What It Is
Clean water means filtering out the contaminants a local test actually finds, then adding calcium and magnesium back if the filter strips them out. Only a short list carries real evidence, so most of the work is finding out which of them apply to you. Most are not natural to water: PFAS come from industrial discharge, nitrate from farm-fertilizer runoff, disinfection byproducts from chlorination itself, and lead from aging pipes. Treated municipal water is a public-health achievement, and chlorination prevents far more disease than its byproducts cause. The job is to take out what a test actually detects.
What the Evidence Shows
A filter matched to a named contaminant removes it, and the harm each contaminant does is well established. That is the solid half. The mineral case is weaker, and it rests on indirect evidence; remineralized water has never been tested on its own.

Lead lowers children's IQ about 6.9 points, with no safe threshold
Lead lowers children's IQ, and the drop per unit of lead is largest at the lowest levels, so there is no amount that has been shown to be safe. Old lead service lines and plumbing are one avoidable source.
Lanphear and colleagues (Environ Health Perspect 2005) pooled individual data from seven birth or infancy cohorts followed to ages 5 to 10. Using a log-linear model, the IQ decrement for a blood-lead increase from 2.4 to 10 micrograms per deciliter was 3.9 points (95% CI 2.4 to 5.3), larger per unit than the loss between higher levels. The lead-associated decrement was significantly greater in children whose maximum blood lead was below 7.5 than in those above it (p = 0.015). Drinking water becomes a lead source where old service lines, solder or brass fixtures corrode, which is why a home with pre-1986 plumbing is worth testing.

Environmental Exposure
PFAS in water and higher LDL cholesterol, about 5 percent per halving of exposure
The forever chemicals that get into water, PFOA and PFOS, are linked to higher LDL cholesterol, and cholesterol fell as blood levels of these chemicals dropped. This overlaps with the plastics story covered on the microplastics page.
Fitz-Simon and colleagues (Epidemiology 2013) used a longitudinal design, less prone to confounding and reverse causation than earlier cross-sectional work, following the same people as exposure declined after contamination was reduced. The within-person association between falling PFAS and falling LDL supports an effect on lipids. PFAS reach drinking water from industrial discharge and firefighting foam, and activated-carbon and reverse-osmosis filters both reduce them.

More magnesium, in blood or diet, and up to 30 percent less heart disease
Getting more magnesium, whether measured in the blood or the diet, is linked to lower rates of heart disease. This is part of why magnesium is the mineral most worth restoring to stripped water.
Del Gobbo and colleagues (Am J Clin Nutr 2013) systematically pooled prospective cohorts with dose-response meta-regression. The dietary association with fatal ischemic heart disease was nonlinear, with benefit up to roughly 250 milligrams per day. The authors called for randomized trials to test causation, which the blood-pressure trial literature has begun to address. Water is a small but real magnesium source, which is the link to remineralization.

Magnesium supplements cut systolic blood pressure about 2 mmHg in randomized trials
In randomized trials, extra magnesium lowered blood pressure by a small amount. This is the controlled evidence behind treating magnesium as worth restoring, since it points to cause rather than just association.
Zhang and colleagues (Hypertension 2016) synthesized only randomized double-blind placebo-controlled trials, the design that best supports causation. A restricted cubic spline suggested a dose of about 300 milligrams per day or one month was enough to raise serum magnesium and lower blood pressure. The effect is modest at a population level but consistent, and it is why adding a magnesium source back to demineralized water is a sensible, low-risk step for most people.

Magnesium-rich water and about 25 percent lower cardiovascular death
People whose water is naturally higher in magnesium tend to have somewhat lower rates of dying from heart disease. It is a long-observed association from ecological and observational data, not a controlled trial, so it cannot prove the water is the cause.
Catling and colleagues (J Water Health 2008) screened 2,906 papers and included 14 (nine case-control, five cohort). The pooled inverse association for magnesium was significant among case-control studies; heterogeneity made a calcium summary inappropriate, and the cohort evidence was weaker. This is the classic soft-water, hard-water cardiovascular literature: consistent and long-standing, but ecological in nature, which is why it is graded emerging and the confound is named.

How it works
Reverse osmosis and distillation strip water's calcium and magnesium
The same processes that make water very pure, reverse osmosis and distillation, also remove the calcium and magnesium that harder water carries. That is the mechanical reason remineralizing stripped water makes sense.
Sengupta (Int J Prev Med 2013) reviews five decades of evidence linking water hardness, and specifically its magnesium and calcium content, to mortality and other health measures, and notes that water can be a meaningful contributor to mineral intake depending on local hardness. Reverse-osmosis membranes and distillation both remove dissolved ions indiscriminately, which is what makes the product water low in minerals. This describes a mechanism and an intake pathway, not proving a clinical outcome, so it is graded emerging with no single direction of effect.

