What It Is

Iodine is a trace mineral with a single essential job in the body: building thyroid hormone. The thyroid pulls it from the blood and assembles it into T4 and T3. These two hormones set the pace of your metabolism, and everything they govern (body temperature, heart rate, and, before birth, how the brain develops) depends on a steady supply.

The dependable everyday sources are iodized salt, dairy, eggs, and seafood, and cows pass iodine into milk from their feed and from milking-time disinfectants. Soil across many inland regions contains almost none, so before salt was iodized, whole populations far from the sea were deficient. The body needs only about 150 micrograms a day, and even the safe ceiling for adults is modest, near 1,100.

What It Does

Below about 150 micrograms a day the thyroid cannot make enough hormone, so goiter and hypothyroidism follow. In the worst-hit inland areas, children were once born with severe, permanent impairment from deficiency before birth. The remedy was among the cheapest in medicine: a trace of iodine added to table salt. It drove goiter and its worst harms down within a generation.

Why Both Too Little And Too Much Cause Harm

Thyroid trouble comes from a shortfall and from an excess alike. Plotted against intake, the response forms a U. Too little iodine harms at the low end. A wide safe range covers normal intake. New problems appear as intake climbs toward the adult ceiling near 1,100 micrograms a day.

When a deficient population reaches sufficiency, thyroid disorders rise for a few years before settling down. A large iodine load can drive a susceptible gland into too little hormone, or occasionally too much. People most exposed to overload are often those supplementing a thyroid that was already sufficient.

Pregnancy Is Where The Stakes Are Highest

Pregnancy is the one time the low end of the curve turns dangerous fast. A woman's iodine before and during early pregnancy matters more than at any other point in life. Severe maternal deficiency does irreversible harm to the child.

What mild-to-moderate deficiency does is less settled. It is now common across parts of the UK and Europe, and among people who have drifted from dairy and iodized salt. In one UK cohort, children of mildly deficient mothers were more likely to land in the lowest band for verbal IQ and reading. A pooled analysis of three European cohorts found only a weak, curved link with verbal IQ, and it did not reach significance at the usual deficiency threshold.

A randomized trial gave iodine to mildly deficient pregnant women; years later, it found no measurable gain in their children's development. Entering pregnancy already iodine-replete matters, and the deficient end is the one to avoid. Starting a supplement once a woman is already close to sufficient has not been shown to add IQ points.

How It Works

The thyroid concentrates iodine far more than any other tissue. What it does with that iodine has been mapped for more than a century.

Anatomy of the Practice

The thyroid traps iodine from the blood through a pump called the sodium-iodide symporter, then attaches it to a protein scaffold to build thyroid hormone. T4 carries four iodine atoms, T3 carries three. When iodine is scarce the gland enlarges, the goiter of deficiency. When it still cannot make enough hormone, the result is hypothyroidism.

2Before birth and in early life

Thyroid hormone directs how the fetal and infant brain grows and connects. In the first weeks, the fetus relies entirely on the mother's iodine and hormone. A severe shortfall in this window causes permanent brain and nerve damage. That is why the developing brain is the tissue most sensitive to iodine deficiency.

3When intake climbs too high

A large iodine load briefly shuts down hormone production, the Wolff-Chaikoff effect, and most people recover within days. People with autoimmune thyroid disease may not, and can stay hypothyroid. Separately, a heavy iodine load can tip a nodular gland into overactivity, the Jod-Basedow effect. Both effects mean excess iodine produces the same thyroid disease that deficiency does.

This machinery is among the best-established in physiology. The mechanism is the same for everyone; what differs is how much iodine a person takes in and how vulnerable the thyroid is.

Ways to Do It

For most people, the everyday food supply already delivers enough iodine. The reasons to be deliberate are a diet without dairy and seafood, and pregnancy.

The cheapest and most reliable route, and the one that eliminated widespread deficiency. About a quarter teaspoon of iodized table salt supplies roughly a day's iodine. Specialty, gourmet, sea, and pink Himalayan salts are usually NOT iodized, so check the label. Most salt in restaurant and packaged food is not iodized either.

Choose an iodine-fortified plant milk, or a modest supplement or multivitamin providing about 150 micrograms, and read the label, since not all contain it.

Kelp tablets, sea-vegetable powders, and iodine drops such as Lugol's solution deliver many times the daily requirement, and their contents vary wildly batch to batch. For someone already replete they add no benefit, and a large iodine load can tip a vulnerable thyroid underactive or overactive.

The Chinese Medicine View

Chinese medicine long used two marine plants for neck swellings: Kun Bu (kelp) and Hai Zao (sargassum seaweed). Both are classed as salty and cold, and both were used to soften hardness, dissipate nodules, and transform Phlegm.

Their classic indication was goiter and the neck swellings later often understood as the enlarged thyroid of iodine deficiency. These seaweeds happen to be iodine-rich, a coincidence the old physicians could not have known.

The tradition also carried its own restraint. Salty, cold marine herbs were understood to burden the Spleen and Stomach and weaken digestion, most of all in someone already damp and sluggish. So they were dosed sparingly and fitted to the person. Hai Zao sits among the classical incompatibility cautions, kept apart from Gan Cao (licorice).

Autoimmune thyroid disease is the main reason not to high-dose

People with Hashimoto's thyroiditis or Graves' disease are the most sensitive to an iodine load, which can tip the thyroid into an underactive or, less often, overactive state. If you have autoimmune thyroid disease, be careful with kelp tablets, seaweed supplements, and iodine drops. Do not self-start iodine: decide it with your doctor.

Nodular goiter and older adults

A gland with autonomous nodules can be driven into overactivity by a big iodine load. It is more common in older adults and in people from previously iodine-deficient areas. A sudden high-iodine intake (from a supplement, or from iodine-containing medicines and contrast dyes) is the trigger to watch for.

Kelp, seaweed, and sea-vegetable supplements vary enormously

Kelp iodine content ranges from roughly 16 to more than 8,000 micrograms per gram. A single serving can deliver many times the 150-microgram daily need, or exceed the 1,100-microgram ceiling. If you use seaweed foods or supplements, keep the amount small and measured. Some products also carry heavy metals such as arsenic.

Lugol's solution and high-dose iodine drops

Lugol's and similar concentrated iodine products deliver thousands of times a day's requirement in a few drops. They are sometimes promoted as a general tonic or thyroid-support product. For someone already iodine-replete they offer no established benefit, only the excess-iodine risk. This is not a supplement to take on your own initiative.

Pregnancy is the one time to be deliberate in both directions

Too little iodine risks the developing brain, and too much can suppress the fetal thyroid, so pregnancy is where both ends of the range matter most. The target is about 220 to 250 micrograms a day from food, iodized salt, and a prenatal with roughly 150 micrograms. A high-dose product is not the way to reach it.
