What It Is
Microplastics are plastic fragments smaller than five millimeters, and nanoplastics are the fraction below one micrometer, small enough to cross membranes that stop larger particles.
Plastics deliver two separate exposures. First, the particles themselves, microplastics and the far smaller nanoplastics. Second, the chemicals plastics are built from or carry, above all bisphenol A (BPA) and the phthalates. The particle science is newer and more uncertain; the chemical science is older, better understood, and holds the strongest evidence of harm.
Particles form as larger plastics weather and shed, and some are manufactured small on purpose. People take them in three main ways: through food and drink, through indoor air thick with synthetic-textile fibers and dust, and through packaging that touches food.
BPA is the building block of polycarbonate plastic and the epoxy resins that line food and drink cans, and it leaches from those linings into food. Phthalates soften plastics and carry fragrance, so they turn up in flexible packaging, vinyl, and many personal-care products.
Both leach out of the plastic over time. The bonds holding BPA into resins can break down, while phthalates only mix into plastic without ever bonding to it. They migrate steadily into food, water and household dust, which is why exposure is so common.
What the Evidence Shows
The first direct measurement of microplastics in human blood quantified plastic particles in most of a group of 22 healthy adults. A separate team found microplastics in placenta tissue, on both the maternal and fetal sides. A 2024 imaging method counted nanoplastics in bottled water at around 240,000 particles per liter, most of them nanoplastic-sized and far above earlier estimates. These are small, method-building studies; they show only that the particles reach the body and are bioavailable.
The best-established harm is hormonal. BPA behaves as a weak estrogen, confirmed in laboratory systems and across a large body of animal work, often at doses once treated as safe. Phthalates act as anti-androgens; they block the testosterone that male development depends on. The clearest human evidence involves male reproductive development. Boys whose mothers carried higher prenatal phthalate levels tend to show a shorter anogenital distance, an androgen-sensitive marker that echoes the animal finding. These human results are associations, and this developmental signal in boys is the steadiest of them.
A 2024 study examined patients who had plaque surgically removed from a neck artery. Most of the plaque samples contained microplastics. Patients whose plaque contained plastic went on to suffer markedly more heart attacks, strokes and deaths over the three years that followed. It is one cohort of a single patient type, observational, so it shows a strong association but cannot establish cause. This is the first human outcome study of its kind, and it needs replication.
The funding record shows up plainly in the low-dose BPA literature. When researchers tallied those studies through 2005, more than nine in ten of the government-funded ones found significant effects, while none of the industry-funded ones did. That split is part of why the regulatory picture trailed the laboratory one for years.

In a study of 22 healthy adults, tiny plastic particles were measurable in the blood of most of them, the first direct evidence that plastics get into the bloodstream.
Leslie and colleagues built a double-shot pyrolysis GC/MS method for particles of at least 700 nm and applied it to whole blood from 22 healthy volunteers. Four high-production-volume polymers were identified: polyethylene terephthalate, polyethylene, polymers of styrene (a sum of polystyrene and related plastics), and poly(methyl methacrylate); polypropylene fell below the limit of quantification. The mean of the summed quantifiable concentration was 1.6 micrograms per milliliter. The sample is small and the work establishes that these particles are bioavailable for uptake into blood, not a health outcome or a dose-response.

A new imaging method found on the order of 240,000 plastic particles per liter of bottled water, most of them nanoplastic-sized and too small for older methods to see.
Qian and colleagues developed single-particle stimulated Raman scattering microscopy, which identifies particles by their chemical fingerprint at high throughput, and applied it to bottled water from several brands. They counted about 2.4 (plus or minus 1.3) times ten to the fifth particles per liter, roughly 90 percent of which were nanoplastics smaller than one micrometer. That is orders of magnitude above prior microplastic-only estimates, because the smaller particles were previously invisible. The study characterizes what is present, not what any dose does in the body.

Ragusa and colleagues examined six placentas from consenting women with physiological pregnancies and found twelve microplastic fragments in four of them, spheric or irregular in shape and 5 to 10 micrometers in size, located on the fetal side, the maternal side and in the membranes. The particles were characterized by morphology and by Raman spectroscopy, and several carried pigments consistent with man-made coatings. It is a first observation of presence in a very small series.

