What It Is

The oral microbiome is the bacteria that live in your mouth, on the teeth, the tongue, the cheek and the pocket below the gum line. Combined culturing and gene-sequencing surveys have catalogued on the order of 700 bacterial species and phylotypes, and about half of them have not yet been grown in a laboratory. Each surface carries its own set of species. Disease starts when the balance among them shifts into a state called dysbiosis, and that shift underlies the two most common oral diseases, tooth decay and gum disease.

Hold the oral community apart from the gut microbiome. The gut community lives largely without oxygen, deep in the intestine, and is sampled through stool. The oral community sits in the open, bathed in saliva and air, and is reshaped several times a day by what you eat and drink. A finding about one does not carry to the other.

How It Works

What the Mouth's Bacteria Do

1Hundreds of species share the mouth

The mouth is a habitat spread across the teeth, the tongue, the cheek and the pocket below the gum line. A stable, diverse community takes up the nutrients and space. That competition limits how far the acid-making and disease-associated species can grow, and it is one of the ways a healthy mouth stays healthy.

2Sugar and frequency turn the mouth acidic

Decay is driven by acid. When you eat something sugary, plaque bacteria ferment it and produce acid, and the pH at the tooth surface drops. Saliva buffers a single exposure within about an hour. Frequent sugar keeps the mouth acidic for longer. That favors acid-tolerant species such as Streptococcus mutans and lactobacilli, whose acid dissolves the mineral in enamel. How often you eat sugar matters as much as how much. That all-day pattern is largely a product of ultra-processed foods and sweetened drinks built for constant snacking.

3Tongue bacteria feed the nitric oxide route

Some bacteria on the tongue perform a step human cells cannot. They convert dietary nitrate, concentrated in beetroot and leafy greens, into nitrite. The body then turns that nitrite into nitric oxide, the short-lived gas that relaxes blood vessels. The body normally makes nitric oxide inside the vessel wall. This second route skips that and runs entirely on living mouth bacteria, so anything that kills those bacteria can raise your blood pressure.

Gum disease works differently from decay. As plaque matures in the pocket below the gum line, the balance shifts toward species that grow without oxygen and provoke the immune system. The body's inflammatory response to them, as much as the bacteria themselves, damages the tissue and bone that hold teeth in place. The advanced form is periodontitis. It is what connects the mouth to the rest of the body, because it is a chronic, low-grade inflammation across an ulcerated surface open to the bloodstream.

What It Connects To

Periodontitis connects to several major diseases, and the strength of those links is uneven.

The most studied link is to the heart. A consensus review of the epidemiology finds periodontitis independently associated with a higher risk of atherosclerotic cardiovascular disease. Treating the gums improves markers of vessel function and inflammation. Because smoking, age, diabetes and social circumstances all raise both gum disease and heart disease, part of the association is shared risk. Treating periodontitis has not been shown to prevent heart attacks or strokes, so the direction of cause is not settled.

The link to metabolism runs both ways. Diabetes raises the risk and severity of gum disease, and gum disease tracks with worse blood-sugar control. A joint diabetes and periodontology consensus set out this two-way relationship. This link is stronger than the heart link because a trial can act on it.

Pooling thirty trials in people with type 2 diabetes, periodontal treatment lowered HbA1c by about 0.43 percentage points at three to four months.

HbA1c is the three-month average blood-sugar marker, and that drop is about the size some glucose-lowering drugs achieve. A smaller reduction persisted to six months. Because the treatment was tested against no treatment, the change can be attributed to it, and it supports gum care as part of managing type 2 diabetes alongside medication.

Three further connections sit at an earlier stage of evidence.

