Un lavement au café consiste à retenir dans le rectum et le côlon inférieur du café infusé et refroidi, promu depuis les années 1930 pour la détoxification et une longue liste d'affections. What It Is

A coffee enema is brewed coffee, cooled toward body temperature, then introduced into the rectum and lower colon and held, usually at home. The practice entered alternative medicine in the 1930s through Max Gerson, a physician who built it into the Gerson therapy.

The Gerson therapy pairs raw juices, a very low-sodium high-potassium diet, supplements, and several coffee enemas a day, promoted for advanced cancer and chronic disease.

The practice spread into the wider wellness world, now sold for detoxification, energy, clearer skin, and digestive complaints.

The original protocol calls for several coffee enemas a day. That high frequency is central to how the practice was designed and to the fluid and salt losses it can cause.

Where The Research Stands

A 2020 systematic review in Medicine (Baltimore) searched for any study of self-administered coffee enemas across every indication, detoxification included. It covered Ovid MEDLINE, Ovid Embase, the Cochrane Central Register of Controlled Trials, CINAHL, and several Korean, Chinese, and Japanese databases. No study reported a benefit for any use.

For harm, the picture was different. The review found nine case reports. Seven were recent: four of colitis, and three of rectal burn or perforation. The other two were earlier deaths.

Zero studies showed benefit; nine case reports showed harm.

Case reviews by the National Cancer Institute and the New York County Medical Society found no usefulness for the Gerson diet. In the closest tested regimen, patients on standard chemotherapy lived a median of 14 months, versus 4.3 months for those who chose an enzyme-and-detox treatment. Those patients chose which treatment to take instead of being assigned at random. That choice weakens the comparison.

How to use it

The healthy colon is not a reservoir of poisons that needs flushing. Regular bowel habits are supported by fiber, fluid and movement, not by cleansing.

How The Idea Is Supposed To Work

The practice rests on a nineteenth-century doctrine called autointoxication: the idea that waste in the colon poisons the body and causes disease. It was the ruling theory of its day. Once the physiology was worked out, the doctrine did not hold up. The healthy colon is not a reservoir of poisons that needs flushing.

Gerson's own claim goes one step further. Absorbed coffee reaches the liver and is said to widen the bile ducts, so the liver releases more toxins. Supporters sometimes frame this as raising the liver enzyme glutathione-S-transferase. That pathway has not been measured in people.

The body already runs this cleanup on its own. The liver and kidneys filter and clear waste from the blood twenty-four hours a day. The liver makes bile continuously, and the gallbladder releases it when you eat, most strongly after a meal that contains some fat. At the cellular level, the body clears and recycles its own worn parts, a process called autophagy.

What People Reach For, And What Helps

People reach for four things from a coffee enema. They want a bowel that empties well, a lighter and less bloated feeling, more energy, and the sense of having done the body good.

Three ordinary measures cover most of that.

  • Fiber from vegetables, fruit, legumes, and whole grains gives the stool the bulk and softness that keep the bowel moving.
  • Fluid works together with the fiber.
  • Movement, even a daily walk, stimulates the muscular wave that pushes things along.

For the liver and kidneys, the two organs that do the filtering, the measures with backing are the ordinary basics. That means a diet built on whole foods, enough fiber and fluid, moderate alcohol, regular activity, and adequate sleep.

The Chinese Medicine View

Coffee enemas are a twentieth-century invention with no place in the classical Chinese pharmacopoeia; no historical text assigns them a channel, a temperature, or a flavor.

Classical texts approach the bowels cautiously. Chinese medicine has a recognized method of purging: bitter, cold, downward-draining herbs such as Da Huang move heat and stagnation out through the stool. Retained heat in the intestines, described in patterns like Damp-Heat, is treated as a clinical problem.

The classics emphasize restraint. Purging is a strong method, reserved for a clear excess pattern, used briefly and then stopped. Draining too hard depletes the body's righteous qi and injures the Spleen and Stomach, described in the classics as the root of digestion and the source of ongoing vitality. Health is maintained by protecting and nourishing that digestive center, and repeated flushing works against it.

A forceful flush of the lower bowel several times a day runs against that restraint. It is strong draining, done often, with no check for whether a real excess pattern is present.

Proctocolitis with abdominal pain and rectal bleeding, in four of nine reviewed harm reports

A 2010 report in the American Journal of Gastroenterology documented proctocolitis caused by coffee enemas. A 2008 Korean Journal of Gastroenterology case reported acute colitis after a coffee enema, presenting with severe abdominal pain and hematochezia. Hot coffee has burned the rectal lining, confirmed on endoscopy

A 2010 Endoscopy report described a rectal burn induced by a hot coffee enema, and a 2008 Gastrointestinal Endoscopy report described a rectal burn caused by a hot-water coffee enema, both confirmed endoscopically. A 1980 report in JAMA documented deaths related to coffee enemas. Repeated, large-volume enemas can deplete fluid and shift sodium and potassium out of their safe range, and this risk rises with frequency. A 1984 report in the Western Journal of Medicine described polymicrobial enteric septicemia from coffee enemas in a patient whose liver clearance was severely compromised. A 2019 IDCases report described rapidly developing, disseminated Clostridium septicum sepsis in a patient who used coffee enemas and kombucha, and proposed that repeated enemas disrupted the bowel mucosal barrier and let enteric bacteria translocate and spread through the blood. Not a substitute for care of a serious illness

Coffee enemas have not been shown to treat cancer or any disease. In the closest tested regimen, choosing it over standard treatment was linked to markedly shorter survival. For anything serious, a proper diagnosis and standard care come first.

Some conditions raise the risk sharply

The risk rises sharply with hemorrhoids, inflammatory bowel disease, diverticular disease, or recent bowel or rectal surgery. It also rises with heart or kidney conditions that make fluid and salt shifts dangerous, and in pregnancy. If any of these apply, take extra care and talk it through with a practitioner first.

Relying on enemas to move the bowel

Relying on any enema to move the bowel can, over time, blunt its own reflex to empty. The regularity most people are after is better built from the everyday measures covered above.

Common Questions

Do coffee enemas actually detoxify the body?

No. The claim that caffeine reaches the liver and drives more toxin release has never been measured in a living person. No study has found a detox benefit.

What are the better-supported ways to get the same result?

For regularity and a lighter feeling, the everyday measures already described do the job. Persistent constipation is worth a proper medical assessment, since it occasionally points to something that needs attention.

Why do some people feel great afterward?

Two effects overlap. The physical sensation of an emptied lower bowel feels light and settling. Then the placebo effect adds to it: a deliberate ritual you expect to help produces a measurable response in the body. That response is strong enough to show up across controlled trials of every kind of treatment. Neither effect means the practice is detoxifying anything.

Is a coffee enema dangerous?

It can be. The published record is drawn entirely from case reports:

  • acute colitis and proctocolitis;
  • rectal burns from coffee that went in too hot;
  • in one case, repeated burns scarred the rectum into a stricture that then perforated;
  • disruption of the bowel wall that let a person's own gut bacteria cross into the blood;
  • deaths linked to the fluid and electrolyte shifts that frequent enemas can cause.

These are all case reports: none of them controlled trials. They prove the harms are real and keep happening, but they cannot tell you how common they are.
