What It Predicts

PURE followed nearly 140,000 adults across 17 countries. Each 11 lb (5 kg) less grip came with about a 16% higher death rate, and a weaker grip predicted death more strongly than blood pressure did.

The pattern repeats at scale. Pooling 42 cohorts and three million people reproduced the link, in men and women alike, and it held after excluding those who were already sick. In the largest cohort, grip showed no association with diabetes, fractures, fall injuries or chest infections, so it points to no single disease. Most of this evidence is observational. A low reading partly picks out people already ill or frail, so the number tells you as much about your health now as about your future.

How to use it

What It Is

Grip strength is how hard your hand can squeeze, measured in kilograms on a hand dynamometer. A hard squeeze needs muscle, nerves, nutrition, and general vitality in shape at once, so the number stands in for the whole body. Your dominant hand is usually about 10% stronger, so note which hand you use.

How to Measure It

Sit with your feet flat and shoulders relaxed, elbow bent to about 90 degrees, forearm and wrist neutral. Squeeze as hard as you can for a couple of seconds. Take three squeezes per hand with a short rest between, then record the best or the average of the three. Because strength varies with age and sex, judge your number against others of your age and sex.

How to Improve It

Because a strong squeeze comes from total-body strength, you raise it by building strength across the whole body. In pooled exercise trials in older adults, whole-body training is the approach with the strongest evidence. Five foundational lifts carry most of the load: squats, deadlifts, rows, presses and loaded carries. A loaded carry, walking a set distance while holding heavy dumbbells or a loaded handle, loads the grip hard while training the whole posterior chain. Dedicated grip work, such as heavy static holds, thick-bar or towel training and farmer-style holds, is a small add-on, useful mainly when a weak grip limits your other lifts.

Grip predicts these outcomes; training the grip alone does not prevent them. The 42-cohort link is with whole-body strength, not the hands.

What It Is Not

European clinicians use grip as the first-line strength check for sarcopenia. Below 60 lb (27 kg) in men and 35 lb (16 kg) in women, they call sarcopenia probable, a threshold to investigate, not a diagnosis.

A low number is a signal, not a diagnosis

A hard squeeze depends on the whole body, so a low reading points to your overall strength, activity, and health. Use it to prompt action and, where warranted, a conversation with a clinician.

Local hand problems change the reading on their own

Hand or thumb osteoarthritis, a recent wrist or hand injury, carpal tunnel, tendon pain, or a rheumatoid flare can all lower grip on their own. None of these reflect whole-body strength. A one-sided drop, especially with pain, points to something local in that hand.

See a clinician for sudden weakness

A gradual change over months fits normal tracking. Sudden weakness in one hand or on one side, numbness, slurred speech, or a drooping face are stroke signs. Get urgent medical care.

Keep the test consistent, and stop if it hurts

The number is comparable over time only when the posture, hand and instrument stay the same, so a change in method can look like a change in strength. Squeeze hard, but stop short of joint pain, and if a maximal squeeze aggravates a sore wrist or thumb, ease off and track the trend with everyday tasks instead.

What is a good grip strength for my age?

Judge it against your age and sex, since men are stronger than women at every age. In large British data, grip peaks in the early thirties, about 112 lb (51 kg) for men and 68 lb (31 kg) for women. It holds through the forties, then declines from midlife.

Will a hand gripper or a tennis ball help?

A gripper or a squeezed tennis ball builds the grip muscles in your hands and forearms. The reserve that makes grip predict health comes from whole-body strength, and resistance training raises grip in older adults by a few kilograms while building that reserve.

What if I do not have a dynamometer?

A dynamometer gives you a number you can compare to research and track to the kilogram. The Jamar is the reference model, and a cheaper digital one still tracks your own trend even where it does not match a Jamar exactly. Without one, watch everyday tasks. A stiff jar that is suddenly hard to open, two grocery bags carried up the stairs, wringing out a towel, holding a heavy pan steady: watch for those. A clear, recent drop in any of them is worth acting on, especially when you also feel generally weaker.

Does grip strength predict dementia?

Stronger people are less likely to develop cognitive decline and dementia. Early, undiagnosed brain changes can lower activity and grip years before a diagnosis, so the evidence is observational and cannot prove cause. That signal pools 16 cohorts, including a new KIHD study, where a weaker grip tracked with aging in the brain and the body.
