urgent care)
- A mouth ulcer, sore, or red or white patch that does not heal within two to three weeks. It needs checking for oral cancer, especially in people who smoke or drink heavily(seek urgent care)
- Teeth that are loosening, drifting apart or changing how they bite together. A dentist should assess and stage the bone loss
- Gums that keep bleeding beyond the first week or two of better cleaning, or persistent bad breath or a bad taste. These are signs the disease is still active
- Diabetes that is hard to control alongside gum disease, since the two affect each other and treating the gums can help the blood sugar
Periodontitis is one of the more controllable chronic conditions. Act fast only on a swelling that could be an abscess or spreading infection.