How It Works
Each targeted contaminant harms through its own route. Lead is a neurotoxin that accumulates in the body and competes with calcium in developing nerves, so no exposure level has been shown to be safe. PFAS persist because the carbon-fluorine bond resists breakdown, which is why they are called forever chemicals. Disinfection byproducts form when chlorine reacts with organic matter in the water. A filter rated for the named contaminant physically traps or excludes it.
The mineral half works mainly through magnesium. Magnesium relaxes the smooth muscle in blood-vessel walls, one route by which higher intake lowers blood pressure. Calcium and magnesium dissolved in water are absorbed much as they are from food. When reverse osmosis or distillation takes them out, adding a little back keeps daily intake up.
What Changed
The common belief is that purer water is simply better, and that a good filter should get it as clean as possible. For removing contaminants, that points the right way. The mineral evidence refined it. The methods that make water purest, reverse osmosis and distillation, also strip out the magnesium that mineral-rich water is valued for. So the aim shifts from maximum purity to the right composition: take out what harms, keep or add back what helps.
This is not settled science. The water-hardness data are ecological, confounded by geography and diet. The strongest causal evidence comes from magnesium supplement trials, where the mineral was given directly. Held at that strength, it still changes what a person does.
How the Filters Work
The filter you need depends on what your water carries. Only reverse osmosis and distillation force the extra remineralizing step; carbon leaves the minerals in. Carbon, reverse osmosis and distillation remove progressively more.
Anatomy of the Practice
1Find out what is in your water
Before choosing a filter, find out what is in your water. A supply may carry lead, PFAS, chlorination byproducts, arsenic, or nitrate, depending on plumbing, industry, geology, and farming. A local water-quality report or a home test shows what your supply actually contains.
2Activated carbon filters
Carbon, in a pitcher, faucet, or under-sink unit, adsorbs chlorine, many disinfection byproducts, some PFAS, and improves taste. It does less for dissolved metals and salts. Carbon is a good general first step; lead, arsenic, and nitrate need a stronger method.
3Reverse osmosis
A reverse-osmosis membrane forces water through pores fine enough to reject lead, arsenic, nitrate, PFAS, and most dissolved salts, covering nearly the whole list at once. It also strips calcium and magnesium and wastes some water. That is why remineralizing the output belongs in the plan.
4Distillation
Distillation boils water and condenses the vapor, leaving behind metals, salts, and most nonvolatile contaminants. Like reverse osmosis it produces very pure, low-mineral water, so the same remineralizing step applies. It is slower and uses a lot of energy, and it does not remove chemicals that boil off with the water.
Test what comes out of your tap, match the filter to what is actually in it, and put minerals back only if your filter strips them out.
Ways to Do It
This is mostly a one-time setup plus a scheduled filter change. Find out what is in your water, match the tool to it, and add the remineralizing step only if your method strips the minerals out.
Read your utility annual water-quality report; it is free and lists what has been detected. With old plumbing or a private well, add a certified home test kit for lead. A private well is not covered by utility testing, so test it for arsenic and nitrate directly. The report and test together tell you what filter you need.
Pick a filter certified for the specific contaminant you are removing. Activated carbon handles chlorine, taste, and many disinfection byproducts. Lead, arsenic, nitrate, and PFAS need a unit rated for each by name, often reverse osmosis or a specific adsorptive media. Look for certification against the named contaminant.
On a well-run municipal supply with no lead plumbing or well-specific issue, a certified carbon pitcher or under-sink filter handles chlorine byproducts and taste. Change it on schedule; the cost and effort stay low. It leaves the mineral content intact, so no remineralizing step is needed.
When the water carries lead, arsenic, nitrate, or PFAS, reverse osmosis or distillation covers them in one unit. Both produce very low-mineral water, so remineralizing applies. Change reverse-osmosis membranes and pre-filters on the maker schedule so performance holds.
Add calcium and magnesium back with an inline remineralizing cartridge on a reverse-osmosis system, mineral drops, or a small pinch of a mineral-rich salt in a jug. This restores what the process removed and improves the flat taste of very pure water. Food supplies most of your minerals, so treat this as a small addition.
Store filtered water in glass, steel, or ceramic to limit plastic-chemical exposure. Keep bottles clean and refrigerate once opened, since remineralized water can grow microbes. Replace cartridges on time, since a spent filter can release what it collected.

The Chinese Medicine View
Water sits near the root of this tradition. It is one of the Five Phases, and it belongs to the Kidney. The Kidney stores what the classics call Jing, the deep essence a person is born with and spends slowly across a life. The Kidney also governs the body's pure fluids and the water pathways that move and transform them. Clean water, in this view, is the raw material the body works with to make and maintain those fluids. The quality of what comes in, day after day, is understood to shape the constitution over years.
The Spleen and Stomach receive what we take in and separate the clear from the turbid. A supply loaded with what the body must work to clear adds, in this reading, to the turbid load. Removing the contaminants lightens that load. The tradition also holds that a well-founded constitution and preserved Jing depend on complete nourishment. Mineral-bearing water suits that view better than water stripped of its minerals.
None of this is a measurement. It is a way of seeing, and the old texts did not anticipate reverse osmosis or know of arsenic. A tradition built on long observation and a modern field of epidemiology reach the same plain point: take in clean water, and keep some minerals in it. A practitioner would still fit the detail to the person, favoring warm water over iced for someone who runs cold or digests weakly.

