What It Is

Melatonin is a hormone the body makes on its own, released into the blood at night and cleared before morning. As a supplement it adds to that same night signal, and the tablets sold hold far more than the body produces across a whole night. In the United States and Canada it is sold over the counter. In the United Kingdom, the European Union, Australia, and Japan it is a prescription medicine, available only through a pharmacy or a doctor.

What It Does

Melatonin helps most when the body clock is set to the wrong time. For jet lag, it advances a rhythm still running on the departure time zone. For a delayed sleep phase, where the clock runs several hours behind, a small evening dose moves the body's own rhythm about 1.2 hours earlier.

The less the problem is about timing, the weaker the effect. Shift work ranks lower, on weaker evidence, with about 24 more minutes of daytime sleep. Ordinary insomnia ranks at the bottom, shaving only minutes off the time to fall asleep.

A large government-commissioned review found melatonin does little for most sleep disorders once a delayed clock is set aside.

How It Works

The master clock in the brain reads light through the eyes. Bright evening light delays the clock; dim light, as in evening light and screens, lets it advance. In darkness the clock signals the pineal gland to release melatonin. That melatonin then feeds back to the clock and out to the body.

Anatomy of the Practice

When light fades, a small gland in the brain releases melatonin into the blood. Levels climb through the evening, peak in the middle of the night, and fall before morning. That nightly rise and fall marks biological night for every organ. It is why melatonin is called a chronobiotic, a substance that moves the body clock.

2When you take a dose

A supplement adds to that signal. Taken in the evening it advances the clock. Taken in the morning it signals darkness at the wrong hour and delays the clock. The same dose shifts the clock in opposite directions depending on the hour, so the timing matters more than the number of milligrams.

3Why a larger dose does not help

The receptors saturate at low levels, so a physiologic dose near what the body makes is enough to shift the clock. A larger dose does not move the clock any further. It mainly keeps melatonin elevated into the next day, which is the source of next-morning grogginess.

What Changed

Most people meet melatonin as a natural sleeping pill: take a big tablet to fall asleep, take more if it does not work. But a sedative and melatonin work in completely different ways. A sedative dampens the nervous system, so sleep comes no matter what time the body clock is set to. Melatonin instead resets the clock, so sleep comes only once the clock has shifted. The evidence follows. A physiologic dose of 0.3 to 0.5 mg shifted the clock and improved sleep as much as the 3 to 10 mg tablets in the trials.

A consistent wake time, a dim evening, and for long-term insomnia the structured program called CBT-I all outdo melatonin for ordinary sleeplessness. Work through the sleep environment and evening light first. Melatonin works best added on top of those, once they are in place.

Children: a decision to make with a clinician

Melatonin is widely given to children, but the labeled dose is unreliable. Flavored gummies are easy for a child to overeat, and accidental ingestions have risen sharply as gummies have spread. The long-term effect on a developing child, including puberty timing, is not well studied. Behavioral approaches come first for children's sleep. If melatonin is being considered, particularly for a child with autism or ADHD where the evidence is stronger, make the decision with the child's clinician. Store it out of reach.

Pregnancy and breastfeeding

There is little safety data on supplemental melatonin during pregnancy or breastfeeding, and doses far above the body's own output have not been well studied here. Talk it through with your obstetrician or midwife before starting.

Medication interactions to check

Melatonin can add to blood thinners such as warfarin and to antiplatelet drugs. It is also active in the immune system, so use caution with immunosuppressants. It may also interact with sedatives, blood-pressure and diabetes medicines, and drugs that affect the liver enzyme that clears it. If you take regular medication, check with a pharmacist or clinician before adding it.

Next-day grogginess and driving

A dose that is too large or taken too late keeps melatonin circulating into the morning. That can leave you groggy and slow the next day. A smaller, earlier dose reduces it. Until you know how it affects you, be cautious about driving or operating machinery the next morning.

Common Questions

Does melatonin actually help you sleep?

For ordinary sleeplessness, only a little. Pooled trials show people fall asleep about 7 minutes faster and sleep a little longer, a small effect. How much should I take, and when?

A low dose, about 0.5 to 1 mg, is enough. For jet lag, take it close to your destination's bedtime. For a body clock that runs late, a small early-evening dose moves sleep earlier.

Is melatonin habit-forming?

It is not thought to be physically addictive the way sleeping pills can be, though some people find the nightly habit hard to drop. Other reported effects (headache, dizziness, and vivid dreams) are usually mild.
