What It Is

This practice is two moves that fit together. The first is filtering out the contaminants shown to cause harm, a short and specific list. The second, when you use a purifier that strips the water bare, is putting minerals back.

The contaminants worth targeting:

  • Lead leaches from old service lines, solder, and brass fixtures, and no exposure level has been shown to be safe.
  • PFAS, the forever chemicals, reach water from industrial discharge and firefighting foam.
  • Trihalomethanes and related disinfection byproducts form when chlorine reacts with organic matter in the water.
  • Arsenic and nitrate are mostly local problems: arsenic in some groundwater and private wells, nitrate downstream of farming.

Treated municipal water is one of the great public-health achievements, and chlorination prevents far more disease than its byproducts cause, so this is targeted removal, not fear of the tap. Filter for what is actually in your particular water. The second move follows from a side effect of the strongest filters: reverse osmosis and distillation take out the good dissolved minerals along with the bad, so water that pure benefits from a little calcium and magnesium added back.

Anatomy of the Practice

1What can be in tap water

Beyond the water itself, a supply may carry lead from plumbing, PFAS from industry, disinfection byproducts from chlorination, and, depending on geology and farming, arsenic and nitrate. A local water-quality report or a home test tells you which of these apply to you.

2Activated carbon filters

Carbon, in a pitcher, faucet or under-sink unit, adsorbs chlorine, many disinfection byproducts, some PFAS, and improves taste. It does less for dissolved metals and salts, so it is a strong general first step but not a complete answer for lead, arsenic or nitrate on its own. Filters certified to a recognized standard for a named contaminant are the ones to trust.

3Reverse osmosis

A reverse-osmosis membrane forces water through pores fine enough to reject lead, arsenic, nitrate, PFAS and most dissolved salts, so it handles nearly the whole contaminant list at once. The tradeoff is that it also strips calcium and magnesium and wastes some water, which is why remineralizing the output belongs in the plan.

4Distillation

Distillation boils water and condenses the vapor, leaving behind metals, salts and most nonvolatile contaminants. Like reverse osmosis it produces very pure, very low-mineral water, so the same remineralizing step applies. It is slower and energy-hungry, and it does not remove chemicals that boil off with the water.

How It Works

Spend the effort where the evidence is. Lead is the clearest case: a pooled analysis of children found that IQ falls with blood lead, and the loss per unit is steepest at the lowest levels, so no threshold has been shown to be safe. A home with old plumbing is worth testing, and if lead is present, filtering. The other targeted contaminants each act through their own mechanism, each has a specific filter, and each carries a graded finding below.

The remineralizing half rests on a separate line of evidence. Water that is naturally harder, richer in calcium and magnesium, tracks with lower cardiovascular death across decades of observational studies, and magnesium intake measured in blood or diet tracks with less heart disease. The causal evidence for that pattern comes from randomized trials, where magnesium lowers blood pressure a small amount. Reverse osmosis and distillation remove those dissolved minerals along with the contaminants, so the water they make is very pure and mineral-poor. Adding calcium and magnesium back restores what was stripped and moves intake in the direction the evidence favors. Water is a minor mineral source next to food, so this is a finishing step, not a treatment.

What Changed

The common belief is that with drinking water, purer is simply better, and that a good filter should get the water as clean as possible. For removing contaminants, that points the right way.

The mineral evidence refined it. A long run of observational studies, summarized in a systematic review, found that people drinking water higher in magnesium had lower cardiovascular mortality, with a pooled odds ratio of 0.75 for the magnesium and cardiovascular-death association. Reverse osmosis and distillation, the methods that make water purest, remove that magnesium along with the lead and arsenic. So the aim shifts from maximum purity to the right composition: take out what harms, keep or restore what helps.

This is not settled science. The water-hardness data are ecological and confounded by geography and diet, and the strongest causal evidence comes from magnesium supplement trials, not from water itself. Held at that strength, it still changes what a person does: filter for the contaminants that matter, and if you strip out the minerals, add a little calcium and magnesium back.