Corrosive water in Flint roughly doubled young children's high blood lead, 2.4 to 4.9 percent
Hanna-Attisha and colleagues (Am J Public Health 2016) compared elevated blood-lead incidence in children younger than five before (2013) and after (2015) the water-source change, using spatial analysis to map risk. Incidence rose citywide from 2.4 to 4.9 percent and to a 6.6 percent increase in high-water-lead neighborhoods, concentrated in socioeconomically disadvantaged areas, while no significant change occurred outside Flint. High-arsenic well water and a 68 percent higher death rate
Argos and colleagues (Lancet 2010) recruited a population-based cohort from Araihazar, Bangladesh, with in-person clinical assessment and biennial follow-up, using Cox models adjusted for potential confounders. A dose-response was evident at the highest exposures. Arsenic in drinking water is a global problem affecting tens of millions; a review by Naujokas and colleagues (2013) catalogs the broad range of associated cancers and chronic diseases. Villanueva and colleagues (Epidemiology 2004) pooled primary data from studies in the US, Canada, France, Italy and Finland, all with individual water-consumption and long-term trihalomethane exposure estimates. Risk rose with cumulative exposure in men across several exposure indices; the absence of association in women is unexplained and may reflect differences in water intake or susceptibility. Nitrate in water and about 16 percent higher colorectal cancer, even below the legal limit
Schullehner and colleagues (Int J Cancer 2018) exploited Danish registers linked in space and time to longitudinal water-quality data from 1978 to 2011, giving high-quality long-term individual exposure. Nitrate can be converted endogenously into carcinogenic N-nitroso compounds, the proposed mechanism. Know your local water and lead risk
The right action depends entirely on what is in your supply. Older homes can have lead in service lines, solder, or brass fixtures, so their water should be tested for lead directly. Lead, arsenic, and nitrate are odorless and tasteless, so a test is the only way to find them. Match any filter to the specific contaminant.
Reverse osmosis and distillation strip minerals, so remineralize
Reverse osmosis and distillation remove calcium and magnesium along with contaminants, leaving very low-mineral water. For most people on a normal diet, that loss is minor. If reverse-osmosis or distilled water is your main drinking water, add calcium and magnesium back.
Distilled-only water over the long term
Relying on distilled or very pure water as nearly all of your intake, alongside a mineral-poor diet, means less calcium and magnesium coming in overall. It is easy to offset by remineralizing or by a mineral-adequate diet. Anyone managing a condition affected by mineral or electrolyte balance should check the plan with their clinician.
Keep it in proportion
Municipal water is treated and monitored, so the tap is safe to drink. The goal is to remove what a local test finds. Once a filter certified for those contaminants is in place, further treatment targets things the test did not detect.

Common Questions
Do I really need a filter if my utility water meets safety standards?
Maybe. Legal limits are set above zero, and for lead there is no level shown to be safe. If you are on a well-run municipal supply with no lead plumbing and no local arsenic or nitrate issue, a certified carbon filter handles chlorine byproducts and taste. That is a reasonable, low-cost step. Old lead service lines, or a private well, change the answer, so test your own tap first.
Do pitcher and fridge filters remove PFAS and lead?
Some do, many do not. Most basic pitcher and fridge filters are activated carbon aimed at chlorine and taste, and their PFAS and lead removal varies widely by product. If lead or PFAS is your concern, choose a model specifically certified to reduce it, or step up to reverse osmosis. Check the certification, since the pitcher on the shelf may not be rated for either.
Does the water I shower and bathe in matter?
For the evidence-backed contaminants, drinking water is the main route, so a drinking-water filter covers most of the concern. Chlorine and its byproducts can be inhaled or absorbed during long hot showers. Some people add a carbon shower filter for that, though the health case is thin. Lead and PFAS matter mainly through what you swallow, so bathing water is a lower priority than drinking water.
Is reverse-osmosis or distilled water bad for you?
It is not harmful in the way the phrase suggests. It is simply stripped of calcium and magnesium along with the contaminants, so very pure water is worth adding minerals back to. For most people the mineral loss is minor next to a normal diet. It matters mainly if reverse-osmosis or distilled water is nearly all you drink.
Is water with minerals added back as good as naturally mineral-rich water?
The aim of adding calcium and magnesium back is to approximate naturally hard water, the kind linked to better heart outcomes. That exact move, remineralizing and then tracking a health outcome, has not been studied directly. For the heart specifically, the stronger direct evidence is for magnesium taken as a supplement (see Magnesium).