None of the industry-funded BPA studies found a low-dose effect, against more than 90 percent of government-funded ones
When researchers counted up the BPA safety studies, government-funded ones usually found effects and industry-funded ones did not, a stark split by who paid for the work.
vom Saal and Hughes reviewed the low-dose BPA literature and found that 94 of 115 studies reported significant effects, many below the 50 microgram per kilogram per day reference dose then treated as safe. Broken down by sponsor, more than 90 percent of government-funded studies reported significant effects while no industry-funded study did, and they noted industry studies that discounted positive controls or used insensitive designs. The paper documents a funding pattern and argues the safety assessment of the day was out of step with the independent evidence.

Three days of fresh, unpackaged food cut BPA levels by 66 percent and phthalate metabolites by about half
Families who switched to fresh, unpackaged food for three days cut their BPA levels by about two thirds and their phthalate levels by about half.
Rudel and colleagues gave 20 participants in five families a three-day fresh-food intervention that avoided canned and plastic-packaged food, measuring urinary BPA and DEHP metabolites before, during and after. Geometric-mean BPA fell from 3.7 to 1.2 nanograms per milliliter (a 66 percent drop) and DEHP metabolites fell 53 to 56 percent, with maxima down 76 and 93 to 96 percent. It is small and short, and it shows that food contact drives a large, quickly reversible share of exposure.

Higher phthalate levels tracked with lower testosterone, about 29 percent lower in boys aged 6 to 12
In a national US survey, people with more phthalate breakdown products in their urine tended to have lower testosterone. The clearest single figure was a roughly 29 percent lower level in boys aged 6 to 12, and the strongest and most consistent associations were in women aged 40 to 60.
Meeker and Ferguson analyzed 2,208 NHANES 2011-2012 participants and found multiple phthalate metabolites associated with significantly reduced total testosterone across sexes and age groups. The strongest single numeric estimate was a 29 percent reduction (95% CI 6 to 47) in boys aged 6 to 12 for di-2-ethylhexyl phthalate metabolites; women aged 40 to 60 showed the strongest and most consistent inverse associations, and in adult men the inverse associations were confined to those aged 40 to 60. Because exposure and hormone were measured at the same time, this is an association, not a demonstrated cause.

Patients with microplastics in carotid plaque had 4.5 times the risk of heart attack, stroke, or death (n=257)
In patients who had plaque removed from a neck artery, those with plastics detectable in the plaque were about four and a half times as likely to have a heart attack, stroke, or die over the next three years.
Marfella and colleagues ran a prospective, multicenter study of patients undergoing carotid endarterectomy. Of 257 who completed a mean 33.7-month follow-up, polyethylene was detected in the plaque of 150 (58.4 percent) by pyrolysis GC/MS, with polyvinyl chloride in 31; electron microscopy showed jagged foreign particles among plaque macrophages. Patients with plastics in plaque had a hazard ratio of 4.53 (95% CI 2.00 to 10.27, P<0.001) for the composite endpoint. It is the first human outcome study of its kind, and it is observational.

How It Works
Both families act by imitating or blocking the body's own hormone signals. BPA binds the estrogen receptor and switches it on weakly, adding a small estrogen signal in responsive tissues like the breast and uterus. Phthalates lower the testosterone that fetal tissue needs to develop along the male pattern.
In rats, prenatal exposure to certain phthalates suppresses fetal Leydig-cell testosterone and produces a reproducible set of male reproductive changes. Researchers have mapped that phthalate syndrome down to its hormonal and genomic steps.
Hormones shape the body's development before birth and in early life, so that is where the risk concentrates.
What You Can Do
Most exposure arrives through food contact, so it is largely avoidable. Twenty people from five families ate only fresh, minimally packaged meals for three days. Their BPA fell by roughly two thirds and phthalate metabolites roughly halved.
- Keep heat off plastic. Warmth speeds migration, so shift food into a glass or ceramic dish before you microwave it, and let hot dishes cool before they touch plastic.
- Choose fresh and frozen. Lean toward fresh or frozen whole foods over canned or heavily packaged; can linings are a major BPA source and flexible packaging carries phthalates.
- Glass, steel, ceramic. These do not leach BPA or phthalates into food the way plastic does, and scratched, cloudy old plastic sheds the most.
- Bottled water. Bottled water carries a heavy nanoplastic load, and a home filter reduces particles in tap water.
- Indoor dust. Dust concentrates phthalates shed by vinyl and synthetics, so ventilating and damp-dusting lower that source, and a fine-filter vacuum traps more of it.
- Personal-care labels. On a label, "fragrance" usually hides phthalates, so a fragrance-free choice cuts it out.

