A diferencia de la mayoría de esos predictores, responde con fuerza al entrenamiento. Un breve cuestionario, una caminata o carrera cronometrada, o un dispositivo vestible estiman tu cifra con bastante aproximación. Eso basta para compararte con otras personas de tu edad y sexo, y para hacer seguimiento de tu propio progreso a medida que entrenas.

What It Is

VO2 max is the most oxygen your body can take in, carry and use per minute at full effort. It is scaled to bodyweight in milliliters per kilogram per minute (mL/kg/min). It sums the whole oxygen chain at once: lungs loading oxygen into the blood, heart pumping it out, blood carrying it, muscles pulling it out to burn. In healthy people the ceiling is set mostly by how much blood the heart can move per beat at maximum, its stroke volume. Endurance training enlarges that stroke volume, so it raises the number.

The reference measurement is a graded test to exhaustion on a treadmill or bike while a machine analyzes each breath for oxygen uptake. That laboratory test, needing a mask and a technician, is what every method below is measured against.

How To Estimate It

Every method here trades some accuracy for skipping the laboratory. The five options run from a desk questionnaire to an all-out run, each validated against that lab standard.

The non-exercise questionnaire. The lowest-effort estimate uses no exercise at all. A validated equation takes your sex, age, body size and a rating of your usual activity, and returns a VO2 max number. In validation it reached a multiple correlation near 0.81 with a standard error around 5 mL/kg/min, and it beat the older submaximal step-test prediction. That error is wide, so the equation gives only a rough placement and can track change over time; it is not precise enough for an exact figure. The accuracy of the activity rating you give sets the quality of the answer, and several free online calculators run it.

The Cooper 12-minute run, or the 1.5-mile run. Two classic field tests for people who can run hard safely. In the Cooper test you cover as much distance as you can in 12 minutes; the other times a 1.5-mile effort. Both convert to a VO2 max estimate through a standard formula, because how far you can keep going over that time depends mostly on your aerobic capacity. They are among the more accurate field options for a fit person, and the most demanding of the menu. The estimate depends on a near-maximal effort, and that effort is the risky part if you are older, out of shape, or have a heart condition.

The Rockport one-mile walk. The best option for people who should not run flat out. You walk one mile as briskly as you comfortably can, then feed your time, your finishing heart rate, your age, sex and weight into the Rockport equation. Validated against a treadmill test in adults aged 30 to 69, it estimated VO2 max with a cross-validation correlation around 0.92, strong for a field test. Your finishing heart rate is an input, so anything that shifts it adds error.

The Astrand submaximal step or cycle test. An older approach that estimates VO2 max from your heart rate response to a fixed submaximal workload, on a step or a cycle. It is gentler than a maximal run, and the least accurate method here. The same study gave the non-exercise equation an error near 5 mL/kg/min. It found the Astrand test's error was wider, from about 5.5 up to 9.7. It works for tracking yourself under identical conditions; for a one-off absolute number it is the weakest choice.

Wearables. A watch estimates VO2 max from the relationship between your heart rate and your pace or power during ordinary activity. A research model combining wearable signals with basic biomarkers tracked lab-measured fitness with a correlation around 0.82, and detected change over as long as seven years. Consumer watches differ from that research model and vary by brand and algorithm. The estimate rests on the heart-rate-to-effort relationship, and anything that shifts that relationship adds error. Use a wearable for the direction of change over months; a single day's figure carries too much noise to trust.

What The Number Means

Read your estimate as a percentile for your age and sex. The large mortality studies matched people to others of the same age and sex, then ranked them, because a number excellent at 70 can be ordinary at 30. As a rough orientation, young men often sit in the 35 to 50 range and young women in the 30 to 45 range, both drifting down across the decades. Where you fall against your own age and sex band matters more than the raw value. Published norm tables vary by source and testing method, so use one consistently and watch your own movement within it.

The number matters because of what it predicts about survival.

Across very large populations, higher fitness lines up with lower death from any cause, fewer cardiovascular events, less type 2 diabetes, and less of several cancers.

