How They Compare
Living Longer
In a cohort of more than 55,000 adults, runners had about a 30% lower rate of death from any cause and roughly three years of added life expectancy. The benefit still showed up even in people running under 50 minutes a week at an easy pace. Walking reaches a comparable benefit, but at its gentler pace it takes more weekly minutes to get there.
These are observational cohorts of self-selected runners and walkers, so they show an association and cannot prove cause. Runners were leaner and carried fewer risk factors at the start. That shrinks the problem, but it does not rule out that healthier people simply chose to run in the first place.
Fitness
Running usually wins at raising your top-end fitness, because it pushes you harder than walking on flat ground ever does. Training trials show that hard, brief effort raises VO2max as much as slow, easy work does, and in less time. One meta-analysis of 723 people put the gain from aerobic training at about 4.9 mL/kg/min against non-exercising controls. A brisk uphill walk, or a weighted pack, closes much of that gap.
Fitness itself predicts how long you live. In more than 122,000 people given treadmill tests, the least fit had roughly five times the death rate of the fittest. The benefit kept rising across the whole fitness range, with no threshold where it flattened.
Weight
Over a 6.2-year follow-up, running was linked to greater drops in body weight than walking, and the difference was widest in the people who started heaviest. Running burns roughly twice the calories per minute, so it reaches a weight-moving dose in the time most people have. These are observational cohorts. No one randomized people to run or walk, so leaner and fitter people being more able to run inflates the gap.
Joints and Injury
In a large knee-osteoarthritis cohort, people with a history of recreational running had no more, and a little less, symptomatic knee osteoarthritis than non-runners. Running does not drive the disease.
A single season of running produces lower-limb overuse injury in a large share of runners, driven by repeated impact and by ramping up too quickly. Walking has no airborne phase, so it lands softer and injures far fewer people.
Accessibility and Sustainability
Walking needs nothing but a door, no equipment, no skill, no recovery. It slots into a full day: a phone call taken on foot, an errand walked, ten minutes after a meal. That low friction is why walking is the habit people actually keep. Running needs more of you in fitness, footwear, and recovery time, though at recreational mileage you can start in shoes you already own.

Both walking and jogging meaningfully ease depressive symptoms, and the research groups them together because they work about equally well. For mood, pick the one you will actually keep doing.
The network meta-analysis pooled hundreds of randomized trials of exercise for depression and ranked modalities against controls and against usual care. Walking or jogging showed a moderate reduction in depressive symptoms, alongside strength training, yoga and other aerobic work, with effects larger at higher intensities but present across the range. That the analysis modeled walking and jogging as one node reflects the absence of a meaningful gap between them for mood.

Over an average 15 years of follow-up, running was associated with 30% lower all-cause and 45% lower cardiovascular mortality, and about 3 years of added life expectancy. Strikingly, participants running less than 51 minutes a week, fewer than 6 miles, slower than 6 miles per hour, or only once or twice a week still had lower mortality than non-runners, and running more did not multiply the benefit proportionally. Cardiorespiratory fitness is one of the strongest predictors of a long life, and both walking and running raise it. Running raises it faster per minute, so its time-efficiency is doing something that matters, not just saving clock time.
In a large cohort undergoing exercise treadmill testing, mortality fell steadily with higher measured fitness, and the least-fit group had about five times the risk of the elite-fit group, with benefit continuing to the top of the scale. Fitness is the outcome both activities move, and the faster a mode raises it the more time-efficient it is for survival, which is the through-line from running burning more per minute to running paying off in less time. It is the fitness itself that was measured here, not the training that built it.

When To Choose Which
Lean toward walking if your joints are cranky, you are coming back from an injury, or you are carrying extra weight. It needs no recovery, so you can repeat it seven days a week without a rest day.
Lean toward running if your time is short and you want the fastest fitness gain. Running packs more work into each minute, so a short session yields a larger fitness gain and calorie burn.
Do both, and many people should. An everyday walking base, with some running layered on top, is a common and sensible pattern. If you can only keep one, keep the one you will actually do.

The Chinese Medicine View
Chinese medicine sizes movement to the person and the season. It matches movement to what the body needs on a given day, and both walking and running have a place. Both move Qi and Blood, but the classic texts place them at opposite ends of one scale, from gentle to vigorous.
Walking is the plainest member of the 導引 (daoyin) family of gentle, guiding movement. It sits close to the Su Wen's instruction for spring: to walk with long strides in the courtyard, unhurried. Running is vigorous Yang exertion: warming, dispersing, and a bigger drain on your reserves.
The governing measure is a single line:
形勞而不倦: the body should labor without becoming exhausted.
Someone who runs cold, tires easily, and grows short of breath on speaking shows the picture of Qi or Yang deficiency. They do better with walking, or with easy movement that warms the body without depleting it. The Neijing holds that over-exertion consumes Qi. Someone robust, warm, and restless, with energy to spare, can take the warming exertion of running.
The season matters too. The classics pair spring and summer with outward, active movement, and autumn and winter with quieter, inward, conserving effort.

Build up gradually, whichever you pick
Most running injuries and a good share of walking overuse come from adding too much too fast. Increase weekly time or distance in small steps; hold each step until it stops feeling like a change. Give a new activity a few weeks before lengthening the sessions. Treat persistent pain as a signal to ease off and let the tissue recover.
Footwear and recovery matter more for running
Running's impact loads the feet, shins, knees, and hips. Well-fitted shoes, a gradual mileage build, and rest days between hard efforts do most of the work of staying uninjured. If you are new to impact or returning from injury, more walking volume with running added slowly is the gentler on-ramp.
Get cleared first if vigorous exercise is a question for you
For almost everyone, walking needs no medical clearance. Running raises the risk, so a few conditions warrant a doctor's sign-off first. See a doctor before starting vigorous exercise if you have known heart disease, an uncontrolled arrhythmia, unstable angina, a recent heart attack, or uncontrolled blood pressure. Treat chest pressure, unusual breathlessness, or near-fainting on exertion as a reason to stop and be assessed.

Is walking enough, or do I need to run?
Walking is enough for most goals, a longer life, heart health, keeping weight in check. For a sedentary person, the gap between doing nothing and walking dwarfs the gap between walking and running. Running mainly adds a faster fitness gain.
Should I do both?
Often, yes. A workable split is to walk most days and add two or three easy runs a week, keeping at least one impact-free rest day.
