L'endométriose, c'est un tissu semblable à la muqueuse de l'utérus qui se développe en dehors de celui-ci — sur les ovaires, la paroi pelvienne, et parfois plus loin. Il dépend des œstrogènes et s'enflamme au rythme du cycle menstruel. La douleur est réelle, et elle est si souvent minimisée que la femme moyenne attend des années avant un diagnostic. Elle répond au traitement.

Le moyen le mieux étayé d'apaiser la douleur est de mettre au repos les œstrogènes qui alimentent les lésions, avec le progestatif diénogest, la pilule combinée, ou les médicaments agonistes de la GnRH. Pour une maladie confirmée qui continue de faire souffrir, la chirurgie par laparoscopie visant à retirer ou détruire les lésions soulage à la fois la douleur et augmente les chances de grossesse. Sous tout cela repose ce que vous maîtrisez vous-même : une alimentation anti-inflammatoire, une activité physique régulière, la physiothérapie du plancher pelvien, et le suivi de votre propre schéma. Si la grossesse est l'objectif, le plan change, car les traitements qui apaisent la douleur empêchent aussi la conception ; la fertilité est donc prise en charge comme une question à part entière, tôt.

What It Is

Endometriosis is tissue like the uterine lining growing outside the uterus, on the ovaries, the pelvic wall, the uterine ligaments, and sometimes the bowel or bladder. It is an estrogen-dependent, inflammatory disease: the deposits inflame and bleed roughly in step with the cycle. That is why the pain tends to track the period, and why treatments that quiet estrogen help.

About one in ten women of reproductive age is affected. The most common form is pain: painful periods, pain deep during sex, and pain that spreads through the month. It can also make conceiving harder, and it can form cysts on the ovary.

Its pain is real, and calling it ordinary period pain is a large part of why diagnosis lags first symptoms by several years. Endometriosis is often missed on an ultrasound scan and confirmed only when a surgeon looks inside at laparoscopy, so a normal scan does not rule it out.

Pain that stops you working, studying, or living your day is a reason to ask for proper assessment.

Endometriosis is really three problems under one name, and the treatment layered on top differs for each:

  • Controlling pain, the day-to-day and cyclical pain the disease causes.
  • Protecting or pursuing fertility, where conceiving is the goal.
  • Dealing with an ovarian cyst, an endometrioma, a pocket of old blood on the ovary.

What Helps

Endometriosis responds to treatment. Its three problems (pain, fertility, and an ovarian cyst) each draw on a different mix of treatments.

Quieting the estrogen signal, the first-line medical treatment

Because the pain tracks the estrogen behind the deposits, the best-evidenced medical treatments quiet that signal, and current guidelines make them first-line for pain. All of them prevent pregnancy while taken, so the plan changes when conceiving is the goal.

  • Dienogest, a 2 mg progestin taken as a daily tablet, lowered pelvic pain by 12.3 mm more than a placebo on a 100 mm scale over 12 weeks. Against a stronger GnRH agonist injection, it matched the pain relief over 24 weeks without the bone loss that drug causes. Spine density rose 0.25% on dienogest and fell about 4% on the GnRH drug. Kept up for about a year, its pain relief deepened and lasted for months after stopping.
  • The combined pill, often taken continuously to skip the bleed, is where many women start. Its trial evidence in endometriosis is thin and low-certainty, so that first-line use rests on long clinical experience and convenience more than strong data.
  • The GnRH drugs give the strongest suppression, usually paired with a little add-back hormone to limit the bone loss and hot flashes they otherwise cause.

Everyday pain relief

Anti-inflammatory painkillers, the NSAIDs such as naproxen or ibuprofen, are the common first step for day-to-day pain, taken around the period when cramping peaks. The trial evidence in endometriosis specifically is very thin: the one small trial that could be pooled, in 24 women, could not show a clear effect either way. They stay a reasonable first thing to try, and carry the usual stomach and kidney effects with regular use.

Surgery when the pain persists

Across 10 randomized trials of 973 women with mild to moderate disease, keyhole surgery to remove the deposits beat a look-only operation. It reduced overall pain and raised the chance of live birth or ongoing pregnancy. For disease that keeps hurting after hormonal treatment, excision or ablation is a well-evidenced, mainstream option. Excision (cutting deposits out) and ablation (burning them off) performed similarly for pain in the one head-to-head study.

