Les bénéfices des fibres sont importants à l'échelle de populations entières. Ce qu'une personne en tire précisément dépend de trois choses : le type, la dose, et qui elle est.
- Le type. Les fibres solubles et visqueuses abaissent le cholestérol et la glycémie. Les fibres insolubles et volumineuses ne font ni l'un ni l'autre ; elles agissent sur le transit intestinal. Les fibres fermentescibles nourrissent les bactéries intestinales.
- La dose. L'essentiel du bénéfice apparaît autour de 25 à 30 grammes par jour, et aller bien au-delà ajoute davantage de gaz que de protection.
- La personne. La même fibre aide l'une et gêne l'autre. Le psyllium abaisse l'HbA1c, la moyenne sur trois mois de la glycémie, dans le diabète de type 2, tandis qu'une fibre fermentescible qui nourrit un intestin sain peut déclencher des ballonnements dans le syndrome de l'intestin irritable.
Le bénéfice le plus connu des fibres est la protection contre le cancer colorectal, sur la base de preuves observationnelles mais exceptionnellement cohérentes. En regroupant 25 études prospectives, chaque tranche supplémentaire de 10 grammes de fibres totales par jour était associée à un risque de cancer colorectal environ 10 % plus faible. Ces 10 grammes correspondent à environ une portion supplémentaire de haricots ou un bol de céréales au son. Le signal le plus net provenait des fibres céréalières et des céréales complètes. Les études concordaient étroitement, ce qui renforce la tendance.
Cela dit, les personnes qui consomment le plus de fibres mangent aussi moins de viande transformée, bougent davantage et fument moins. Une partie du bénéfice provient donc du mode de vie plus sain qui entoure les fibres, et non des fibres seules.

Les trois rôles passent par trois mécanismes distincts, si bien qu'une fibre peut bien remplir l'un et pas du tout l'autre. Un gel abaisse le cholestérol et la glycémie postprandiale. Une fibre qui ne forme pas de gel ne fait ni l'un ni l'autre. Le volume et l'eau font avancer le transit intestinal, une voie physique qui n'a rien à voir avec le sang. La fermentation modifie le microbiome, et produit aussi les gaz pour lesquels les fibres sont connues.

1Soluble and viscous fiber
Dissolves in water into a thick gel. The gel slows glucose absorption, so blood sugar rises less after a meal. It also binds bile acids; the liver then pulls cholesterol from the blood to make more, which lowers LDL. This is the fiber in oats and barley (beta-glucan), psyllium, pectin from fruit, and beans. It is the fiber that moves the numbers on a cholesterol or blood sugar test.
2Insoluble and bulking fiber
Does not dissolve. It holds water, adds bulk and stimulates the colon, so transit speeds up and stool softens. This is the fiber in wheat bran, whole grain skins, and the tougher parts of vegetables. Its job is regularity.
3Fermentable fiber
Gut bacteria ferment it into short-chain fatty acids that feed the cells lining the colon. This is inulin, the fiber in onions, garlic, chicory and legumes, plus resistant starch and much soluble fiber. It feeds the microbiome, and it is the fiber most likely to cause gas when you add it quickly. Oat beta-glucan is soluble and fermentable, psyllium is soluble but barely fermented, and wheat bran is insoluble and barely fermented.

Closing the gap is mostly a food problem. The fiber tied to the lowest disease risk was eaten as whole plants. Whole plants also carry the vitamins, polyphenols and minerals that ride along with the fiber. A supplement delivers one concentrated fiber for a single job, cholesterol or regularity, on top of a decent diet.
Legumes, whole grains, vegetables, fruit, nuts and seeds, working toward the 25-to-30-gram target from a range of them. A varied plate covers soluble, insoluble and fermentable fiber at once, so no single food or supplement replaces it. The population evidence rests on fiber eaten this way, as whole plants.
To move LDL cholesterol you need soluble viscous fiber specifically. A large bowl of oats or barley supplies roughly 3 grams of beta-glucan, and psyllium at about 10 grams a day lowered LDL more. Only that concentrated viscous fiber reliably moves LDL; mixed fiber from a general diet does not.
Psyllium or wheat bran, both taken with plenty of fluid, make stools more frequent and softer. Psyllium is the more versatile since it also lowers cholesterol; wheat bran is the cheaper pure bulking option.
Psyllium husk powder adds a concentrated viscous, low-fermentation fiber for either cholesterol or regularity. A teaspoon stirred into a full glass of water before a meal is a normal starting dose. The active ingredient is the psyllium husk itself, so generic and branded husk powders hold the same fiber.

Add it gradually, and drink enough water
The gas, bloating and cramping people blame on fiber mostly comes from adding a lot at once, especially fermentable fiber. Raise your intake by a few grams every week or two, with enough fluid. Fluid is also how bulking fibers like psyllium and bran do their job. Most tolerance problems settle as the gut adapts over a few weeks.
If you have IBS, choose the fiber carefully
Fermentable fibers, including inulin and some added fibers, can worsen bloating and pain in irritable bowel syndrome. A low-fermentation soluble fiber such as psyllium is usually better tolerated. Introduce it slowly, and cut back if bloating worsens.
Narrowed bowel, obstruction risk and swallowing
Bulking fiber adds volume to the colon. If the bowel is already narrowed (by a stricture, severe diverticular disease, past obstruction, or very slow stomach emptying) extra bulk can make things worse. Get medical advice first. Psyllium powder also swells on contact with liquid, so anyone with trouble swallowing should take it with plenty of water and care, or avoid it.
Keep fiber supplements away from your medicines
The same gel and bulk that make viscous and bulking fiber work can slow the absorption of medicines taken at the same time, including levothyroxine and some others. Separate a fiber supplement from your medications by a couple of hours, and mention regular psyllium use to your pharmacist so timing can be checked against what you take.
If you are treated for diabetes, watch your readings
Fiber that lowers blood sugar can add to the effect of glucose-lowering medication. If you take diabetes medication and add psyllium or a lot more fiber, check your readings over the first couple of weeks. Bring them to your doctor, the medication dose may need lowering.
