Findings & Outcomes

Most nutrition attention goes to the two ends of the scale: outright deficiency, the scurvy-and-rickets territory a doctor can diagnose, and megadosing, the shelf of high-strength bottles. Most people live in the middle, taking in a little less of several nutrients than they need, year after year, with no symptom to point at. In 2006 the biochemist Bruce Ames proposed that this middle ground still does slow harm over a lifetime.

What Inadequacy Means

A deficiency is a shortfall large enough to cause a clinical problem a doctor can diagnose. Too little vitamin C and gums bleed. Too little vitamin D in a child and bones soften. Inadequacy sits above that: an intake below the amount set as sufficient for the population, mild enough that nothing shows up day to day. Nutrition scientists size it with the estimated average requirement, or EAR: the daily intake that meets the needs of 50% of healthy people in an age and sex group. Counting how many people fall below the EAR is the standard way to measure a shortfall. It measures reported intake against a benchmark; it does not measure the level in your blood.

The Triage Theory

Ames's reasoning is evolutionary. For most of human history, shortages of vitamins and minerals came and went. So a scarce nutrient is used first for reaching reproductive age, and less is left for repairs that pay off only in old age. When a micronutrient runs short, the proteins and enzymes that depend on it for immediate survival keep their supply. The ones that handle slow maintenance (repairing DNA, clearing damaged mitochondria, keeping arteries and bone in order) lose theirs first.

Each shortfall causes damage that no test picks up as a deficiency. The damage can show up decades later as higher risk of cancer, heart disease and other diseases of aging. In a 2018 follow-up, Ames sorted nutrient-dependent proteins into survival proteins and longevity proteins. He named a set of candidate "longevity vitamins" whose supply drops the same way when intake is low. That framework rests on mechanism and evolution. It has not been tested as a chain of cause and effect in people.

The Evidence For Triage

The hierarchy has been traced in two nutrients, both in reviews by Joyce McCann and Ames. Vitamin K is the clearer case. It activates a family of proteins that need a chemical step called carboxylation, the Gla proteins. Some are clotting factors, and clotting is a survival function: bleed out and you do not reproduce. Others do slow maintenance work. Matrix Gla protein helps keep calcium out of artery walls, and osteocalcin builds bone. McCann and Ames argued that when vitamin K runs modestly short, the clotting proteins get carboxylated first while the maintenance proteins go undercarboxylated. That matches the observation that low vitamin K tracks with artery and kidney calcification and with fragile bone.

Selenium gave a second test. Humans make 25 selenoproteins, and if triage is real they should not all fail together when selenium runs low. McCann and Ames classified them and found a hierarchy. About five selenoproteins tied to short-term survival held their function on low selenium, while the seven or so tied to long-term protection lost theirs first. The same aging diseases that track with a modest selenium shortfall also track with genetic faults in those long-term selenoproteins. The ranking comes from cell and animal work and from disease associations; no human trial has tested it. Still, this is exactly what triage predicts, and the selenium data matched it.

How Common Shortfalls Are

Modest shortfalls of several nutrients are widespread, the best-supported piece of the theory. Using US national intake data (NHANES), researchers compared what people actually eat against the EAR for 19 micronutrients. A handful were rarely short: fewer than 8% fell below the requirement for folate, zinc, the B vitamins, iron and selenium once fortified foods were counted.

Several others were short in a large share of the population: vitamin D and vitamin E at the top, then magnesium, calcium, vitamin A and vitamin C. Only about a third of people reached the adequate-intake mark for vitamin K, and potassium was reached by almost nobody. Choline, absent from that count, falls short for most adults in other national analyses. These figures come from diet recalls that people report imperfectly, so the exact percentages carry error. The shortfalls cluster in nutrients that ordinary whole foods supply. Ultra-processed foods now supply about 58% of calories in the average US diet, and they are lower in magnesium, potassium and vitamin E than the whole foods they displace.

Does A Multivitamin Fix It

The common conclusion is that everyone should take a multivitamin. Large randomized trials test that step directly, and carry more weight than intake surveys. A systematic review for the US Preventive Services Task Force pooled dozens of trials and found the evidence insufficient to say multivitamins prevent cardiovascular disease or cancer. It recommended against beta-carotene and vitamin E for that purpose outright. A cohort that followed 390,124 US adults for up to two decades found no lower mortality among daily users. If anything the rate ran slightly higher early on, the sign of sick people starting supplements when their health is already failing.

Followed for up to twenty years, daily multivitamin users lived no longer than non-users: if anything they died slightly sooner early on.

The signals that do exist are modest. In the Physicians' Health Study II, a trial in 14,641 male doctors, a daily multivitamin lowered total cancer by about 8% over roughly 11 years. The drop was small in absolute terms, with no effect on cancer deaths, and the same trial found no reduction in cardiovascular events. In a separate three-year trial, older adults on a multivitamin scored a little better on cognitive tests than those on placebo. Some companion analyses have not reproduced that result, and it is still argued over.

From all sources combined (food, fortification and supplements), the share below the estimated average requirement was 70% for vitamin D, 60% for vitamin E, 45% for magnesium, 38% for calcium, 34% for vitamin A and 25% for vitamin C, while fewer than 8% were short on folate, zinc, iron and the B vitamins. Only about 35% exceeded the adequate intake for vitamin K and almost none for potassium.

Some companion analyses using different cognitive batteries have not reproduced the size of the effect, so it remains debated.

What The Evidence Points To

The common shortfalls are real, and the triage hierarchy has held where tested. No trial shows that pills for a mild shortfall make a person live longer or better. Eat whole food first, and correct the specific nutrients you lack. Most of the commonly short nutrients come cheaply from food:

  • Magnesium: nuts, seeds, legumes, whole grains and leafy greens.
  • Vitamin K: leafy greens, and as K2, fermented foods and animal fat.
  • Omega-3: oily fish such as salmon and sardines.
  • Choline: eggs and liver; selenium: a couple of Brazil nuts.
  • Calcium: dairy, tinned fish with bones and fortified plant milks.
  • Vitamin D is the exception: food alone rarely supplies enough, so it sits at the top of the shortfall list. Sunlight and, in dark months, a modest supplement are the usual routes.
  • Direct-to-consumer lab panels measure vitamin D, ferritin, magnesium and B12 without a referral. A real shortfall is worth correcting.

Common Questions

Does inadequacy make you feel tired or run down?

Not reliably. That is the trap: there is nothing to point to. A modest shortfall produces no day-to-day change, so fatigue or low mood cannot tell you which nutrient, if any, is short. By the triage theory, the damage runs for years with nothing to feel, which is the idea's central claim. If you feel unwell, get checked; do not assume a vitamin gap.

Is the triage theory proven?

No. What would settle it is a specific trial: take healthy people with a mild shortfall and correct it. Then follow them for decades to see whether cancer, heart disease or lifespan actually shift. That trial would need to isolate a single nutrient at a modest dose and run long enough for slow damage to surface. Nobody has mounted a study on that scale.

Do fortified foods already cover this?

Partly. Fortification is the reason some nutrients are rarely short while others still are. Grains and cereals were fortified with folate, iron and B vitamins decades ago, so those rank low. No such program reaches magnesium, vitamin D or vitamin E, which keeps them near the top of the shortfall list.

Who actually benefits from a multivitamin?

A multivitamin earns its place when there is a gap to fill. That means a restricted or low-calorie diet, pregnancy, an older adult who eats little, or a diagnosed low level of a specific nutrient. Outside those cases, correcting the one nutrient you actually lack, from the foods that carry it, does the same job.