Calafat and colleagues measured total urinary BPA in 2,517 NHANES 2003-2004 participants aged six and older and detected it in 92.6 percent, with levels varying by age, sex and race or ethnicity. Silva and colleagues measured seven phthalate monoester metabolites in NHANES 1999-2000 and found monoethyl phthalate, mono-n-butyl phthalate, monobenzyl phthalate and mono-(2-ethylhexyl) phthalate detectable in more than 75 percent of samples. Boys with the highest prenatal phthalate exposure had about ten times the odds of a shorter anogenital distance (n=85)
Swan and colleagues measured nine phthalate monoester metabolites in prenatal maternal urine and examined 85 boys after birth. Four metabolites were inversely related to the anogenital index (anogenital distance divided by weight), and a summary phthalate score decreased the age-adjusted index in a dose-dependent way; boys in the top exposure quartile had an odds ratio of 10.2 (95% CI 2.5 to 42.2) for a shortened index. Exposure is universal, so aim for less, not zero
Everyone carries some plastic-chemical exposure, and how strongly the particles themselves act at ordinary doses is not yet clear. The sensible response is steady reduction, handled the way you would ease off ultra-processed food, without alarm.
Pregnancy and infancy are where minimizing matters most
The hormone effects fall hardest on development, and the steadiest human signal is male reproductive development before birth. During pregnancy and early childhood, the low-cost packaging changes here are the ones most worth keeping up.
BPA-free does not mean well studied
When BPA is removed it is often replaced by close relatives such as BPS and BPF. These carry thinner safety data and seem to share some of the same hormone activity. Lowering plastic food contact overall is more reliable than swapping one additive for a close relative.
None of this needs to be total to matter, and the packaging moves are cheap and reversible. Treat it as sensible reduction, and consult a professional if you have specific concerns, especially in pregnancy.
Common Questions
How widespread is BPA and phthalate exposure?
Near-universal. National US surveys detected BPA in 92.6 percent of people sampled and common phthalate metabolites in most. That measures exposure, not damage. A single test says even less. A spot urine reading captures only the last day or two and swings from day to day, so one result is a rough gauge for one person. The population-level detection is what holds up.
Is BPA an endocrine disruptor?
Yes. The open question is size: how big BPA's effects are at the low, everyday doses people actually carry. That estrogen activity itself is not in doubt; what is unsettled is how much it changes health at those doses.
Do phthalates lower testosterone?
Human evidence points that way, though only from observational studies. In one national US survey, higher phthalate metabolites tracked with lower total testosterone across sexes and ages. For the DEHP group, boys aged 6 to 12 averaged about 29 percent lower. Exposure and hormone were measured at the same moment, so this is a correlation and cannot prove cause. The animal evidence is stronger.
Do microplastics cause heart attacks?
Unsettled, from a single 2024 study. Patients whose neck-artery plaque contained microplastics had about 4.5 times the combined rate of heart attack, stroke and death. People with plastics in their arteries may also differ in diet, pollution exposure and other ways the study could not fully account for.
Has industry downplayed plastic chemical harms?
The tally alone hides how the split happened: some industry studies discounted their own positive controls, or used designs unlikely to detect an effect. That also helps explain why regulation lagged the independent science for years. Where an additive is easy to avoid, lowering it does not depend on any one agency's timeline.
Do microplastics cause cancer in humans?
Not established. Whether the finished-plastic particles most people encounter change cancer risk is still unstudied. The known cancer link is to plastic building-block chemicals in industrial settings, such as vinyl chloride.
