What It Is
Tai chi (太極拳, taijiquan) is a martial art built around a form: a fixed choreographed sequence of 8 to 108 movements. You perform it continuously while your weight transfers from one leg to the other and the waist turns. Learning it means memorizing a sequence, which takes months. Qi gong (氣功) is older and broader, a family of health exercises, usually a handful of movements repeated many times or held standing, coordinated with the breath and directed attention. A single movement from the Baduanjin set, the Eight Brocades, can be taught in five minutes.
Most classes in the West teach Yang style or a simplified Yang routine like the 24-form. Sun style uses higher stances and suits stiffer knees. Chen style's low stances and fast releases are the most demanding.
What The Evidence Best Supports
Falls is the strongest finding and the reason to start. In a large Cochrane review, older adults who practice tai chi fell about 20% less, and that figure carries the review's highest grade of certainty. It also held up against the toughest comparison group. In a 2018 trial of older adults already at high risk of falling, a therapeutic routine cut falls 58% against stretching. Against a full balance, strength and aerobic program it still cut them 31%. The effect is smaller against active comparison groups than against a passive control.
The strongest falls trials used one specific routine: Tai Ji Quan: Moving for Better Balance, an 8-form therapeutic adaptation built to load balance and gait. Dr Fuzhong Li led both the high-risk falls trial and the Parkinson's trial below. He founded Exercise Alternatives LLC and licenses the branded program. Public-health and aging agencies then spread it at no cost to participants. The trial evidence is strong on its own terms, and the developer holds a commercial interest in the program.
Below falls, the strongest results are in balance and pain:
- Balance, fear of falling and Parkinson's disease. Tai chi eases fear of falling and steadies dynamic balance. In Parkinson's disease it cut fall rate and improved balance more than resistance training or stretching, with gains that held three months after the program ended. Parkinson's progression itself is unchanged; the benefit is steadier balance and fewer falls.
- Fibromyalgia. Against supervised aerobic exercise, the standard exercise advice, tai chi improved the main symptom score by about 5.5 points at 24 weeks. That score is the Fibromyalgia Impact Questionnaire, a 0–100 scale. More of it worked better than less.
- Knee osteoarthritis. Tai chi matched standard physical therapy, with no significant difference between them, so it is a reasonable alternative for someone who prefers it or can reach it more easily.
Outside those two, the results are mixed:
- Blood pressure. In people with hypertension, tai chi lowered the top number, but by about 10 mmHg in English-language trials against about 19 mmHg in Chinese-language ones for the same practice. That gap is a measured sign of publication bias.
- Chronic heart failure. 12 weeks of tai chi did not move six-minute walk distance or peak oxygen uptake at all, though quality of life and mood improved. Its value in heart failure is wellbeing and exercise confidence, not measured fitness.
- Thinking in older adults. An apparent benefit more than halved once the review kept only its higher-quality trials.
- The movement itself. In a 2025 trial in breast cancer survivors, even a sham qi gong beat education and support on sleep and cognition. The sham was matched gentle movement with the breath and attention stripped out, so the movement itself likely carried much of the benefit.

How It Works
Most of the measured benefit has a plain mechanism under it. Tai chi shifts your weight slowly between your legs, slow enough that you must catch and correct your own balance. That trains the proprioception, gait and leg muscles that keep you upright. The breathing is deliberately slow and tied to the movement. That activates the parasympathetic nervous system, the body's calming, rest-and-digest state.
And it is group exercise done regularly, so the benefit tracks how often you turn up.
Because nobody in a tai chi trial can be blinded, the control group determines what a result means. Against stretching or a waiting list, a trial mostly captures the effect of turning up twice a week and moving. Against an active exercise program, it captures only what is specific to the form.
Anatomy of the Practice
1The first sessions
You learn the outline of the form and begin moving slowly from foot to foot. Most people feel calmer and steadier by the end of a first session, before anything has changed in the long run.
2Over the first weeks
Practiced twice a week, balance, gait and proprioception improve, and confidence on your feet grows with them. Twelve weeks is the shortest span that reliably moved an outcome in the trials, and balance gains often show by about three months.
3Over months
Fall rates drop in older adults, and fibromyalgia symptoms, mood and self-confidence keep improving the longer the practice continues.