The same laparoscopy that treats the deposits also confirms the diagnosis, so for many women a single operation does both. Pain can return over time, so surgery is planned around the goal, whether pain relief or fertility.

If you are trying to conceive

The hormonal treatments that ease pain also block conception, so they are paused when conceiving is the goal, and fertility becomes its own job. Laparoscopic surgery to remove deposits improved pregnancy rates in the same trials that showed it eased pain, so it can raise the odds in selected cases.

Assisted reproduction, including IVF, is often the faster route, especially where the disease is more advanced or where time matters. The fixable problems endometriosis causes, blocked or scarred fallopian tubes and disrupted ovulation, respond better the sooner they are treated. An early fertility review, before years of trying, helps most.

The base you steer yourself

These measures do not dissolve deposits already present. They sit beneath the medical and surgical treatments, lowering the odds of the disease over time and making it easier to live with.

Anti-inflammatory eating is the food-first step. In large cohorts followed for over a decade, women eating the most long-chain omega-3 fat from oily fish were 22% less likely to be diagnosed with endometriosis. Those eating the most trans fat, the industrially produced fat in ultra-processed food, were 48% more likely. More dairy and higher vitamin D tracked with lower risk too. These figures are about the chance of developing endometriosis over a decade of follow-up. Health-conscious eaters differ in many other ways, so some of the link may come from those habits. A Mediterranean pattern rich in oily fish, dairy, and whole foods is still a reasonable thing to adopt.

Steady movement you can sustain helps inflammation, mood, and how much the pain takes from your day. Pelvic-floor and pain-focused physiotherapy addresses the tight, guarding muscles and the heightened pain signaling layered on top of the disease. Few trials have tested it in endometriosis specifically, so it counts as sensible pain care that fits alongside the medical treatment.

Heat on the lower abdomen, protected sleep, and tending stress and mood all change how much the pain costs you day to day. Tracking your own cycle and pain lets you and your clinician see the pattern and judge whether a treatment is working.

Across 10 small trials, the six that measured pain found acupuncture eased endometriosis pain by 1.36 points on a 0 to 10 scale. Only one of those trials used a placebo needle, so the result is an early signal that still needs confirming. Acupuncture also lowered CA-125, a blood marker of the disease. In Chinese medicine it targets the wider pattern and does not act on the deposits themselves.

For an ovarian endometrioma, N-acetylcysteine was linked in one observational study to far fewer women going ahead with planned surgery, a signal that needs a randomized trial to confirm.

Pain on a visual analogue scale improved in the vitamin D arm (7.0 to 5.5), but the placebo arm improved by a nearly identical amount (6.0 to 4.4), and neither supplement arm differed significantly from placebo. Measured in: 69 women aged 12 to 25 with surgically confirmed endometriosis and pelvic pain, randomized to vitamin D3, fish oil or placebo for 6 months. The trial was small and the large placebo improvement makes any true supplement effect hard to detect, so this argues against relying on these capsules as a standalone pain treatment, not settling their value for prevention.

Only one small pilot used a placebo needle and assessed blinding; sample sizes ranged from 8 to 36 per arm. Measured in: Ten trials of acupuncture for endometriosis-related pain, with 8 to 36 patients per arm and mostly non-placebo controls. Almost none of the trials could blind participants and all were small, so the pooled effect is an encouraging early signal for pain, not a settled result, and acupuncture is used for the wider pattern, not to clear the deposits.

A Cochrane review found two Chinese randomized trials (158 women). After laparoscopic surgery, Chinese herbal medicine gave symptomatic relief comparable to the drug gestrinone (RR 1.04, 95% CI 0.91 to 1.18) with fewer reported side effects. Neither trial compared herbal medicine against a placebo. Measured in: Two Chinese randomized trials of 158 women taking Chinese herbal medicine after laparoscopic surgery for endometriosis. With only two small trials and no placebo comparison, this is a preliminary signal; in Chinese medicine the pattern decides the formula, so a mixture that suits one woman can be wrong for another, and products sold direct to the public have a documented history of adulteration.