In one long women's cohort it also tracked less dementia. The relationship runs the full length of the range, with no clear point where more stops helping. VO2 max also declines with age on its own, faster the older you get, so the number is a training target as much as a readout.

How To Improve It

VO2 max responds to two kinds of work, and both belong in a week. The base is easy aerobic movement you can talk through, and it grows the capillaries, mitochondria and recovery capacity the hard efforts draw on. On top sits a small amount of hard work near your limit, which raises the ceiling. Build the base first, especially if you are older, deconditioned or returning from illness.

The main hard session is the 4x4 interval:

  1. Warm up.
  2. Work four minutes at a hard effort, around 90 to 95% of your maximum heart rate.
  3. Recover about three minutes easy.
  4. Repeat for four bouts, then cool down.

One to two of these a week, laid on your easy aerobic days with a day between hard sessions, is the dose the research used. More is not automatically better, since the efforts only pay off if you are recovered enough to hit them. Clear a hard maximal test with a clinician if you are higher-risk

A near-maximal effort pushes the heart to its ceiling, so clear it first if you have known heart disease, an uncontrolled arrhythmia, or uncontrolled high blood pressure. Clear it too for warning symptoms: chest pain or tightness on exertion, unusual breathlessness, palpitations, or fainting when you exert yourself. None of these is an automatic no. Choose the walk test or a wearable estimate instead, and get assessed before testing your limit.

Stop the test for warning symptoms

Whichever method you use, stop straight away for chest pain or pressure, sudden or unusual breathlessness, dizziness or faintness, or a fast or irregular heartbeat, and seek medical advice. A fitness estimate is not worth pushing through a symptom to get.

Estimates are approximate

Every field and wearable method here carries several points of spread, from around 5 mL/kg/min for the better ones to wider for the submaximal step test. Read your result as a band a few points wide, keep the method and conditions consistent, and trust the trend over any single reading. A heart rate altered by beta blockers, caffeine or an irregular rhythm distorts the walk-test and wearable estimates in particular.

Start slow, be smart, read the research, and consult a professional if you have any concerns. This is here to inform your choice, not make it for you.

Common Questions

Can I estimate my VO2 max without a lab?

Yes, closely enough to be useful. A non-exercise questionnaire from your age, sex, body size and activity lands within a few points of a laboratory result for most people. A timed run or the Rockport one-mile walk does a little better, and a wearable tracks your fitness over years. None matches a graded lab test with a breathing mask, the gold standard. Each can still rank you within your age and sex group and show whether your training is working.

Which estimation method should I use?

Match it to your fitness and your health. If you can run hard safely, the Cooper 12-minute or a 1.5-mile time trial is one of the most accurate field tests. If you should not run all-out, the Rockport one-mile walk is the best choice and nearly as accurate. For the lowest effort, the non-exercise questionnaire gives a rough starting point. A wearable will track your trend month to month. Whatever you pick, run it under the same conditions each time so your comparisons stay fair.

How much does VO2 max predict?

A great deal, consistently. In 122,007 adults the least fit had about five times the death rate of the fittest. That gap is larger than heart disease, smoking or diabetes, with no ceiling where more fitness stopped helping. Pooled cohorts link each small step up in fitness to roughly 15% fewer cardiovascular events and 8% lower risk of type 2 diabetes. A million-man cohort tied higher youth fitness to less of several cancers. These come from watching large groups over time; no trial raised people's fitness and then measured the payoff. They are powerful associations with a plausible mechanism, and the training response itself is trial-tested.

How fast can I raise it, and does age stop me?

Meaningfully, and age does not stop you. Across 28 controlled trials in adults, aerobic training raised VO2 max by roughly 5 mL/kg/min against controls, with the biggest gains in those who started least fit. Older adults raise it too, from a lower base and more slowly. Since the number falls faster with each decade left alone, training in later life is partly about holding on to the capacity you have. Retest with the same method every couple of months to see the change.