Ways to Do It
The trials that worked ran two 60-minute sessions a week. Start at the first rung and add the others as they suit you. If you are older, unsteady, or new to it, begin with a higher stance and shorter sessions, and build slowly.
Full-length 8-form, 24-form and Baduanjin videos are on the open web. A video teaches the shape of the sequence for free: most of the way in. It can't check your knee position, so use it to learn and maintain the form, and pair it with a class or teacher for correction.
Senior centers, hospital wellness programs, libraries and public parks run classes at no cost in most cities, and morning park practice is open to almost anyone. Ask by name for a fall-prevention or arthritis-adapted class: that is the version the strongest trials tested.
Seated tai chi and qi gong have their own trials: in older wheelchair users they lowered depression and lifted quality of life over 26 weeks. Seated work trains the trunk, the arms, the breath and the attention. It is a sound place to begin for anyone not yet steady on their feet. It does not train standing balance. If fall prevention is the goal, the practice eventually has to happen on your feet, supported within reach of a rail or chair.
A paid class or private teacher can see and correct what a video cannot, your knee position and stance depth in real time. Look for one who names their style and lineage without being asked, and offers a higher stance or a chair to whoever needs it. A good teacher can also tell you what each movement is training; if they can't, you are only learning the steps.

The Chinese Medicine View
The ancestor of both practices is 導引, daoyin, "guiding and pulling". The oldest surviving picture of it is the Daoyin Tu, a silk painting of forty-four figures in exercise postures from a tomb at Mawangdui sealed in 168 BCE. What the tradition says these exercises do is regulate three things at once: 調身, 調息, 調心, the body, the breath and the mind. Posture opens the pathways, Qi follows the breath, and attention directs both.
The lower Dan Tian (丹田, "cinnabar field") sits in the lower abdomen. The instruction 氣沉丹田, sink the Qi to the Dan Tian, means letting the breath and the center of gravity settle low in the belly, below the chest. The governing maxim is 以意領氣, intent leads the Qi: where attention rests, the classical claim goes, Qi gathers. This whole frame belongs to Yang Sheng, nourishing life, the preventive half of Chinese medicine, where the aim is to keep a person well before anything has gone wrong.
The trials, by contrast, measured proprioception, gait mechanics, blood pressure and self-reported symptoms. The two meet on the breath: each tradition treats slow, settled breathing as the link between body and mind. The trials credit that same slow breathing for the calming effects.
The tradition also holds that the practice does not suit everyone the same way, and the guidance turns on your pattern:
- Someone who is Yang deficient and runs cold is directed toward shorter sessions, warmer rooms and more movement than static standing. Practicing outdoors in wind or cold is discouraged.
- Someone with Yin deficiency and heat is pointed toward slow, quiet, low-effort practice and away from forceful breath work.
- Someone who is depleted is told to do less and rest; the tradition holds that practicing to exhaustion depletes reserves.
Practice is traditionally done on an empty stomach, before eating. If you have a practitioner, this is a good thing to ask about, because the answer depends on your pattern.

Knees take the load in low stances
The problems come from a few positions: deep stances, long holds in a bow step, the front knee traveling past the toes, and turning on a foot that bears your weight. Keep the knee behind the toes, raise the stance, and pivot on the heel; don't twist a planted, weighted knee.
If standing unsupported is a fall risk
For the first sessions, practice within reach of a rail or chair, with someone present. If standing is not yet safe, start seated and progress to standing once you are steadier.
Severe osteoporosis, acute joint injury, or a recent joint replacement
With severe osteoporosis, keep to higher stances, avoid forceful trunk rotation and any partner or push-hands work, and practice where a fall cannot happen. After an acute joint injury or a recent joint replacement, start seated and build up slowly. Uncontrolled vertigo, symptomatic aortic stenosis and unstable angina are the situations to have a doctor sign off on any exercise program, this one included.
Forcing the breath or over-concentrating
The classical picture of practice going wrong (走火入魔) is consistent: forcing or holding the breath, or concentrating on the Dan Tian too hard or too long. It also comes from chasing sensations and practicing while agitated or grieving. The reported results are chest tightness, headache, palpitations, a sense of Qi stuck in the head, insomnia, and at the far end agitation. In most reported cases the practice triggered symptoms in someone already vulnerable; it did not create the state on its own. Build up slowly, keep the breath easy, and if these symptoms recur, ease off and find a teacher before continuing.

Common Questions
What is the difference between tai chi and qi gong?
Tai chi is a memorized form built on stepping and weight transfer; qi gong is a short set of movements repeated with the breath. That stepping is why the falls evidence is almost all tai chi. The qi gong record is mostly gentler standardized sets, like the eight-movement Baduanjin, measuring blood pressure, glucose, fatigue and mood, with fewer independent replications.
How long before anything changes?
More is better where it was tested: 24 weeks of practice beat 12 by 9.6 points on the same 0–100 scale. And if falls are your reason for starting, hold to twice a week for the first six months, not just the twelve-week minimum.
Is tai chi enough exercise on its own?
For balance, mobility and fall risk in an older adult, it is the single best-evidenced choice. As general conditioning it runs only about 2 to 3 metabolic equivalents, roughly a slow walk. Pair it with resistance training for strength and bone, and with harder cardio for aerobic fitness.