In detail

In an observational study of women with ovarian endometriomas, 24 of those taking N-acetylcysteine cancelled scheduled laparoscopy because the cyst shrank or disappeared or pain eased, against 1 among untreated women. Eight pregnancies occurred among the N-acetylcysteine group and 6 among the untreated. The authors reported the compound was well tolerated with no side effects. Measured in: Women with ovarian endometriomas choosing N-acetylcysteine compared with untreated women awaiting surgery. What could explain it instead: Women who opted for the supplement may have had smaller or less aggressive cysts, or been more motivated to avoid surgery, either of which could explain the difference apart from the supplement.. The women were not randomly assigned, so those who chose the supplement may have differed from those who did not; the result needs a proper randomized trial before it can be relied on.

Where the evidence runs thin

In a randomized trial of 69 young women with endometriosis, vitamin D and fish oil capsules eased pain. But a dummy capsule eased it by almost exactly as much, so neither beat placebo. Correcting a measured deficiency is still reasonable on its own terms.

The older claim that exercise strongly protects against endometriosis did not hold up. In a cohort of over 100,000 women the most active had at most a slightly lower risk, a rate ratio of 0.89. Pain itself can keep women from exercising, so the direction of cause is unclear.

Curcumin, the active part of turmeric, calmed uterine pain in mice by damping inflammation in the pain-signaling nerves. It has not been tested in women.

Everything to be aware of is here, in one place. This practice suits most healthy people; a few situations call for real care.

The prescribed treatments are monitored

Dienogest, the combined pill, and the GnRH group are prescribed and monitored. All of them suppress ovulation and prevent pregnancy while taken, so they are paused when you are trying to conceive. Do not start or stop a prescribed treatment on your own, that is a decision with your prescriber.

Supplements and products sold for "hormone balance"

Unregulated slimming and "hormone balance" products sold direct to the public have a documented history of adulteration with undeclared drugs, with no clear dose or contents. N-acetylcysteine for an endometrioma rests on a single observational study, with no randomized evidence yet.

Chinese herbs belong with a practitioner

In Chinese medicine the pattern decides the formula, so a mixture that suits one woman can be wrong for the next. Strong blood-moving herbs are avoided in pregnancy and around heavy bleeding. Use a qualified practitioner and a supply you can trace.

Endometriosis is a real, treatable disease. The hormonal treatments and, where needed, surgery work on the estrogen and the deposits. Diet, movement, pelvic-floor work, and tracking your own pattern are yours to manage. Educate yourself, start there, and consult a licensed practitioner about anything that needs weighing.

When to See Someone

Most of living with endometriosis is steady, day-to-day management. These are the signs to seek care about, urgently for the first three:

  • Sudden severe one-sided pelvic pain, with faintness, nausea, or a racing heart: can mean a twisted or bleeding ovarian cyst; needs same-day assessment(seek urgent care)
  • A positive pregnancy test with pelvic pain, shoulder-tip pain, or bleeding, which needs urgent care to rule out an ectopic pregnancy(seek urgent care)
  • Fever, pelvic pain, and unusual discharge together can signal a pelvic infection(seek urgent care)
  • Cyclical bleeding from the bowel or bladder, or pain passing stool or urine that tracks with your period: can point to deep disease; needs specialist review
  • Pain deep during sex that has been dismissed as normal, a reason to ask for a specialist assessment
  • A planned pregnancy not happening after a reasonable time trying, a reason to seek a fertility review early
  • Low mood, anxiety, or distress from living with chronic pain: common with endometriosis and worth proper care

Any change to a prescribed treatment is a decision you make with your prescriber. If pregnancy is your goal, the timing of pausing those treatments is planned the same way.

Common Questions

Do I have to have surgery?

Not necessarily. Hormonal treatment is first-line and helps many women without an operation. When pain persists despite it, or when fertility is the goal, laparoscopic excision or ablation is the next step (Duffy, Cochrane 2014).

Will endometriosis affect my chances of getting pregnant?

Yes, it can. Because the treatments that calm pain also block ovulation, they are paused while you try. A fertility review early, well before years pass, gives the best odds (Duffy, Cochrane 2014).
