What It Is

Tretinoin is all-trans-retinoic acid, the active form of vitamin A, applied to the skin as a cream, gel, or lotion. In the United States it needs a prescription, sold as Retin-A and as generic tretinoin, and it is the reference retinoid every other one is measured against. Milder retinoids (adapalene, and the retinol and retinaldehyde in over-the-counter creams) convert to the same acid in the skin, only more slowly. Two uses carry the strongest evidence. The first is photoaging: the wrinkles, coarse texture, and blotchy pigment that build up after years of sun exposure. The second is acne, where the 2024 American Academy of Dermatology guideline ranks topical retinoids first-line. One cream treats both because it changes how skin cells mature and how the dermis underneath is rebuilt.

What It Does

In a 16-week trial, dermatologists applied 0.1% tretinoin to one side of a face and a matching placebo cream to the other, then graded both blind. On 14 of 15 faces the tretinoin side improved in fine wrinkling, roughness, and mottled color; not one placebo side did.

Longer trials followed. Tretinoin emollient cream at 0.05% and 0.01%, used for 48 weeks, kept improving photoaged skin across the year as the early side effects settled. The gains stay shallow: the cream softens fine lines, texture, and pigment, but does little for deep folds or sagging. The damage it repairs is also largely preventable: daily sun protection is the free lever that keeps that damage from forming, so the cream is repair after prevention. Copper peptides (GHK-Cu) are a gentler cream sold for the same sun-aged skin; their human evidence is far thinner than tretinoin's.

For acne, a network meta-analysis pooled 179 randomized trials. A topical retinoid paired with benzoyl peroxide cleared about 26% more lesions than a placebo cream in mild-to-moderate acne. Retinoids act on the pore plug itself, so they clear inflamed spots and the small bumps that come before them. Many people keep using one at a low frequency to hold acne off after it clears.

How It Works

Tretinoin binds receptors inside skin cells, switching on a different set of genes. In the epidermis, the surface layer, it speeds the turnover of skin cells and loosens the glue between dead cells that plugs pores; that is the acne effect. Deeper down it acts on collagen, the protein that gives skin its firmness, and sun degrades collagen. On sun-exposed forearm skin, collagen formation ran 56% lower than in sun-protected skin from the same people, and the drop tracked how sun-damaged the skin looked.

Tretinoin reverses part of that loss. After 10 to 12 months of nightly 0.1% tretinoin, collagen formation rose about 80%, while the placebo cream showed a 14% fall.

Tretinoin works from underneath. Over about a year it prompts the living cells to rebuild collagen the sun has degraded, so the visible change is slow and it reverses when you stop.

How to Use It

In a 48-week head-to-head, 0.1% and 0.025% tretinoin improved photoaging by the same amount, while the stronger cream caused significantly more redness and scaling. The benefit comes from the drug itself, so the sensible move is to start at a low strength and build up only as your skin allows.

Ways to Do It

A clinician matches the strength to your skin, then almost everything that goes wrong comes from using too much too soon. The dose that works is small and consistent, so aim for a schedule your skin tolerates each week; more than that only adds irritation. Start at the lowest strength and least-frequent schedule, and increase only once your skin is calm.

A clinician can start you at a lower strength such as 0.025% tretinoin. Use a pea-sized amount for the whole face, at night, every other night to begin, on dry skin. Build up to nightly only once that is comfortable, for most people a few weeks to a couple of months.

Apply a plain moisturizer before or after the dose to soften the early redness and peeling. Expect a stretch where skin looks worse and acne can flare before it improves; this purge settles on its own. Keeping the frequency low and the moisturizer generous is how most people stay on it long enough to see the benefit.

Wear sunscreen and give it months$Easy

Use a broad-spectrum sunscreen of SPF 30 or higher every morning. Sun is what causes the damage tretinoin treats, and fresh skin can be more sun-sensitive early on. Daylight also breaks tretinoin down, so it is used at night. Judge it at three to six months, not three weeks; the collagen change behind the smoother texture takes months to show.

The Chinese Medicine View

Chinese medicine has no equivalent to tretinoin. The tradition treats skin from the inside, reading dullness, dryness, and slow healing as signs of Blood deficiency or a weak Spleen. It works through diet, herbs, and lifestyle.

Stop tretinoin before conception and throughout pregnancy. Oral retinoids are among the strongest known causes of birth defects, which is why the caution holds for the cream.

Do not use in pregnancy, or while trying to conceive

Stop tretinoin before conception and throughout pregnancy. A meta-analysis of women exposed in the first trimester found no significant rise in birth defects: reassuring after an accidental exposure. The numbers are small and cannot rule out added risk. Oral retinoids are among the strongest known causes of birth defects, which is why the caution holds for the cream. Ask your clinician about breastfeeding.

Expect irritation in the first weeks

Redness, dryness, stinging, and peeling are common early on. If irritation is severe, cracking, or not settling, cut back the frequency or speak to your clinician.

Protect against cold, dry weather

Wind, cold, and low humidity all deepen the dryness, so winter often calls for less frequent use and more moisturizer.

Watch what you combine it with

Layering tretinoin with strong exfoliating acids, benzoyl peroxide in the same application, or physical scrubs multiplies the irritation. Benzoyl peroxide pairs well for acne but is usually best used at a different time of day. Waxing over treated skin can tear it. Tell your clinician about every skincare product and any other retinoid you use.

Common Questions

How long before I see anything, and does it get worse first?

For acne, the first clear improvement usually lands around 8 to 12 weeks, often after a few weeks when spots look worse while the pores empty. Photoaging is slower, with texture and tone shifting over the months that follow.

Can I use tretinoin if I am pregnant or breastfeeding?

No. Avoid it while trying to conceive and through pregnancy, and ask your clinician about breastfeeding.

Do I really need sunscreen with it?

Yes. A formal light test of a tretinoin gel found it had low potential to make skin burn, so the cream itself does not raise your burn risk in daylight. Wear a broad-spectrum sunscreen each morning regardless.