What the Evidence Shows

Two bodies of research sit under this practice, and they are not equally strong. Getting contaminants out is the strong, actionable half: removing lead, arsenic, disinfection byproducts, PFAS, and nitrate where they occur is what changes health, and each finding is graded on its own evidence below. The mineral half is softer. It rests on observational water-hardness data and on magnesium supplement trials, so restoring calcium and magnesium to reverse-osmosis or distilled water is a low-risk step that moves intake the right way, though the specific benefit of remineralizing water has not been shown on its own.

Lanphear and colleagues (Environ Health Perspect 2005) pooled individual data from seven birth or infancy cohorts followed to ages 5 to 10. Using a log-linear model, the IQ decrement for a blood-lead increase from 2.4 to 10 micrograms per deciliter was 3.9 points (95% CI 2.4 to 5.3), larger per unit than the loss between higher levels. The lead-associated decrement was significantly greater in children whose maximum blood lead was below 7.5 than in those above it (p = 0.015). Drinking water becomes a lead source where old service lines, solder or brass fixtures corrode, which is why a home with pre-1986 plumbing is worth testing.

The Chinese Medicine View

Water sits close to the root of this tradition. It is one of the Five Phases, and it is the phase of the Kidney, which stores what the classics call Jing, the deep essence a person is born with and slowly spends across a life. The Kidney governs the pure fluids of the body and the water pathways that move and transform them. Clean water, in this framing, is not a small matter of taste; it is the raw material the body works with to make and maintain those fluids, and the quality of what comes in day after day is understood to shape the terrain over years.

Two ideas connect this practice to the tradition. First, the classical view treats clean food and clean water as foundational, part of the plain daily care that protects the body from what it cannot easily transform. The Spleen and Stomach receive and separate the clear from the turbid in everything we take in, and a supply loaded with what the body must work to clear adds, in this reading, to the turbid load. Removing the contaminants themselves lightens that load.

Second, the tradition holds that a well-founded constitution and preserved Jing depend on complete nourishment, and mineral-bearing water suits that view better than water stripped of its minerals. This is a lens, not a measurement, and it does not mean the old texts anticipated reverse osmosis or knew of arsenic. A system built on long observation and a modern one built on epidemiology arrive at the same plain conclusion: take in clean water, and do not strip it of everything. A practitioner would still fit the detail to the person, favoring warm over iced water for someone who runs cold or digests weakly.

Hanna-Attisha and colleagues (Am J Public Health 2016) compared elevated blood-lead incidence in children younger than five before (2013) and after (2015) the water-source change, using spatial analysis to map risk. Incidence rose citywide from 2.4 to 4.9 percent and to a 6.6 percent increase in high-water-lead neighborhoods, concentrated in socioeconomically disadvantaged areas, while no significant change occurred outside Flint. Know your local water and lead risk

The right action depends entirely on what is in your supply. A home with plumbing from before lead solder was restricted, or a private well, deserves a direct test for lead, and wells also for arsenic and nitrate, since these are odorless and tasteless and a utility report will not cover a private well. Match any filter to the specific contaminant and pick one certified for it by name.

Reverse osmosis and distillation strip minerals, so remineralize

These methods remove calcium and magnesium along with contaminants, leaving very low-mineral water. For most people the mineral loss is minor against a normal diet, but if reverse-osmosis or distilled water is your main drinking water, add calcium and magnesium back with a remineralizing cartridge, mineral drops, or a small pinch of a mineral-rich salt. Water is a contributor to mineral intake, not the main source.

Distilled-only water over the long term

Relying on distilled or very pure water as essentially all of your intake, with a diet also low in minerals, means less calcium and magnesium coming in overall. Chasing ever-greater purity past the point where the contaminants are handled adds cost and effort without a matching gain.
