Vitamine D heeft één duidelijk vastgestelde taak: het voorkomen en omkeren van de botverzachtingsziekte die een tekort veroorzaakt, rachitis bij kinderen en osteomalacie bij volwassenen. De grote studies van het afgelopen decennium hebben de rest van zijn reputatie ingeperkt. What It Is

Vitamin D is a fat-soluble hormone. The body can make its own from sunlight, so it is not truly a vitamin. It comes in two forms: D3 (cholecalciferol), made in skin and found in animal foods, and D2 (ergocalciferol), from some plants and supplements. The body converts both to the same active hormone. A blood test measures neither form directly. It reports 25-hydroxyvitamin D, the circulating storage form, in nanograms per milliliter (ng/mL) or nanomoles per liter (nmol/L). The same status reads as about 30 ng/mL on one scale and about 75 nmol/L on the other, a small number and a large one for identical vitamin D.

Anatomy of the Practice

1Made in the skin

Ultraviolet B light converts a cholesterol precursor in your skin into vitamin D3. How much you make depends on skin tone, latitude, season and how much skin is exposed. This is the main natural source for most people; food supplies only a little.

2Activated in liver and kidney

Vitamin D from skin or food is converted first in the liver to 25-hydroxyvitamin D, the form a blood test measures, then in the kidney to its active hormone. That active form tells the gut to absorb calcium and helps lay it down in bone.

3Working on bone and muscle

With sufficient vitamin D, calcium is absorbed and bone mineralizes normally. Without it, bone softens and a proximal muscle weakness can set in. This is why the clearest benefits of supplementation appear in people who were deficient, and are absent in those who already had plenty.

How It Works

The mechanism is calcium. Vitamin D lets the gut absorb dietary calcium and lay it down in bone; without it, the skeleton loses mineral and softens. The reserve a blood test tracks, 25-hydroxyvitamin D, is what the body draws on when intake falls.

Vitamin D has a threshold. Below sufficiency, more helps a great deal; at sufficiency, more does nothing; sustained intake far above 4,000 IU a day causes harm.

What Changed

For a decade the message was simple: almost everyone is deficient, and a daily dose prevents cancer, heart disease, and fractures. It rested on observational studies, where people with low vitamin D were also the sicker ones. A low level marks poor health as much as it causes it. The housebound, the chronically ill, and people with obesity all tend to run low, and that confounds the correlation. The randomized trials of the 2010s tested the claim directly.

The trials that found nothing shared one feature: they enrolled people already sufficient in vitamin D. VITAL tested 2,000 IU a day against cancer and heart disease in 25,871 adults. D2d tested 4,000 IU a day against diabetes in adults with prediabetes. The VITAL fracture analysis and DO-HEALTH tested fractures and physical function in healthy adults over 70. All came back null, because there was no shortfall to correct.

The trials that found a benefit enrolled the deficient and the frail. Around 800 IU a day, usually paired with calcium, cuts fractures in older, often institutionalized people. Pooling 46 trials in over 75,000 people, vitamin D modestly cut acute respiratory infections, an odds ratio near 0.92. The reduction concentrated in the trials that dosed steadily each day.

Over a median 5.3 years, confirmed autoimmune disease was lower in the vitamin D arm (HR 0.78, 95% CI 0.61 to 0.99, P=0.05), a 22% relative reduction, spanning conditions such as rheumatoid arthritis, polymyalgia rheumatica, autoimmune thyroid disease and psoriasis. Omega-3 alone did not significantly reduce confirmed cases, and the vitamin D and omega-3 effects were independent. Most participants were vitamin-D-sufficient at baseline, and because autoimmune disease is uncommon the absolute number of cases prevented across five years was small.

How to Get Enough

Ways to Do It

Most people need no plan beyond sunlight and food. Get some sun with skin exposed, eat the foods that carry vitamin D, and if you are likely to be deficient, add a modest daily dose. Testing is for people with a specific reason to check.

Short, regular time outdoors with some skin exposed lets your body make its own vitamin D. Dark skin, high latitude, or little time outside all mean you make less. If you live somewhere with few daylight hours through winter, sun alone will not cover those months.

Food supplies only modest amounts, but it adds up. Fatty fish such as salmon, sardines, and mackerel are the richest natural source; egg yolks and cod liver oil carry some. For an adult likely to be deficient, generic vitamin D3 (cholecalciferol) at around 1,000 to 2,000 IU a day is a sensible amount. Large bolus doses increase falls

The harm in the research is specific to very large, infrequent doses. A single 500,000 IU annual dose increased falls and fractures in older women, and high monthly doses aimed at raising blood levels caused more falls than a moderate dose. If you supplement, take a modest daily amount; a large occasional dose carries the downside without the benefit.

Some people should only supplement with medical guidance

People with conditions that already raise blood calcium, such as sarcoidosis or primary hyperparathyroidism, should supplement only under medical guidance. Extra vitamin D can push their blood calcium higher still.

Toxicity comes only from sustained high doses

Toxicity occurs only with sustained intake well past the 4,000 IU a day ceiling, not with a dose that corrects a deficiency. The gap between a sensible dose and a harmful one is wide. It comes from high-dose supplements, not from sun or food.

Should everyone take vitamin D?

The large trials of routine supplementation in already-sufficient people came back null, so a blanket habit for everyone is not supported. Correcting a genuine deficiency is a different matter and is worthwhile, since it reverses the bone-softening disease a severe shortage causes. The people who gain are those actually low: many older, frail, housebound, or malabsorbing people, and those who get little sun. A 25-hydroxyvitamin D reading is the only way to know whether you are one of them.

Does vitamin D prevent cancer or heart disease?

No, at least not in people who already have enough. VITAL tested exactly that and found nothing over about 5 years. It never enrolled deficient adults, the only group vitamin D is known to help.

How much should I take?

For an adult likely to be deficient, about 1,000 to 2,000 IU of D3 a day, taken every day rather than in one large occasional dose. Correcting a low level is the whole benefit; the trials found nothing beyond it.

Should I get my vitamin D level tested?

For most people, no. Routine testing is easy to over-order and rarely changes the plan. A 25-hydroxyvitamin D check is worth doing only when a specific reason applies, and a result already in the sufficient range needs no action.

Can you take too much?

Yes, but only from sustained megadosing. Sun and food never produce a toxic amount; that risk sits only with high-dose supplements.
