The Old Practice of Lightening
A cleanse, in the older sense, is a stretch of eating more simply, resting the digestion, and leaning on plants that support the liver and gut. The urge is old and near-universal, and it takes similar forms across cultures:
- Across Europe, people ate bitter spring greens, dandelion, and nettle after a heavy winter diet.
- Ayurveda times its cleansing practices to the turn of the seasons.
- Chinese medicine associates spring with clearing and favors plainer, lighter meals to unburden the digestion.
- Fasting for a set stretch runs through many traditions, from Lent to Ramadan to the fast of Yom Kippur.
- Bitter herbs for the liver and digestion, among them milk thistle, dandelion root, artichoke, and gentian, appear wherever people have tended the gut and the liver.
People have followed this for thousands of years. More have tried it, over more generations, than any trial could enroll. The common thread is sound: eat more simply, favor bitter and fibrous plants, rest the digestion, follow the seasons.
What Your Body Already Does
Modern physiology finds this clearing work already running around the clock, across three organs: the liver, the kidneys, and the gut. They respond to how you eat, drink, sleep, and move, the same levers the old practice pulls.
How Your Body Clears Waste
1The liver does the chemistry
The liver runs the main work in two linked stages. Phase I enzymes, mostly the cytochrome P450 family, change a fat-soluble compound by adding or exposing a reactive group. Phase II enzymes then attach a water-soluble handle, such as glucuronic acid, sulfate or glutathione, so the product can leave the body in bile or urine.
2The kidneys filter to urine
The kidneys filter the blood continuously and pass water-soluble waste into urine. They do this job better when you are well hydrated. That is the plain reason drinking enough water matters.
3The gut, and the exits
Waste bound to bile leaves the liver and exits in the stool. That is why fiber and regular elimination matter. The skin and lungs carry off smaller amounts. Together these routes are what people are really pointing at when they say detox.
Because the system runs continuously, you can only help these organs work: there is nothing to restart. Alcohol, smoking, and a diet stripped of fiber and vegetables make the work slower. Fiber, cruciferous vegetables, water, and sleep make the job of all three organs easier.
What the Evidence Supports
Broccoli, cabbage, kale, and Brussels sprouts switch on the phase II enzymes the liver uses to package waste for removal. In a randomized trial of 291 adults in a heavily polluted part of China, a daily broccoli-sprout drink raised urinary excretion of the airborne carcinogen benzene by 61% against placebo.
Dietary fiber, around 25 to 30 grams a day, tracks with better bowel function and with lower all-cause mortality in large population studies. Read the mortality link with care. People who eat more fiber also tend to be more active and better fed across the board, so how much of the benefit comes from the fiber itself is uncertain. What fiber directly causes is the bowel effect: more stool bulk, and the binding of bile acids for removal.
Milk thistle (Silybum marianum) has been used for liver and gallbladder complaints since at least the time of Dioscorides and Pliny. Its seed extract, silymarin, is the most studied liver-support botanical in modern medicine.
The clearest human evidence is death-cap mushroom poisoning (Amanita phalloides), where intravenous silibinin blocks the transporter that carries the toxin into liver cells. Across roughly 1,500 recorded cases, treated mortality runs under 10%, against more than 20% with older regimens. It is now a first-choice hospital treatment.
For established chronic liver disease the picture is mixed. A Cochrane review of 13 trials in 915 people with alcohol- or hepatitis-related disease found no clear effect on survival. A rigorous US trial in hepatitis C found no change in liver enzymes or viral load. A 48-week trial in fatty-liver disease (non-alcoholic steatohepatitis) missed its main endpoint. But more people on silymarin had liver scarring improve by at least one stage: 22.4% against 6.0% on placebo.
Taken together, milk thistle is life-saving in one acute emergency, shows a modest signal in fatty liver, and has no confirmed benefit in established viral or alcoholic disease. Part of the reason is poor absorption by mouth. As a low-risk daily herb for general liver support it is reasonable, alongside less alcohol and better eating.
The cellular version of clearing out is autophagy: cells breaking down and recycling their own worn-out parts. It switches on when fuel is scarce, so a form of time-restricted eating is the evidence-aligned way to lean on it. In a small crossover trial, an early daily eating window nudged up an autophagy gene, though most of the strong evidence is still from animals.
Where the Products Fall Short
The cleanse industry took that old language of clearing out and attached it to products. When those products are tested, they do not do what the label claims:
- Reviews of commercial detox diets found no trial showing removal of any named toxin. The quick weight change on a juice fast is water and calorie restriction that returns.
- Detox teas mostly act as stimulant laxatives, such as senna.
Ways to Support Your Detox Organs
The enduring version of a cleanse is a change in how you eat plus a few well-chosen plants. The organs that do the work are already on the job; what helps is giving the liver, kidneys, and gut the conditions to do it well.
Cruciferous vegetables drive the same clearing enzymes; fiber from vegetables, legumes and whole grains keeps the gut moving. Eating this way is the most reliable single change you can make.
Pale-yellow urine is a simple everyday marker that you are drinking enough, with no need to track liters. That is all the kidneys need.
Milk thistle has the most modern human data of the bitter liver herbs, usually taken as a standardized silymarin extract with food. It helps most as a small addition to cutting back on alcohol and eating better; on its own the effect is modest. Dandelion, artichoke and other bitters sit in the same long tradition of supporting the liver and digestion.
Alcohol is the single largest avoidable load on the liver for most people. Drinking less is one of the highest-return supports you can give the organ, and it costs nothing.
Repair and clearance run mostly during sleep. Cutting smoke removes a major stream of compounds the liver must process. Both are unglamorous, and both do real work.
A diet built on vegetables, fruit, whole grains, legumes, nuts, fish and plain dairy gives all three organs what they need to work well.
This leans on autophagy, the cellular cleanup that fasting switches on. It does not suit everyone, so read the fasting page before starting.
Detox foot baths, teas, and juice cleanses have been tested, and none removes a toxin the liver and kidneys were not already clearing.

