Findings & Outcomes
The whole physical set takes about 15 minutes and draws no blood. A self-measure is a body number you can record at home without a clinician. Two ideas sort the ones worth doing from the rest. A marker reads your current state; a lever changes it. Grip strength and the balance tests are markers you cannot usefully train toward. Walking speed, the chair stand and blood pressure respond to being acted on. Practice a test directly and your score improves, but the risk it once predicted does not improve with it. For most measures here, almost no studies ask the next question: does raising your score actually improve your health?
Grip strength
Across 139,691 adults in 17 countries, each 11 lb (5 kg) less grip strength came with a 16% higher rate of death from any cause. In the same PURE cohort grip showed no association with injury from a fall, with fracture, or with new diabetes. It is a good general alarm and a poor specific one. Grip reflects whole-body strength; the hands are only where you read it. Squeezing a tennis ball every day raises the grip number, with no sign it lowers mortality, so the strength worth building comes from resistance training. A figure comparable to the published one needs a hand dynamometer, and the reference values were established on a Jamar dynamometer.
Walking speed
Walking speed predicts survival across its whole range, and improvement in it predicts survival too. A pooled cohort of 34,485 adults over 65 averaged 0.92 m/s, and every 0.1 m/s faster came with about 12% lower mortality. It carries as much prognostic weight as a model built from age, sex, chronic conditions, smoking, blood pressure, body mass index and hospitalization combined. When 439 adults over 65 were followed for eight years, only one of six health and function measures predicted who lived: improved gait speed. Mortality ran 31.6% in those who gained at least 0.1 m/s, against 49.3% in those who never did. Here the marker doubles as a lever: resistance training raised gait speed by 0.08 m/s across 24 trials, and plain walking is the daily version.

The 30-second chair stand
The 30-second chair stand reads lower-body strength, the thing that gets you off a low seat unaided. In its original validation the count tracked weight-adjusted leg press at r = 0.71 to 0.78. The US CDC STEADI fall-risk screen uses it and flags any count below the age and sex average. Those cut-offs mark capacity for staying physically independent; no falls study set them. It is the most trainable test here: progressive resistance training produced a moderate to large improvement in rising from a chair. Whether the better score carries lower fall risk with it has not been shown.

A straight-backed chair without arms, seat height 17 inches (43.2 cm), placed against a wall so it cannot slide. Sit in the middle, back straight, feet about shoulder width apart and flat.
Seat height is part of the test. A lower sofa or a higher stool gives a number that compares to nothing published.

Arms crossed at the wrists against the chest, and they stay there for the whole test. Pushing off your thighs measures something else.

The 10-second single-leg stand
The 10-second cut-off comes straight from the study that tracked who lived and who died. At a Brazilian exercise-medicine clinic, 1,702 adults aged 51 to 75 tried it, and one in five could not hold the pose. Over a median seven years, 17.5% of those who failed died, against 4.6% of those who held it, an adjusted hazard ratio of 1.84. The people studied signed themselves up, were 68% men, and the result had a wide margin of error. Treat it as a free signal and judge it against your own age group. Failing at 54 means something different from failing at 74. To try it, stand barefoot beside a counter you can grab, tuck the free foot behind the standing calf, and take up to three attempts on either leg.
The sitting-rising test
The sitting-rising test scores strength and flexibility together, from 0 to 10, by counting the hand and knee supports used to get down to the floor and back up. Among 2,002 adults at the same clinic, the lowest scorers, at 0 to 3, carried 5.44 times the death rate of a perfect 10. Each one-point rise came with 21% better survival. It shares that clinic population's limits. Like the balance and falls work, it needs a spotter if you are frail or have had a fall in the last twelve months.
The timed up and go
The timed up and go measures mobility reliably, correlating with the Berg Balance Scale and an activities-of-daily-living index. As a fall predictor it is weak. Pooled across 10 studies, its 13.5-second threshold had a specificity of 0.74 and a sensitivity of 0.31, missing roughly two thirds of the people who went on to fall. The review's own analysis could not show that the score predicts falls at all, so no cut-off separates the fallers from the rest.
Blood pressure, done right
Blood pressure is the number with the largest correctable measurement error, and the one most often taken wrong. A randomized crossover trial showed how much technique matters. A regular cuff read 19.5 mmHg high on arms that needed an extra-large one, and 4.8 mmHg high on those needing large.
Measuring badly adds more error than almost any change you could make in response to the reading.
Arm position adds its own error. Take a desk-supported arm at heart height as the baseline. An unsupported arm hanging at the side reads 6.5 mmHg higher; a hand resting in the lap, 3.9 mmHg higher.
When 2,081 Finnish adults aged 45 to 74 were followed for nearly seven years, home and office pressure went into the same model. Only the home number still predicted cardiovascular events, and the office reading stopped predicting anything. So a home reading is worth getting right. Nothing here, though, is a reason to change a prescription; a home reading is information for a conversation. The high blood pressure page covers what a raised figure means.

