What It Is
A strict carnivore day is meat, fish, eggs, and animal fat, and nothing else. Some people add dairy; others drop it. Nothing from a plant is allowed: no vegetables, fruit, grains, legumes, nuts, or seeds, the six plant groups a normal diet is built from. That leaves zero fiber and none of the vitamin C, folate, and other micronutrients plants supply. Most followers eat red meat every day and rely on salt for flavor.
This is the far end of the low-carbohydrate spectrum. A ketogenic diet cuts carbohydrate hard but keeps some plants. Carnivore takes every one of them out. It runs as a very-low-carbohydrate diet and a total elimination of plant food at the same time.
What It Does
The one large description of real users surveyed 2,029 adults who had eaten this way for a median of 14 months. In it, 95% reported better overall health, well-being rose for most, and median body mass index fell from 27.2 to 24.3. Reported side effects were uncommon. The weakness is who answered: everyone chose the diet and stuck with it. Anyone who felt worse and quit was never counted. With no comparison group, the survey cannot separate the diet's effect from the choosers' own habits.
There are no long-term randomized trials of a true carnivore diet.
Every claim about the diet over years rests on self-report, on short trials of related low-carbohydrate diets, and on established nutrition science. The one part with controlled evidence is the elimination itself. In a controlled crossover trial, people with irritable bowel syndrome scored their gut symptoms at 22.8 on a 100 mm scale on a low-FODMAP diet. On a typical diet they scored 44.9. Carnivore strips out far more than the fermentable carbohydrates that trial removed.
Weight falls at first, as on other very-low-carbohydrate diets. Pooling the longest trials of ketogenic against low-fat eating, people on the very-low-carbohydrate side were about 0.91 kg (2 lb) lighter at a year or more. That average is small, and it comes from ketogenic diets. Carnivore itself has never been measured over a year.
The concerns rest on firmer numbers than the benefits do. In controlled trials, low-carbohydrate diets raised LDL cholesterol by about 0.16 mmol/L (6 mg/dL) against low-fat diets, and carnivore carries more saturated fat than those trials used. In the user survey, those who reported labs had a mean LDL cholesterol of 172 mg/dL, a high reading. For cancer, the International Agency for Research on Cancer classifies processed meat as a cause of colorectal cancer, at about 18% higher risk for each 50 g eaten daily.

How It Works
Elimination comes first. Taking out every plant food also removes anything a person reacts to. It removes all ultra-processed food, refined sugar, and alcohol too, since none of those are animal foods. About 57% of the calories Americans eat come from ultra-processed food. Much of what people credit to cutting plants is really cutting engineered food built around fat, sugar, salt, and preservatives. The second change is metabolic: with carbohydrate near zero, the body drops into ketosis, the same fat-burning state a ketogenic diet produces.
Anatomy of the Practice
1The first two weeks
Water weight drops as stored carbohydrate empties. The scale drops fast, but the loss is water, not fat, until glycogen is gone. Some people feel flat, headachy, or cramped in this window, the same adjustment ketogenic dieters describe, and it usually settles. Gut symptoms and joint aches ease for some people here. This is the elimination effect, and the reason many report feeling better.
2The first months
Fat loss continues while total calories stay lower, partly because protein is filling and food choice is narrow. Blood sugar and triglycerides often improve on any very-low-carbohydrate diet. LDL cholesterol commonly rises over the same period, more so given how much saturated fat the diet carries, and how far it rises varies widely between people.
3The long run
This is where measurement stops. No trial has followed a carnivore diet long enough to answer three questions. Do the early gains hold? Does the raised LDL cholesterol become heart disease? And what do years without fiber, vitamin C, or folate do to the body? A shortfall in fiber, vitamin C, or folate takes months to show, not days. That is why the real question is what years of this diet do.

Get a baseline lipid panel before day one. The safest version is the shortest one. If you take blood-sugar or blood-pressure medication, talk to your prescriber first. A very-low-carbohydrate diet can lower both, and your usual dose may become too strong.
Get baseline bloodwork, including a lipid panel, then eat only animal foods for about four weeks and track what changes. This is long enough to notice an effect on gut or joint symptoms and short enough to carry little nutritional risk. Then reintroduce plant foods one group at a time, which is the only way to learn which food, if any, was the problem. Most people find a specific trigger this way and do not need to stay carnivore.
If you continue past a trial, keep as much variety as an all-animal diet allows. Add fish for omega-3 fats, eggs, and organ meats such as liver. Those carry the vitamin C, folate, and micronutrients muscle meat lacks. A diet of only steak is the narrowest, highest-risk form. This does not fix the missing fiber or make the diet studied, but it closes the widest nutrient gaps.
Repeat the lipid panel after two to three months. A large LDL rise is common, and it is the finding most worth acting on. No trial has measured where this much saturated fat leads over years. Take the numbers to a clinician. A person can feel well while LDL climbs, and only the lab number tracks the risk.

Anyone with kidney disease, established heart disease, familial high cholesterol, gout, an eating disorder history, or who is pregnant or breastfeeding should not start this without medical advice. If you take insulin, a sulfonylurea, or blood-pressure medication, a very-low-carbohydrate diet can push your glucose or pressure too low for your current dose. Plan that adjustment with your prescriber before you start.
Watch the LDL cholesterol
This much saturated fat pushes LDL cholesterol up for most people. A large jump is the one change worth catching early, because the long-term risk of this eating pattern has never been measured. If your number climbs, take it to a clinician.
The missing nutrients are real
An all-meat diet has no fiber and little vitamin C or folate, and on its own muscle meat is low in several other micronutrients. Adding liver and other organ meats, plus fish, helps but does not fully close the gap or replace fiber. Constipation is common. The long-term consequences of eating this way for years are unstudied, so the absence of a reported problem in a short survey is not evidence of long-term safety.
It is an elimination diet, so reintroduce
The value of cutting foods out is learning which one mattered. You only get that by adding groups back one at a time and watching. Staying on the strictest version indefinitely trades a short diagnostic diet for a lifelong restriction that no trial has tested. If symptoms return with a specific food, you have your answer and do not need the whole restriction.

Is it the red meat that raises cancer risk?
Processed meat carries the clearer cancer signal; it sits in the top certainty tier, Group 1, for colorectal cancer. Carnivore relies mostly on unprocessed red meat, rated one step lower at Group 2A, a probable rather than confirmed cause. Fresh, unprocessed cuts keep the diet in that lower tier, away from the bacon-and-deli-meat end.
What causes the rough first week, and can I blunt it?
As carbohydrate drops, insulin falls, and the kidneys flush out sodium and water. The lost sodium, potassium, and magnesium drive the early headaches, fatigue, and cramps. Salting food to taste and drinking to thirst covers most of it, and bone broth adds sodium and potassium. It usually eases within 1–2 weeks.
With no fruit or vegetables, is scurvy a risk?
Overt scurvy needs vitamin C to stay under about 10 mg a day for weeks. Fresh meat and organ meats carry small amounts, enough that scurvy is rare on a varied all-animal diet. Muscle meat alone is the thin margin. The read is a low but unconfirmed risk, kept lowest by including liver and eating fresh meat.