  • Pregnancy. Maternal gum disease is associated with higher odds of preterm birth and low birth weight. Trials that treated periodontitis during pregnancy did not reduce those outcomes, so this stays an association whose cause is not established.
  • Rheumatoid arthritis. Laboratory work has shown a periodontal bacterium can make immune cells tag the body's own proteins the same way the rheumatoid immune system does. That is a possible molecular link from gum infection to autoimmunity, still only a mechanism.
  • Alzheimer's disease. Porphyromonas gingivalis, a leading gum-disease bacterium, and its toxic gingipain enzymes have been found in the brains of people who died with Alzheimer's disease. Mouse experiments support the link. That work was funded in part by a company developing a drug against the bacterium, so it carries a conflict of interest. A later human trial of that drug did not meet its main goal. That keeps a role in human Alzheimer's disease an early lead.

Two more connections work through the nitric oxide route, and they are the ones you can act on most directly. Because tongue bacteria do the step that turns dietary nitrate into nitrite, eating nitrate-rich vegetables lowers blood pressure. A daily glass of nitrate-rich beetroot juice lowered blood pressure by roughly 8/4 mmHg in people with hypertension. In a mechanistic study the effect disappeared when volunteers stopped swallowing their saliva, which shows the tongue bacteria are necessary. The same dependence explains an unexpected cost of antiseptic mouthwash. Chlorhexidine mouthwash cut the tongue's conversion of nitrate to nitrite by about 90 percent and raised systolic blood pressure by roughly 2 to 3.5 mmHg. The effect held in healthy volunteers over a week and in treated hypertensive adults over three days.

Several findings in the table above are graded as mechanism, because you cannot randomly assign someone a different set of mouth bacteria. The decay and nitric oxide mechanisms are well established. The diabetes and heart links are moderate and mostly observational. The Alzheimer's disease and rheumatoid arthritis links are early leads.

What You Can Do

The aim is to keep the mouth's community in balance. Stripping the mouth of bacteria removes the species that limit decay and the ones that supply the nitric oxide route. Brushing, cleaning between the teeth, and eating nitrate-rich vegetables protect both the teeth and the blood-vessel route.

A few everyday habits keep the community in balance:

  • Clean mechanically. Brushing and cleaning between the teeth removes plaque and is the foundation. Cleaning between the teeth reaches the pocket where gum disease starts, which a brush cannot. Cleaning the tongue removes the coating where a large share of the community lives, without wiping out bacteria everywhere the way a strong antiseptic does.
  • Treat gum disease. Bleeding or receding gums are the start of periodontitis, and having them treated has the firmest whole-body payoff of anything here. In people with type 2 diabetes it modestly improved blood-sugar control.
  • Space out sugar. What drives decay is how long the mouth stays acidic, so how often sugar reaches the teeth matters more than the total. Keeping sweet drinks to mealtimes gives saliva time to recover the pH.
  • Eat nitrate-rich vegetables. Leafy greens such as spinach and arugula, along with beetroot, feed the tongue bacteria that supply the nitric oxide route, at low cost.
  • Antiseptic mouthwash and the nitrate route. A strong daily antiseptic rinse lowers the tongue bacteria that supply nitric oxide, and the studies above measured a rise in systolic blood pressure when that happened.

Common Questions

Does gum disease cause heart disease?

Not proven. The two travel together even after other risks are counted, but the same drivers push both up, so part of the tie may be shared risk. No trial has shown that treating the gums prevents a heart attack or stroke.

Is mouthwash bad for me?

It depends on the mouthwash and the reason. A strong antiseptic used every day lowers the tongue bacteria behind the nitric oxide route, which is why it can raise blood pressure. A rinse a dentist prescribes for a specific problem, used for a set time, is a separate question from a daily habit.

Does flossing matter?

The goal is to clean the space between the teeth, where a brush does not reach. Floss, an interdental brush, or a water flosser all reach it: use whichever one you will actually do each day.

Can my mouth affect my blood sugar?

Yes, and the effect is causal, because the trials tested gum treatment against no treatment. In type 2 diabetes, gum disease and blood sugar each make the other worse, so gum care earns a place in the plan next to medication.

Do gum bacteria cause Alzheimer's disease?

No. The evidence so far does not support it. A gum bacterium and its enzymes have turned up in affected brains. But that is a lead under investigation, and the one drug built on the idea failed its main trial.