What the research did, and what we would suggest
The trial dose
TASMINH4 taught 1,182 hypertensive patients to use a validated automated monitor on the non-dominant arm. They measured twice each morning and twice each evening for the first week of every month, and their GP titrated medication on those numbers. At twelve months systolic pressure was 3.5 mmHg lower than usual care with self-monitoring alone, and 4.7 mmHg lower with telemonitoring added.
What we would suggest
If you have been told your pressure is high, do the same week-long block before your appointment and take the whole week's average with you. If you have never been told anything, one properly done week a year is enough to know whether the subject applies to you.
Why the difference: The effect came only where a prescriber acted on the readings; self-monitoring alone moved nothing. With nobody adjusting anything, a home number has no reason to fall. What the week does do is replace a single clinic measurement with a week's average. The clinic reading is taken in the worst conditions available; the average matches the conditions the risk associations were established under. Telemonitoring added only 1.2 mmHg over self-monitoring alone, which was not statistically significant, so the app is not what makes the method work.
The smallest version still worth doing: One correctly sized cuff, one arm supported at heart height, one week of mornings. That is most of the available accuracy.
Adjusting it to you: Chinese medicine has no equivalent of a pressure number and does not treat one. What it holds is that the pattern behind a raised reading differs between people. The red-faced, irritable, headachy picture of rising Liver Yang is settled and directed downward. A pale, exhausted, dizzy-on-standing picture is supported instead. Classical practice reads the surrounding pattern, the face, the energy, the tongue, and interprets the same monitor reading differently from one person to the next.

Waist and waist-to-height
Waist circumference tracks the fat that sits around the organs, the kind that carries risk. Some 359,387 European adults were followed for a mean of 9.7 years. Men in the highest fifth of waist had 2.05 times the death rate of the lowest fifth, adjusted for body mass index. Women in the top fifth had 1.78 times. Body mass index stayed significant in the same models, so waist and BMI measure two different things. Where you put the tape barely matters for what waist predicts. Across 120 studies the measuring spot made no substantial difference across a population, though a consistent spot matters for tracking yourself.
Measure standing, at the end of a normal breath out, the tape snug without denting the skin. Divide waist by height and you get a sharper predictor. Across 31 papers and more than 300,000 adults, waist-to-height ratio out-discriminated body mass index for hypertension, diabetes and cardiovascular outcomes by 4 to 5%. A 35-inch (90 cm) waist on a 69-inch (175 cm) frame is 0.51. The same visceral fat drives the insulin and glucose picture.
Resting heart rate
Resting heart rate is the pace your heart keeps at rest. Sit still for five minutes, then count your pulse for a full 60 seconds, at the same time of day and ideally before you rise. Pooled across 46 studies and 1,246,203 people, each 10 beats per minute higher came with a 9% higher rate of all-cause death. The line climbs straight from 45 bpm upward. Even inside the normal 60-to-100 range, a higher rate still means higher risk. Change counts too.
Among 29,325 Norwegian adults measured twice about a decade apart, a rise from under 70 to over 85 bpm carried a 1.9-fold adjusted risk of ischaemic-heart-disease death. A fall over the same years showed no general mortality benefit. One thing to know: an irregular rhythm makes both a manual count and a cuff unreliable. Heart rate variability is a separate measure. Its spread between people makes comparing your figure to anyone else's close to meaningless; breathwork and HRV covers it.
Sleep regularity
Ask people how long they sleep and they miss by nearly an hour. Measured against wrist actigraphy in 669 middle-aged adults, real sleep averaged 6.0 hours against 6.8 reported, and the two agreed only at 0.45. The shortest sleepers, near five hours, over-reported by about 1.3 hours. People report when they sleep far more accurately than how long they sleep. More than 10 million hours of accelerometer data came from 60,977 UK Biobank participants. The most regular four fifths had 20% to 48% lower all-cause mortality than the least regular fifth. Regularity beat duration as a predictor. You can catch it on paper: write your bed time and get-up time every day for two weeks. A paper log misses the night you woke at three and lay there until five. It records when you were in bed, which captures regularity but nothing about sleep quality. Sleep regularity is the practice built on this.
Wearables
Wearables count steps well and measure almost everything else poorly. Across 158 publications, Fitbit, Apple and Samsung devices counted steps accurately in laboratory conditions. No brand reads energy expenditure accurately, so the calorie figure is unreliable. Sleep staging fails where it is tested. Over three nights against polysomnography in 34 healthy young adults, consumer devices caught sleep well, at a sensitivity of 0.93 or better. They caught wake poorly, at a specificity between 0.18 and 0.54, and every one did worse on broken nights. The readiness or recovery score has no outcome literature behind it, unlike a walking speed or a blood pressure number.
It is computed largely from the sleep-staging inputs the devices measure least reliably. Total sleep time and step count are worth reading. There is a cost to watching too. Among 172 patients with atrial fibrillation, the 83 who used a wearable reported more symptom monitoring and preoccupation. One in five felt anxiety and always called a doctor after an irregular-rhythm alert, running significantly more ECGs, echocardiograms and ablations. That study looked backward, so it cannot say whether the wearable drove the anxiety or anxious people chose to wear one.