A daily broccoli-sprout drink raised benzene excretion 61% in a 291-person trial
In a large trial in a polluted part of China, people who drank a daily broccoli-sprout drink cleared about 60% more of a cancer-causing air pollutant, benzene, in their urine than those on placebo. Cruciferous vegetables switch on the body's own phase II detox enzymes.
Egner and colleagues randomized 291 residents of Qidong to a broccoli-sprout beverage or placebo. The beverage produced a rapid and durable 61% rise in the urinary excretion of the benzene-derived mercapturic acid and a 23% rise in the acrolein metabolite. The mechanism is induction of the Nrf2/ARE pathway by sulforaphane, upregulating phase II enzymes such as glutathione S-transferases that conjugate reactive electrophiles for excretion. A later dose-ranging trial confirmed the benzene effect was dose-dependent.
This supports the ordinary advice to eat cruciferous vegetables such as broccoli, cabbage, kale and Brussels sprouts regularly as support for the body's clearance enzymes; it points to food, not to buying concentrated sprout-extract supplements, which carry their own risks.

Fiber near 25 to 30 g a day tracks with 15% to 30% lower mortality and better bowel function
Eating more fiber, roughly 25 to 30 grams a day, is tied to living longer and to more regular bowel movements. Fiber helps the gut do its everyday job of carrying waste and cholesterol by-products out in the stool.
Reynolds and colleagues pooled prospective studies and randomized trials on carbohydrate quality. Higher fiber consumers had a 15% to 30% lower risk of all-cause and cardiovascular mortality and of several diseases, with a dose-response relationship and intakes of 25 to 29 g/day as a reasonable target. Fiber's laxation effect (raising fecal mass and frequency) and its binding of bile acids in the gut are the mechanistic links to the gut's role as an elimination and cholesterol-disposal route.
Aim toward 25 to 30 g of fiber a day from vegetables, fruit, whole grains, legumes, nuts and seeds, raising the amount gradually with enough water so the gut adapts comfortably; the fiber page covers how to build up and the exception for some people with IBS.

Milk thistle cuts death-cap poisoning deaths below 10%; modest, mixed help for chronic liver disease
Milk thistle has been a liver herb for about two thousand years, and its active extract silymarin is the best-studied liver-support plant we have. It is a first-choice hospital treatment for death-cap mushroom poisoning, where it clearly lowers the death rate. For everyday chronic liver disease the trials are mixed: no clear survival benefit, but a fatty-liver trial saw less scarring on silymarin.
Silymarin is a flavonolignan complex (silibinin, silydianin, silychristin) from Silybum marianum seeds; documented use for liver and gallbladder complaints runs from Dioscorides and Pliny to the present (Abenavoli 2018). Proposed mechanisms are antioxidant and anti-inflammatory action, stabilization of the hepatocyte membrane against toxin uptake, and stimulation of ribosomal protein synthesis aiding hepatocyte regeneration. In amatoxin poisoning, IV silibinin competitively blocks the OATP1B3 transporter that carries the toxin into liver cells; a review of about 1,500 cases reports mortality under 10% with Legalon SIL versus more than 20% with penicillin-based regimens, with no RCT on ethical grounds (Mengs 2012). The Cochrane review (Rambaldi 2007; 13 RCTs, n=915) found RR 0.78 (0.53 to 1.15) for all-cause and 0.57 (0.28 to 1.19) for liver-related mortality restricted to high-quality trials, i.e. no confirmed benefit and generally low trial quality. The interferon-nonresponder hepatitis C RCT (Fried 2012, JAMA; n=154; 420 or 700 mg three times daily for 24 weeks) found no difference from placebo in ALT normalization (3.8 to 4.0% across arms) or HCV RNA. The NASH RCT (Wah Kheong 2017; n=99; 700 mg three times daily for 48 weeks) missed its primary NAFLD-activity endpoint (32.7% versus 26.0%, P=0.467) but found fibrosis improvement of at least one stage in 22.4% versus 6.0% (P=0.023) and a 30% or greater fall in liver stiffness in 24.2% versus 2.3% (P=0.002).
Milk thistle is a reasonable, low-risk herb for general liver support, best taken as a standardized silymarin extract with food. It is not a cleanse and will not undo heavy drinking, so it sits alongside less alcohol, fiber and vegetables, not replacing them. It is not a home treatment for suspected mushroom poisoning, which is a hospital emergency.

The Chinese Medicine View
Chinese medicine has its own long tradition of clearing what the body cannot process, and it reads best on its own terms. It puts the Spleen and Stomach at the center of digestion. The Stomach receives and ripens food; the Spleen transforms it, sending the useful part up and the turbid part down.
When that transforming work is strong, food and fluid become what the body needs. When it is weak or overloaded, they stagnate. What stagnates congeals into what the tradition calls dampness, and in time phlegm, felt as heaviness, bloating, fatigue, and a thick tongue coat. The Liver, in this system, courses the Qi and keeps things moving; when it stagnates, that movement stalls. Heavy, greasy, over-sweet, and alcoholic food is understood to generate Damp-Heat and turbidity, the very burden the body then has to clear.
The tradition and the modern picture reach a similar place by different routes. Both value clearing as much as taking in, and both point away from rich excess toward simpler eating. The tradition clears accumulated turbidity through food and habit:
- plain, warm, moderate meals that support the Spleen,
- movement that keeps the Qi coursing, and
- seasonal attention to lightening the diet, spring in particular being a time the tradition associates with clearing.
A practitioner fits this to the person. The tradition steers the already depleted, cold, or weak away from fasting and purging. A harsh cleanse can injure the Spleen it is meant to support.

Slimming and detox supplements are spiked with hidden drugs like sibutramine
The assessment of the FDA database catalogued unapproved and approved drugs found in dietary supplements over fifteen years. Weight-loss, sexual-enhancement and muscle-building products dominated, and the concealed agents included sibutramine (withdrawn for cardiovascular risk), prescription stimulants, diuretics and laxatives. An oxalate-rich green-smoothie cleanse caused biopsy-confirmed acute kidney injury
Makkapati and colleagues report a patient who developed acute kidney injury after an extended green-smoothie cleanse; biopsy showed abundant polarizable calcium-oxalate crystals in the tubules, the hallmark of oxalate nephropathy. Leafy greens such as spinach are high in oxalate, and consuming them in large concentrated quantities over days raises urinary oxalate enough to crystallize in the kidney. Take extra care with fasting and juice cleanses if you have kidney or liver disease, diabetes, or are pregnant
Cleanses, prolonged fasting, juice-only regimens, and high-dose supplements can be risky for people whose kidneys or liver are already compromised, for anyone managing diabetes, and in pregnancy. If any of these apply to you, talk it through with a clinician before starting, instead of following a general protocol.
Laxative and juice regimens can collide with your medicines
Laxative-based teas can lower potassium, which matters if you take heart or blood-pressure drugs, and both laxatives and juice-only days change how and when medicines are absorbed. If you take regular medication, tell whoever prescribes it before a cleanse.
Some "detox" and weight-loss supplements have injured the liver
Herbal slimming and detox supplements have caused liver injury in otherwise healthy people, with garcinia cambogia and high-dose green-tea extract among the documented examples. A product being herbal does not make it gentle on the liver, so treat a concentrated detox or fat-burning supplement with the same caution as a drug, and stop it if you develop nausea, dark urine, or yellowing of the skin or eyes.
Slimming and detox products sold online are often adulterated
Slimming and detox products bought online have repeatedly been found to contain undeclared prescription drugs, including sibutramine, a weight-loss drug that was withdrawn from the market for raising the risk of heart attack and stroke. The capsule does not always match the label, so avoid unregulated weight-loss products sold on their promises alone.

Does my body need help detoxing?
No. Your liver, kidneys, and gut clear waste continuously, so nothing sits stored for a product to release. The help that works is plain: cut the load with less alcohol and less smoke, then give the organs fiber, water, and sleep. That slow, ordinary version is what actually works.
Do detox teas and juice cleanses remove toxins?
Not in any test so far. They also run low on protein.
Juice-only cleanses carry a risk the label skips: built on oxalate-rich greens, they have caused acute kidney injury in reported cases.
Why does the water turn brown in a detox foot bath?
The brown comes from a chemical reaction, and nothing is leaving your feet. Run the test both ways, feet in the salty water and no feet at all, and the water browns the same.