Two of these tests need equipment worth $30 or more; the rest run on household objects.
The limits
Screening yourself for a long list of conditions does not lower your risk of dying. 17 randomized trials pooled 251,891 participants. A package of checks for many diseases and risk factors made no difference to total mortality, at a risk ratio of 1.00 (95% CI 0.97 to 1.03). It made none to cancer mortality either, on high-certainty evidence. More screening found more disease; finding it did not help. The second limit is a ceiling on what home tools reach.
Among 122,007 adults referred for treadmill testing, the least fit quarter had 5.04 times the risk-adjusted mortality of elite performers. That gap outweighed coronary artery disease, smoking, or diabetes in the same model. Fitter was always better, with no point where the benefit leveled off. That test needs a lab and cannot be reproduced at home. Resting heart rate and self-reported walking pace are its rough proxies, and VO2max training is what actually raises it.

What Chinese medicine watches
A TCM pattern comes from several signs together; one sign alone means little. What a person is asked to notice belongs mostly to the asking pillar of the four examinations. That covers how sleep goes and how it breaks; appetite and digestion; thirst; stool and urine. It reaches sweating and its timing; where energy sits through the day; whether cold or heat troubles more; and, in women, the roughly 28-day cycle. The tongue belongs to looking, and you can watch your own in a mirror, first thing, before coffee, tea, toothpaste or a scrape, in daylight, tongue out and relaxed. Read it as a trend across weeks; a single day's look proves nothing. Tongue diagnosis sets out what the signs mean.
How far these observations agree between trained eyes has been measured. Thirty practitioners rating the same 10 tongue slides hit the 80% agreement mark on 17.3% and 19.1% of occasions across two sessions. Agreement fell to 5% where the response choices grew more complex. A later study using a formal classification scheme found agreement moderate for tongue coating and fair for tongue body color. Both traced the gap to loose definitions, and agreement rose once practitioners used an explicit scheme. Pulse palpation is not a self-measure at all: a review of 12 studies found acceptable agreement only where the method carried concrete operational definitions. Inter-rater studies measure agreement on named signs; they do not score a practitioner's whole-picture skill.
The Neijing's account of nourishing life, roughly 2,000 years old, leans on regulating sleep, food, work, emotion and the seasons. It does not ask you to inspect yourself, and it warns that constant self-examination disturbs the Shen. That a practice is very old does not by itself establish a mechanism.
Common Questions
What is the single best health test I can do at home?
Walking speed at your usual pace over 13 feet (4 meters). Across 34,485 older adults it predicted survival about as well as a full risk model built from the usual clinical factors. It takes seconds and needs only a stopwatch and a floor.
If I practice these tests, will my health improve?
Your score will improve; whether the risk improves depends on the test. Resistance training lifted gait speed 0.08 m/s and improved chair rising, and gains in walking speed over a year predicted better eight-year survival. For grip and the single-leg stances, no one has shown that a higher score lowers the risk it measures.
Why is my blood pressure different at the doctor's office?
Partly the setting, partly the technique. An unsupported arm adds about 6.5 mmHg, a hand in the lap about 3.9, and a cuff too small for the arm as much as 19.5. In 2,081 Finnish adults, once home and office readings shared one model, only the home number kept predicting cardiovascular events.
Are smartwatch sleep scores accurate?
For how long you slept, reasonably well. For sleep stages, no. Against polysomnography in 34 adults, the devices flagged sleep almost always but missed wake often, with specificity as low as 0.18. All of them faltered further on nights of interrupted sleep. Day-to-day changes in total sleep time and bedtime are the parts worth reading.
Should I get a full panel of blood tests to check everything?
Testing more finds more and changes nothing. Across 17 randomized trials and 251,891 participants, general health checks did not lower deaths from any cause, on high-certainty evidence. A specific test for a specific reason is different, and that conversation belongs with a doctor who knows your history.
Using the numbers
Repeat the physical tests about every three months and take your waist about once a season. Run the week of blood pressure about once a year, unless a prescriber wants it more often. The average of that week's 28 readings beats a single clinic one.
